1.Mucous Gland Adenoma of the Bronchus: A case report.

Mucous gland adenoma, one type of bronchial adenoma, is first introduced by Rosenblum and Klein2) in 1935 and very rare. Only twenty-one cases had been reported in english literature. This tumor must be differentiated from other types of bronchial adenoma because of completely benign clinical course. A case of mucous gland adenoma is presented which showed marked secondary calcification.

2.Interleukin-1 beta , -2, -6 Production, Serum Concentration and Hypothalamic-Pituitary-Adrenal Axis in Patients with Major Depression.

The present study was carried out in order to investigate the relationship between immune function and the activity of hypothalamic-pituitary-adrenal(HPA) axis in patients with major depression. The subjects were 16 female major depressives and 16 female healthy controls. We measured mitogen-induced production of IL-1 beta, IL-2, IL-6 and serum level of IL-1 beta, IL-2, IL-6 and basal plasma cortisol levels at 8 00 a.m. We measured post-DST(dexamethasone suppression test) cortisol levels in 16 major depressives. The result were as follows : 1) Basal cortisol level was significantly higher in the patients with major depression than in the healthy controls(14.4+/-4.6 microgram/dl, 10.1+/-5.2microgram /dl, respectively, p<0.05). 2) IL-2 production was significantly lower in the patients with major depression than in the healthy controls(1747.3+/-387.9 pg/ml, 2520.2+/-884.1 pg/ml, respectively, p<0.05). There were no significant differences in IL-1 beta and IL-6 production between the patients with major depression and the healthy controls. 3) Serum level of IL-2 was detectable in 12 of 16 patients with major depression and in 10 of 16 healthy controls. There was no significant difference in serum level of IL-2 between two groups. Serum level of IL-1 beta was detectable in 3 of 16 patients with major depression and of 16 healthy controls. We could not detect serum level of IL-6 in both groups. 4) There was significant negative correlation between IL-2 production and post-DST cortisol level(r= -0.89) in the 16 patients with major depression. There was significant negative correlation between serum level of IL-2 and post-DST cortisol level(r= -0.97) in the 12 patients with major depression. There was significant negative correlation between serum level of IL-2 and basal cortisol level(r= -0.65) in the 12 patients with major depression. But there was no significant correlation between IL-2 production and basal cortisol level in the 16 patients with major depression. These findings suggest that immune function is decreased in major depression and the decreased immune function is highly related to the hyperactivity of the HPA axis.

3.Hereditary Hypotrichosis ( Marie Unna Type ).

We report a family suffering from Marie Unnas hypotrichosis (8 members affected in 4 generations). At birth they had normal hairs, subsequently loss of hair on the scalp, eyebrow and eyelash. At about 3rd years, coarse twisted scalp hair regrow until puberty when progressive loss from vertex and scalp margin was noticed. On physical examination and lsboratory studies, there was no associated abnormality in the patients. The scanning electron microscopic examination of the affected hair showed cuticular peeling and twisting or angulation of hair shaft.

6.A Case of Squamous Cell Carcinoma of the Skin Resembling Atypical Fibroxanthoma.

Both spindle cell variant of squamous cell carcinoma and atypical fibroxanthoma are most commonly presented as a solitary, often ulcerated nodule, occurring on sun-exposed skin of the elderly. These lesions also share the histologic features of diffuse or indistinct fascicular arrangement of polygonal or pulmp spindle cells. Therefore it is not easy to differentiate immunohistochemistry is done. A case of 73 year old woman with squamous cell carcinoma of the skin resembling atypical fibroxanthoma is presented.

7.Totally Laparoscopic Total Gastrectomy Using Intracorporeally Hand-Sewn Esophagojejunostomy.

PURPOSE: Laparoscopic total gastrectomy (LTG) for gastric cancer is still uncommon because of technical difficulties, especially in esophagojejunostomy (EJ). There are many reports for various laparoscopic procedures of EJ using linear or circular staplers. On the other hands, there has been no report for hand-sewn anastomosis. We report successfully performed intracorporeally hand-sewn EJ after LTG. MATERIALS AND METHODS: The clinicopathologic data and short-term surgical outcomes of 6 patients who underwent totally laparoscopic total gastrectomy for upper gastric cancer from December 2010 and July 2011 were retrospectively reviewed. RESULTS: The mean age was 66.5 years and mean body mass index (kg/m2) was 24.6. All patients had medical comorbidities. The mean patient ASA score was 2.17. Among the 6 patients, previous abdominal operation was performed for 2 patients and combined operation was performed for 3 patients. The mean blood loss, operation time, and EJ anastomosis time was 130 ml, 379.7 minutes, and 81.5 minutes, respectively. The mean time to first flatus, first oral intake, and postoperative hospital stay was 3.0, 3.0, and 12.5 days, respectively. There was no 30-day mortality case. Postoperative aspiration pneumonia and multiple periventricular lacunar infarctions developed in 1 patient. There were no anastomosis-related complications and other major surgical complications. CONCLUSIONS: When the intracorporeal anastomotic technique becomes popular in LTG the intracorporeally hand-sewn EJ may be accepted as one method among the various laparoscopic procedures of EJ.

8.Clinicopathological observation of the patients with isoniazid responsive inflammatory nodules on the legs.

We performed a clinicopathological study of nineteen patients with chronic inflammatory nodose lesions of the legs which responded to the treatment with isolniazid. The results were summarized as follows: 1. Seven patients had a personal or family history of tuberculcsis and all patients showed a high tuberculin sensitivity. But, no one showed the evidence of active pulmonary tuberculosis. 2. The inflammatory nodules and swelling of the legs were resolved within 12 months in all cases. Resolution of the nodules was more rapid than that of leg swelling. 3. The clinical characteristics of the patients with chronic inflanimatory nodules were the same as those of the cases with erythema nodosum or erytiema induratum reported previously in Korea. The basic histopathologic process of inflarr matory nodules seemed to be vasculitis.

10.The Effectiveness of Dementia Education for the Nursing Method of Family Caregivers of the Demented Elderly.

PURPOSE: This study attempted to identify the effectiveness and substantiality of a dementia education program for family caregivers of the demented elderly. METHODS: This quasi-experimental study using a nonequivalent control pretest-posttest design was carried out from April 15 to July 29 in 2012 with 137 subjects divided into an experimental group (n=70) and a control group (n=67). This study examined the subjects' general characteristics, and evaluated their nursing method four times, respectively, before the intervention, right after the intervention, and 2 and 4 weeks after the intervention, and then analyzed collected data through t-test and chi2-test. RESULTS: In the experimental group that had received the dementia education, the nursing method score was significantly higher right after the intervention (t=8.33, p<.001) and 2 weeks after the intervention (t=8.01, p<.001) than before the intervention, but their score 4 weeks after the intervention was not significantly different from that of the control group (t=1.08, p=.28). CONCLUSION: The dementia education was found to be effective in improving the nursing method of family caregivers of the demented elderly. This study implies that further follow-up dementia education is necessary between 2 and 4 weeks to maintain the effectiveness.
