2.Internal Fixation Using Clavicle Hook Plates for Distal Clavicle Fractures.

BACKGROUND: To report the radiological and clinical outcomes of internal fixation using distal clavicle hook plates for distal clavicle fractures. METHODS: From April 2008 to December 2012, 32 patients with distal clavicle fractures underwent surgery using an AO hook plate. The reduction was qualified and evaluated according to the radiological findings. The evaluation of the clinical outcomes was performed with the University of California at Los Angeles (UCLA) score, the Korean Shoulder score, and the visual analogue scale (VAS) pain score. RESULTS: By radiological evaluation, we found that 31 of 32 patients showed anatomical reduction and solid bone union. Although we obtained satisfactory UCLA scores, Korean Shoulder Scale scores, and VAS pain scores, 12 cases of complications were present. We found 4 cases of osteolysis of the acromion, 1 case of nonunion, 3 cases of periprosthetic fractures, 3 cases of subacromial pain, and 1 case of skin irritation. We performed re-operations in 2 patients. CONCLUSIONS: To avoid complications associated with clavicle hook plates, choosing the appropriate hook size and bending of the hook according to the slope of the acromion undersurface is critical. Also, we believe that early removal of clavicle plates may help reduce complications.

3.L-NAME, a Non Selective Nitric Oxide Synthase, Affects Stress-Related Behaviors and Glial Cell-Derived Neurotrophic Factor Expression in Restrained Adolescent ICR Mice.

OBJECTIVE: Depending on genetic or environmental effects over adolescent development, typical behavioral responses come out in adolescence. Also, alteration of nitric oxide (NO) levels in the brain has been associated with modifications of stress related behavior. Present study was designed to investigate the possible influence of chronic stress from restraint on the generation of depression in adolescent mice, and also to evaluate whether NO has modulatory roles in the behavioral and biological reactions. METHODS: ICR mice exposed to stressful restraint, 2 h per day, was treated with NG-nitro L-arginine methyl ester (L-NAME) (10 mg/kg), a non-selective NO synthase (NOS) inhibitor. To evaluate depression-like behavior in the mice, forced swim test and open field test were performed after the last restraint. To investigate stress-induced changes in the expression level of glial cell-derived neurotrophic factor (GDNF), free-floating immunohistochemistry was performed. RESULTS: The results showed that stressed group has longer immobility time and less crossing number in forced swimming and open field test, and that these stress responses were significantly prevented by L-NAME. Furthermore, decreased GDNF expression in the hippocampus by stress was prevented to that of controls within the L-NAME treated group. CONCLUSION: The results suggest that stress and NO signaling could be involved in generation of depression in adolescence. It also suggested that GDNF might contribute to prevent stress-related behaviors.

Traumatic peroneal tendon dislocation in association with fracture of the lateral process of the talus is a rare injury, which is difficult to diagnose. As a result, early detection is often delayed, which in turn leads to ankle pain and dysfunction. We treated a patient by open reduction and screw fixation in fracture of the lateral process of talus and primary repair of the superior peroneal retinaculum. We report this case with a brief review of the literature.

5.A histopathologic study of congential melanocytic nevi.

BACKGROUND: since congenital nevi may not always be identified clirically, it remains a challange for histologists to separate an acquired from a congehital nevus. The camparative histologic feature of congenital and acquired lesions have been described by several authors. OBJECTIVE: In an attempt to establish reliable microscopic recognition of congenital nevi, the histologic features of 52 congenital melanocytic nevi were studied. MATERIAL AND METHODS: By reviewing HMB slides, histologic paterns were observed depending on the extent of nevus cell infiltration. An immunohistochemical study wi h anti S-100 protein and HMB-45 antibody was also performed. RESULTS: Diffuse infiltratien of upper and lower dermis with nevu. cells was observed in 21 cases (40.0%). Nevus cell infiltration of appendages and neurovascular structures was observed in 29 cases (55.8%). Indian filing of dermal nevus cells was observed in 33 casesl(64.7%). Epidermis showed elongation of rete ridges in 36 ca.es(69.2%). Immunohistochemical staining with HMB 45 showed a positive raction in 9 cases out of 51 having dermal nevus cells, and epidemal melanocytes showed positive reaction in 20 cases. CONCLUSION: Congenital melanocytie nevi can be classified by histolcgic appearance into several patterns. Nevus cells had a trend to infiltrate into the deeper dermis as the clinieal size increrased. Nevus cell infiltration of appendages and neurovascular structure was a common finding in congenital melanocytic nevi.

