1.Histopathologic Resemblance of Ovarian Dermoid Cyst to Various Skin Tumors.

BACKGROUND: The wall of ovarian dermoid cysts shows various kinds of morphologic patterns. Some of them resemble the features of epidermal and epidermal appendageal tumors. OBJECTIVE: The purpose of this study is to classify the morphologic features of the wall of ovarian dermoid cyst from the dermatohistopathologic viewpoint. METHODS: H&E stained slides of 203 ovarian dermoid cysts were reviewed RESULTS: Thirteen different patterns of morphologic features were found. They included features of organoid nevus, sebaceous hyperplasia, steatocystoma multiplex, epidermoid cyst, nevus comedonicus, epidermal nevus, apocrine hidrocystoma, syringoma and trichilemmal cyst. CONCLUSION: Various epidermal and epidermal appendageal tumors may be associated with ovarian dermoid cyst.

Synovial sarcoma is an uncommon, malignant tumor which originates from the synovial lining of the joints, bursae and tendon sheaths. But it rarely occurs from bones. We have experienced a synovial sarcoma of the left ulna. Microscopically cleft spaces lined by epithelioid cells and bundle formation by spindle spaped cells were seen. Above elbow amputation of the left arm was done.

Miescher syndrome comprises congenital acanthosis nigricans, hypertrichosis, failure to thrive and short stature, dysmorphism especially of the jaws and oral cavity, insulin-resistant diabetes mellitus, and a characteristic general appearance. This report concerns a rare case of 12-year-old girl having insulin resistant diabetic mellitus with Miescher syndrome. The relevant literature was reviewed.

5.Microscopic Approach of Mass Involving Neurovascular Pedicle in the Hand.

PURPOSE: Mass can compress around tissue and cause deviation of normal anatomical structures. Often, mass grows toward neurovascular pedicle and encircles depending on the nature of mature mass. Neglecting neurovascular involvement of the mass is a serious problem not to be overlooked. Authors have performed microscopic approach regarding mass involving the neurovascular pedicle in the hand. MATERIALS AND METHODS: From January 2007 through February 2012, retrospective analysis for nine cases of mass involving neurovascular pedicles was done. Patients were evaluated preoperatively by ultrasonography or MRI and checked intraoperative finding. Masses were evaluated by site, preoperative evaluation, involved neurovascular pedicle, histopathologic diagnosis, complication, and recurrence. RESULTS: The site of mass involving neurovascular pedicles was 4 cases on the wrist, 2 cases on the palm, 2 cases on the finger, 1 case on the hand dorsum. Involved neurovascular pedicles were 3 radial arteries and nerves, 3 proper digital arteries and nerves, 1 radial artery, 1 superficial branch of radial nerve, 1 common digital artery and nerve. The histopathologic diagnosis of mass were 3 ganglions, 2 giant cell tumors, 2 epidermal cysts, 1 fibroma, and 1 benign spindle tumor. There were 2 cases of recurrence and secondary excisions were performed. CONCLUSION: Neurovascular pedicle injury can lead to serious complication like sensory and motor disorders, distal part ischemia, and so on. In case of mass suspected neurovascular invasion, accurate preoperative evaluation such as ultrasonography or MRI is necessary. To prevent any neurovascular related complication during mass excision, delicate surgical technique using a microscope becomes essential.

6.Surgical Fixation of Sacroiliac Joint Complex in Unstable Pelvic Ring Injuries.

PURPOSE: To evaluate the effectiveness and clinical outcomes of surgical fixation of a sacroiliac joint complex in unstable pelvic ring injuries. MATERIALS AND METHODS: We selected sixteen cases in our hospital from 2006 to 2010 that underwent surgical fixation of the sacroiliac joint complex and had unstable pelvis ring injuries corresponding to B and C of Tile classification. Plate fixation through an intra-pelvic anterior approach was performed for 9 cases, tension band plate fixation for 2 cases, iliosacral screw fixation for 4 cases, and spino-pelvic fixation through extra-pelvic posterior approach for 1 case. Radiological and clinical evaluations were implemented to determine the results of treatment. RESULTS: Bone union was observed in all patients. For radiological evaluation using the Matta and Saucedo criteria, 15 cases were above the nearly-anatomic reduction. Out of 10 cases that performed rotational displacement analysis, 9 cases were above fair. All cases had above moderate clinical results as well. CONCLUSION: For unstable pelvic ring injuries, the satisfactory radiological and clinical results have been obtained through the anatomical reduction of the sacroiliac joint complex and firm internal fixation. And from the evaluation of fracture types, the Type C fracture and vertical shear type fractures showed relatively poor results.

8.The Change of the Pathogen in Cultured Jones Tubes according to the Postoperative Periods.

PURPOSE: To evaluate the change in pathogens in cultured Jones tubes used in lacrimal bypass surgery according to postoperative period and to provide basic data related to preventive antibiotics or functional lacrimal stent development. METHODS: Fifty patients who underwent Jones tubes removal were enrolled in this study. Removed Jones tubes were cultured to identify bacteria and were tested for antibiotic sensitivity. The results were further analyzed according to the period between lacrimal bypass surgery and tube removal. RESULTS: Among 50 cases, 24 (48%) showed cultured bacteria of Staphylococcus aureus, 5 (10%) were Pseudomonas, and another 5 (10%) were Gram-positive bacilli. Although Staphylococcus aureus was the most frequently cultured organism, Proteus mirabilis was the most common cultured organism in patients who underwent tube removal more than 10 years after lacrimal bypass surgery. There was no significant correlation between cultured organism and the period between lacrimal bypass surgery and tube removal. Eighty four percent of cultured Staphylococcus aureus showed resistance to penicillin, and 53% of cultured Staphylococcus aureus showed resistance to methicillin. CONCLUSIONS: Staphylococcus aureus was the most frequently cultured organism according to Jones tube-related lacrimal bypass surgery. A large proportion of cultured Staphylococcus aureus showed resistance to penicillin and methicillin. Proteus mirabilis should be considered the most common pathogen in patients more than 10 years after lacrimal bypass surgery.

9.Clinical Evaluation of Subretinal Neovascular Membrane.

Subretinal neovascular membrane in the macula is a major cause of central visual loss. We reviewed the clinical records, fundus photography, fluorescein angiography of the 172 eyes of 166 patients in order to disclose the causes and clinical characteristics of subretinal neovascular membrane and performed argon and krypton photocoagulation in 22 patients. The common causes of subretinal neovascular membrane are idiopathic neovascular membrane, myopic and senile macular degeneration in order(27.1, 26.5, 22.3% aa). There was no significant difference in changes of visual acuity and extent of subretinal neovascular membrane between treatment and non-treatment groups.
