5.Fibrous Hamartoma of Infancy : Report of A Case.

We report a case of fibrous hamartoma of infancy in a 7-month-old male infant, in which a solitary, asymptomatic, walnut-sized, nomal skin-colored plaque occurred on the left middle back for about one month. Histopathologic findings showed the distinctive features of fibrous hamartoma of infancy, ie, trabeculae of dense collagenous tissue, whorls of immature spindle cells within a mucoid matrix, and interspersed mature adipose tissue, in the lower dermis. The lesion was excised without recurrence.

6.A Case of Keratoderma Palmoplantaris Striata et Marginata.

We herein report a case of keratoderma showing the combined clinical manifestations of striate and marginal keratoderma. The patient was a 18-year-old giri who about a year ago noticed slightly yellowish, well-circurnscribed, hyperkeratotic plaques on the volar aspect of the fingers and palms of both hands in a linear pattern with bands of hyperkeratotic plaques at the edeges of the 2nd fingers. We also found hyperkeratotic plaques on the both soles which were confined to sites of pressure. She denied a history of any chronic pressure or trauma. There was no family member showing the same clinical features. The histopathological findings from the palm, volsr aspect of finger and a sole revealed marked hyperkeratosis, hypergranulosis, and acanthosis.

7.A Case of Vulvitis Circumscripta Plasmacellularis.

We present a 40-year-old woman who had a well circumscribed, pea sized red-brown glistening erosive papule on the foreskin of clitoris for four months which had been resistant to various forms of treatment. Histopathologic examination revealed flattened epidermis with lozenge keratinocytes and watery spongiosis, In the upper dermis, dense bandlike infiltration of plasma cells, endothelial proliferation, and capillary dilatation were noted. Peroxidase-antiperoxidase stain showed polyclonal pattern of reaction in the cytoplasm of plasma cells.

8.Effect of Radioactive Isotope 131I on Epiphyseal Plate of Tibia in Mice

The purpose of this investigation was to study the changes in the epiphyseal plate of growing mice by histological, histochemical and electron microscopic observation through the administration of radioactive isotope I which induced hypothyroidism. To confirm hypothyroidism induced, radioimmunoassay for triiodothyronine was carried out and the morphologic changes of thyroid gland were observed by a light microscope. Of the total of eighty ICR strain of mice, fifty received single intraperitoneal injection of 20ue/gm of radioactive isotope I which were dissolved in 0.2ml of saline. Thirty received single intraperitoneal injection of only 0.2ml of saline and these mice were used as control groups. At the end of first, second, third, fourth, sixth and eighth week after injection, both saline injected and I treated mice were sacrifised. The thyroid gland and epiphyseal plate being removed were stained with hematoxylin and eosin, alcian blue and with uranyl acetate lead citrate in each for histological, histochemical and electron microscopic studies, The results obtained are as follows: 1. The thickness of epiphyseal plate was narrowed in the I treated animals. In each zone of epiphyseal plate the cell size was reduced although the cell numbers of each cell column were not changed. This means that the reduction of thickness in the epiphyseal plate was due mainly to decreased cell size and scanty territorial matrix not to decreased cell number. 2. Alcianophility was markedly decreased ln proliferative zone, indicating that acid mucopolysaccharide diminished markedly or was absent in the proliferative zone. 3. In electron microscopic examination, the number of rough surfaced endoplasmic reticulum was decreased and their cisternae were narrowed. Decreasing tendency of dilatation and sacculation of cisternae, .reduced number of attached ribosomes on the endoplasmic reticulum, and atrophy of the Golgi apparatus with diminished number of its vesicles were noted, However glycogen particlea were accumulated. 4. The number and length of each trabeculae in the metaphysis just adjacent to the epiphyseal cartilage plate were reduced markedly, and occasionally chondrocytes which were not consumed by endochondral ossification were observed in the cartilage lattice.

9.Total Hip Replacement in Dysplastic Hips.

The dysplastic hip presents problems to the orthopedic surgeon in the reconstruction of hip using total hip prosthesis. The use of bone grafts to reconstruct the lateral acetabular bone deficiency is very popular. We investigated in 23 patients(5 cemented cup and 18 cementless cup). Sixteen patients were women and seven were men, whose average age was 42.4 years old; their average followup was 4.6 years. The average Harris hip score improved from a preoperative value of 48 points to a postoperative value of 87 points. Five cases showed radiographic loosening, in which 3 cases loosening from five cemented cup and 2 cases from cementless cup arthroplasty. Radiolucent lines were observed more frequently with cemented cups at last follow-up. Of the 23 cases, 18(78%) was successful. Postoperatively limping was a major problem because most patients in this series had unilateal disease. The grafted bone was incoporated by an average time of 7.4 months, and minor resorption was showed at last follow-up. In our series, cementless cups with autogenous bone graft noted good results than cemented cup arthroplasty.

10.Endoscopic Posterior Cruciate Ligament Reconstruction Using a Quadrupled Hamstring Graft and Endobutton(r): Preliminary Report of a New Technique.

We present our technique for reconstruction of the posterior cruciate ligament (PCL) using the quadrupled hamstring tendons with EndoButton(Acufex Microsurgical, Mansfield, MA). This paper describes the surgical technique of the procedures and reports the preliminary results in 10 patients with a more than 12 months follow-up. There were 8 males and 2 females ranging in age from 17 to 37 years (average 25 years). The average time from injury to operation was 3 weeks (range; 2 weeks to 4 weeks). Of ten patients, 6 were classified as an isolated tear, 2 were associated with lateral collateral ligament tears. A torn meniscus was present in 2 cases. We used one-incision technique. The results of these PCL reconstructions with a minimum follow-up of 12 months are as follows. Patients were evaluated preand postoperatively using the Lysholm, and Hospital for Special Surgery Knee ligament rating scales. Average knee ligament evaluation scores were Lysholm preoperative 48, postoperative 87.6; and Hospital for Special Surgery preoperatively 33.5, postoperative 88.8. We chose the semitendinosus and gracilis tendons with EndoButton fixation for PCL reconstruction for specific reasons: (1) No injury to the extensor mechanism and (2) Easy passage of the graft through the tibial tunnel. (3) The problem about the screw and patellar bone-tendon-bone can be resolved. (4) It preserves intact meniscofemoral ligament, some intact fibers of PCL bundle. (5) When rupture of the PCL is associated with ACL tear, each ligament should be reconstructed. In such a case, we preserve patellar tendon for reconstruction of the ACL, The technique presented here is not applicable to all individuals requiring PCL reconstruction but does provide significant advantages to those patients for whom it is utilized. Although the clinical results are very promising at this length of follow up, long-term study is required to evaluate our PCL reconstruction procedure more precisely.