6.Prepuncture Ultrasound Examination Facilitates Safe and Accurate Common Femoral Artery Access for Transfemoral Cerebral Angiography.

OBJECTIVE: We aimed to introduce our method involving prepuncture ultrasound scan for cannulation of the common femoral artery (CFA) during transfemoral cerebral angiography (TFCA), and to assess the clinical and radiological outcomes. MATERIAL AND METHODS: Our study included 90 patients who underwent prepuncture ultrasound examination of the inguinal area for TFCA between April 2015 and June 2015. Prior to skin preparation and draping of the inguinal area, we identified the CFA and its bifurcation using ultrasound. Based on the ultrasound findings, we marked cruciate lines in the inguinal area. Thereafter, we inserted a puncture needle at the interface between the horizontal and vertical lines at a 30–45° angle, simultaneously palpating the pulsation of the femoral artery. After TFCA was completed, femoral artery angiography was performed in the anteroposterior and oblique directions. Clinical and radiological parameters, including CFA cannulation, the ultrasound scan time, the first pass success rate, the time required for the passage of the wire, and complications, were evaluated. RESULTS: The mean ultrasound scan time of the CFA and its bifurcation was 72.6 seconds, and the mean time between administration of local anesthesia and wire passage was 67.44 seconds. The first pass success rate was 77.8% (70/90 patients), and the CFA puncture rate was 98.8% (89/90 patients). Although minor complications were noted in 7 patients, no patient reported serious complications (a large hematoma [≥ 5 cm], pseudoaneurysms, dissection, and/or a retroperitoneal hematoma.) CONCLUSION: Prepuncture ultrasound examination might be a simple, safe, and accurate technique for cannulation of the CFA during TFCA.

7.Studies on intestinal trematodes in Korea XVII. Development of egg lying capacity of Echinostoma hortense in albino rats and human experimental infection.

The worm development and egg laying pattern of Echinostoma hortense(Trematoda: Echinostomatidae) were studied in albino rats and the brief clinical course was observed in human volunteers. A total of 21 rats were infected with 20-69 metecercariae each and two humans were with 7 and 27 metacercariae, which were collected from loaches. For recovery of worms, the rats were sacrificed at irregular intervals from the 6th to 150th day after infection and the human volunteers were treated with praziquantel and purged with magnesium salt on the 26-27th day. The stools of the rats and humans were examined for the eggs. The results were as follows: The worm recovery rate from the rats was not affected by the increase of infection time but varied individually; 9.1-50.0% (31.1% in average). From humans, 14.3% and 37.0% (32.4% in average) of challenged were recovered. In the rats, it was revealed that the worms rapidly grew for the first 14 days to become 7.59 mm in average length and 1.17 mm in average width but the growth became much slower thereafer until the 150th day; 7.95 mm in length on the 21th day, 9.04 mm on the 28th day, 10.21 mm on the 49th day and 12.62 mm on the 150th day. During the early stage of infection, the growth of genital organs(male or female) was expressed as sigmoid curves whereas non-genital organs(such as suckers) was simply as straight lines. The prepatent period of this fluke was 10-12 days in the rats and 16-17 days in men. After the start of oviposition, the egg production by the worms remarkably increased, reached maximum on the 32-33th day, followed by decrease thereafter. The maximum value of E.P.G./worm was 390. The major subjective symptoms in human volunteers were abdominal pain and diarrhea during the early stage of infection. The results show that human is as susceptible as the rats to E. hortense infection and the amount of egg production in the rats is greatly affected by the age of worms.

Pelvic actinomycosis is an infrequent chronic suppurative granulomatous disease, caused by a gram-positive bands Actinomyces israelii. It is often reported as a complicated case of an intrauterine device (IUD). Ureteral obstruction and subsequent hydronephrosis are rare complications of pelvic inflammatory disease. Sometimes pelvic actinomyosis is simulating ovarian malignancy. We report a case combined with hydronephrosis, multiple pelvic lymph nodes enlargements and elevated CA 125 as complications of pelvic actinomycosis.
