3.A Clinical Observation of Cutaneous Malignant Tumors.

A clinical study was made on 148 cases of cutaneous malignant tumors that visited the Department of Dermatology at Seoul National University Hospital during a 11-year period from 1973 to 1983. The results were summarized as follows: 1) The incidence of cutaneous malignant tumors in the total number of outpatients visiting the Department of Derrnatology was 0. 025g. 2) There were 90 cases of male (60. 8p,) and 58 cases of female (39. 2g). The ratio of male to female was l. 55: l. Among the 148 cases of cutaneous malignant tumors, the most frequently observed type was basal cell carcinoma, 58 cases (3j.2 p:). The next most frequent types were squamous cell carcinoma, 42 cases(28. 4g) and malignant melanoma, ]1 cases (7. 4g). We described the clinical characteristics of the patients with basal cell carcinoma, squamous cell carcinoma and malignant melanoma that have been observed.

4.Clinical Study of Hospitalized Patients with Drug Eruption During a 10-Year Period (1976~1985).

The study comprises 127 inpatients with drug eruption, treated at the Department of Dermatology, Seoul National University Hospital, during a 10-year period. The results are summarized as follows: 1. Out of 1,434 dermatologic inpatients, 127(8. 9%) patients were diagnosed as drug eruption. 2. The cutaneous manifestations of drug eruptions in the order of frequency were as follows: exanthematous eruption, urticaria, erythema multiforme, Stevens Johnson syndrome, TEN, exfoliative dermatitis, fixed drug eruption and purpura. 3. Antibiotics and antimicrobials were the most common causative agents followed by antipyretics and analgesics, CNS depressant drugs and herb drugs. 4. The 5 most common drugs causing drug eruptions were ampicillin, acetyl salicylic acid, diphenylhydantoin, sulfonamide and phenacetin.

5.A Case of Estrogen Dermatitis.

Sensitivity to estrogen has been described previously. The clinical picture is varied with pruritus, either generalized or localued or as urticaria. The hallmark of estrogen dermatitis is the cyclic premenstrual flare. The patient reported here had cyclic erythema multiforme-like lesions and showed a positive intradermal skin test to estrogen. A 20-year-old female patient presented with a periodic 5 year duration of skin lesions on both hands. The eruption commenced in the second half of the menstrual cycle, worsened through the luteal phase but the lesion almost disappeared during menstruation. An intradermal skin test to estrone showed positive results. However, a skin test with medroxyprogesterone acetate was negative. After systemic steroid and antihistamine therapy, the lesions were found to be significantly improved.

6.A Case of Lupus Pernio.

A 36-year-old female patient with sarcoidosis is presented showing lupus pernio, papular skin lesions, cervical lymph node enlargement and lung lesions. Diagnosis was confirmed by characteristic histologic findings of noncaseating granulomas in skin and cervical lymph node, supported by compatible clinical and laboratory features. Methatrexate was administered orally, which resulted in considerable improvement of skin lesions.

7.Selective angiography of Hip in Avascular Necrosis of Femoral Head

The diagnosis and treatment of avascular necrosis of femoral head advanced, after Freud issued examples of bilateral avascular necrosis of femoral head (AVN) in 1926. But still not fixed in treatment and diagnosis. Recently early diagnosis and treatment produced good result. Whole body bone scan, intraosseous pressure mornitoring, intramedullary venography, C-T, or MRI, Selective femoral angiography also used in early diagnosis. Selective femoral angiography was performed for 56 patients, who has suspected as AVN by sumple X-ray and WBBS in order to estimate. How the femoral angiographic finding exist in AVN. In the control group femoral angiography performed to 5 person with normal stage of clinical and radiologic finding. So the result are as following l. All cases in the control group were not observed terminal branch of superior and inferior capslar branch. 2. In AVN the alteration of vasculature appeared 22 patients among 56 patient. 3. The altered vasculature presented 87.5% in traumatic group and 31.5% in nontraumatic group. 4. Collateral circulation was 22.2% in the cases of patients under 6 moths of the time interual between etiological events and clinical symptoms, 89.5% over 6 months. 5. Collateral circulation received 86.8% from inferior gluteal artery two cases from superior gluteal artery, and three cases form obturator artery. 6. Clinical tolerance was supposed to in well developed collateral circulation. According to the above results, Vascular alteraion were noted very much in traumatic group and development of collateral circulation were related promote clinical tolerance. Selective femoral angiography was not effective to assist early diagnosis and treatment of AVN for it could not detect terminal branch of superior and inferior capsular artery that reflected intraossous vasculsture. We need the new nethod of angoigraphy in order to early diagnosis.

8.Eccrine Angiomatous Hamartomam: Report of Case.

