To evaluate the cost-effectiveness of excretory urography, results were reviewed in 60 patients with chronic epididymitis. In 37 patients(of total 60 cases) who have neither scrotal fistula nor hematuria (microscopic or gross) nor positive urine AFB staining, there was no tuberculous finding in excretory urography. And findings consistent with renal tuberculosis were found in 11 cases (73% ) of 15 patients with scrotal fistula, in 9 cases (75%) of 12 patients with hematuria and in 4 cases (80%) of 5 patients with positive urine AFB staining. Therefore, routine use excretory urography in chronic epididymitis is revealing neither scrotal fistula nor hematuria nor positive AFB in urine is not always necessary for detection of renal tuberculosis in terms of cost effectiveness.

Cytokines are hormone-like proteins which mediate and regulate inflammatory and immune responses. The purpose of this study was to investigate the effect of lipopolysaccharide (LPS), Staphylococcus enterotoxin B (SEB), and Mycoplasma lysates on regulation of IL-6 and IL-8 production by human nasal fibroblasts. Primary cultured cells were incubated with LPS (1.0 ug/ml) from E.coli, SEB (1.0 ug/ml) from S.aureus, or Mycoplasma lysates (M.pneumoniae, Mp; M. fermentans, Mf; M. hominis, Mh, each 1.0 ug/ml). The culture supernatants were collected at 2, 6, and 24 hr and assessed for IL-6 and IL-8 production by enzyme-linked immunosorbent assay. The production of IL-6 in the culture supematant was downregulated by LPS, SEB, or Mycoplasma lysates. But IL-6 was upregulated by mixed exposure with Mp+LPS (2 hr), Mp+LPS+SEB (24 hr), Mf+LPS (24 hr), Mf+LPS+SEB (2 hr), Mh+LPS (24 hr), Mh+SEB (24 hr), or Mh+LPS+ SEB (24 hr). The production of IL-8 in the culture supematant was similar to that of IL-6 by same stimulants. But IL-8 was upregulated by mixed exposure with Mf+LPS+SEB (2 hr), Mh+LPS (24 hr), Mh+ SEB (24 hr), or Mh+LPS+SEB (24 hr). These studies show that costimulation of LPS or SEB with Mycoplasma whole cell lysates upregulates the production of IL-6 and IL-8.

Synovial chondromatosis is a rare entity characterized by metaplastic cartilage formation within the synovial connective tissue and the shedding of loose bodies into the joint. Of the four cases of synovial chondromatosis, experienced by authors, three involved the knee joint and one the proxiaml phalanx of the left index finger. A huge chondroma was found in the knee joint of 2 years old boy and the other one in the proximal phalanx of the left index of 11 years old girl. Histological findings disclosed the metaplastic transformation of synovium into cartilage in all cases.

Primary phycomycosis of the stomach within the ulcerative portion of early gastric carcinoma is presented in a sixty one year old male alcoholics. In the center of IIc type submucosal adenocarcinoma was a large deep ulcer (UL-IV) which was the only seat of phycomycetes infection accompanied with angioinvasion and exudative endovasculitis aside from unusual granuloma formation with Splendore Hoeppli phenomenon and eosinophilic response as in allergic granulomatous aspergillosis. The above features were reconstructed to indicate that the phycomycetes infection was confined to the type III area of IIc + III early gastric carcinoma, and that the local ulcerogenic lesion seems more contributory in its development than systemic factors including gastric carcinoma per se or alcoholic consumption.