Progressive cribriform and zosteriform hyperpigmentation (PCZH) is a distinctive clinical entity first described by Rower et al. in 1978. It is characterized by a single area of uniformly tan cribriform macular pigmentation in a zosteriform distriggtjpp with no preceding history of skin rash, injury or inflammation. It begins well after birth with gradual extension and is not associatad with any other cutaneous or internal abnormalities. Histologically, there is a mild increase in melanin pigment in the basal cell layer and complete absence of nevus cells. Recently, we observed clinical and histological featurea of 5 cases of PCZH. The age at onset ranged from 4 to 35 years. The lesions were unilaterally distributed on face, neck, chest, or abdomen. Histologically there was a mild increase in melanin pigment in the basal cell layer. PCZH must be differentiated from Beckers hairy nevus which begins with a unilateral lesion of irregularly macular pigmentation but occasionally it may be multiple or bilateral. In early stage, overgrowth of hairs may be absent but after a few years it is usually present. PCZH is characterized by uniformly tan cribriform pigmentation in zosteriform distribution, which is unilateral and does not cross the midline of the body. Overgrowth of hairs is absent even after a few years. So the lesions of Beckers hairy nevus previously reported, which were unilateral and did not cross the midline of the body and did not show overgrowth of hairs even after a few years, must be called progressive cribriform and zosteriforrn hyperpigmentation.

9.Immunologic Effect of Gamma Interferon on Human Melanoma Cell Lime A - 375 - With Special Emphasis on Cytolytic Activity , Antiproliferative Activity and HLA Antigen Expression.

Interferon preparations, in addition to their antiviral properties, may inhibit cell growth and multiplication, enhance the expression of cell surface antigens and influence some functions of T-1ymphocytes, macrophages and natural killer cells. The study was performed in order to investigate the cytolytic effect, antiproli ferative effect and HLA antigen expression effect of the gamma-interferon(IFN- r ) on human melanoma cell line A-375. the cytolytic activities were checked by 'Cr release assay, the antiproliferative activities were analyzed by the 'H-thymidine uptake test and HLA antigen expressions were observed by the indirect imrnuno fluor esent method. The results were as follows : 1. The cytolytic effect of the peripheral rnononuclea.r cells treated with INF- on A 375 human melanoma cell line was increased to 31.4%, 36.5%, 33.9%, 53.9 %, 13.9% respectively in the experiment 1, 2, 3, 4, 5 as compared to control group (-11.4%, 3.6%, 18.5%, 35.5%, 6.7% respectively). 2. 1ri the dose response check of the peripheral mononuclear cells treated with INF r to A 375, the cytolytic effect was definitely observed at 200U/ml concentration of the INF r and maximal effect was observed at 500U/ml concentration. 3. The IIUF r treated natural killer cells did not show any significant increase in cytolytie activity as compared to that of untreated natural killer cell and the same results were obtained with monocytes. This cytolytic activity was significantly increased when INF-r treated natural killer cells were co-culturd with monoeytes. 4. The cyaolytjc activity of natural killer cells was increased at the same degree when using culture supernant of INF- treated monocytes as in the case of using

10.Statistical Study on Optimal Placement of Immediate Dental Implantation Using Denta Scanr.

PURPOSE : The purpose of this study was to determine proper position and angulation of an implant for immediate implantation. MATERIALS AND METHOD : From the years 1997 to 2000, 52 Denta scanR views, 22 upper and 32 lower jaw with an average age of 43 and 40 respectively, were investigated, which comprise intact upper and lower 6 anterior teeth and premolars. on the Denta scanR, the optimal placement for the immediated implantation was simulated. The measuring methods included. 1) Angulation difference between tooth long axis and alveolar bone process. 2) Angulation difference of long axis between tooth and installing fixture 3) Distance between center of tooth at cervical area and center of fixture. 4) Distance from root apex to the bone limit of vital structure. One sample t-test was used for statistical analysis. RESULT : The results were as follows. 1) At the maxillary central incisor and lateral incisor, angulation difference of long axis between tooth and installing fixture was respectively 0.5 and 3.2degrees with the fixture center's palatally positioned 2mm apart from tooth center. 2) At the lower anterior 6 teeth, that was about -2.8degree to -4.6degree with the fixture center's lingually positioned 1mm apart from tooth center. 3) At the maxillary canine and premolar, that was respectively 11.8degree and 7.2degree with the fixture center palatally positioned 2~2.4mm apart from tooth center. 4) At the lower premolar area, that was about 0degree to 2degree with the fixture center's lingually positioned 0.5~1mm apart from tooth center. 5) Distance from root apex to the bone limit of vital structure, at the maxillary anterior and premolars, was the range of 10 to 12mm, and at the mandibular anterior teeth and the 1st premolar, that was the range of 18 to 20mm. CONCLUSION : The proper implant position of maxillary anterior and premolar teeth is as paralleled as or more buccally angulated than long axis of tooth with the fixture center's palatally positioned. in mandibular anterior region, long axis of implants is lingully angulated compared with compared with long axis of tooth and in premolar, almost parelleled with long axis of tooth and alveolar process.
