1.Clinical Observation of Tibial Plateau Fracture

The tibial plateau fracture is a common fracture in the knee joint. Tibial plateau fracture involves most always the articulating cartilage and produce the disability of the knee joint oftenly. The 25 cases of tibial fractures have been classified by Hohls method and analysed their clinical courses more than three month period. The 13 cases out of 25 had treated in conservative method such as plaster of Paris with or without closed reduction. Remaining 12 cases were treated operatively due to the failure of closed reduction or associated injury of major ligaments. More than 8 mm of depression or 5 mm split of the plateau fragment is classified as failure of closed reduction. Excellent end result is obtained in 21 cases with free of pain on walking and full range of joint motion without instability and muscle weakness, Three cases were classified as Good result with occasional pain on walking, range of joint motion is available more than 0~90 degree and good grade of muscle power without instability. One case was poor in result with instability of joint and pain on walking. The radiological end result of accurate reduction of fragment is not closely correlated with functional result in the treatment of tibial plateau fracture. The most important factor in the treatment of tibial plateau fracture is the exercises of the joint as early as possible.

3.A Case of Ehlers - Danlos Syndrome.

We presented a case of Ehlers-Danlos syndrome(ED syndrome) in s 21-year- old male, who had hyperextensibility of skin, skin fragility, and cigarette paperlike atrophic scars with no joint problems since infancy. There was a family history which was suggestive of autosomal dominant trait. Microscopic examination of the skin biopsy specimen from the normal appearing skin on the right forearm showed no abnormal appearance. In this case the manifestations of skin including marked hyperextensibility might be in accord with type I (gravis) ED syndrome, but the joint manifestation did not correspond to this type. It might be hard to classify this case according to the eleven types of ED syndrome. Wed rather consider this case as a. mixed form of type I and type Il ED syndrome.

6.Computed tomographic staging of renal cell carcinoma

It is generally agreed that CT is a good staging method of renal cell carcinoma. However, CT has its ownpitfalls. Herein we reviewed 31 patients with renal cell carcinoma whose CT and pathologic stages were available.CT accurately staged 12 of 14 patients with Robson's stage I (86%), 6 of 8 with stage II (75%), 4 of 4 with stageIIIa (100%), 2 of 3 with stage IIIb (67%), and 1 of 2 with stage IV (50%). Overall staging accuracy of CT was 81%(25 of 31). CT failed to differentiate stage I and stage II in 4 cases. CT also failed to diagnose the microscopiclymph node metastasis in 1 case, and invasion of Gerota's fascia in another case. However CT was satisfactory indifferentiating surgically curable stage I to III from surgically incurable stage IV, and high reliable in thediagnosis of inferior vena caval thrombosis. Therefore it is suggested that CT is highly useful in determining thetreatment plan of renal cell carcinoma.

7.A Comparison of Prophylactic Efficacy of Intravesical Instillation with Bacillus Calmette-Guerin and Thiotepa in Superficial Bladder Cancer.

We compared the prophylactic efficacy of intravesical BCG with thiotepa instillation after complete removal of superficial bladder cancer. Forty patients received BCG and 32 received thiotepa. The mean followup period was 29 months in BCG group, and 34 months in thiotepa group. The overall recurrence rate was 35% in BCG group compared with 53% in thiotepa group, showing difference statistically(p<0.05) The mean time to recurrence was 8.7 months in BCG group, and 7.2 months in thiotepa group(p>0.05). The overall recurrence index per 100 patient-months was also lower for the BCG versus the thiotepa group(1.21 versus 1.56, p>0.05). One patient in the BCG group and 4 in the thiotepa group had recurrent tumors with progression in stage. Side effects were irritative voiding symptoms(100%), fever and chills(10%), and inguinal lymphadenitis(2.5%) in BCG group, while irritative voiding symptoms(25%), fever and chills(6.3%), and myelosuppression (6.3%) in thiotepa group. Our results suggest that BCG is significantly superior to thiotepa in reducing bladder tumor recurrence and in retarding tumor progression.

8.A Case of Ectopic Thoracic Kidney.

Ectopic thoracic kidney is an extremely rare developmental anomaly. Most cases are discovered incidentally on a routine chest roentgenogram. Since the ectopic thoracic kidney may be mistaken for a space-occupying lesion originating above the diaphragm, it is of clinical and diagnostic importance to the thoracic surgeon. We report a case of ectopic thoracic kidney that was diagnosed by chest roentgenogram, intravenous pyelogram, computerized tomogram, and aortogram.

10.Malignant Lymphoma Associated with Hashimoto's Thyroiditis.

Primary lymphoma of the thyroid gland is relatively rare disease. Its incidence varies in different series from 1% to 5% of all thyroid malignancies. Most primary thyroid lymphoma are of non-Hodgkin's type, have a B-cell phenotype, and arise in a background of autoimmune thyroiditis in about 80% of cases. A 69-year old woman presented with anterior neck swelling. Ultrasonography and computed tomography demonstrated a heterogenous soft tissue mass in both thyroid lobes and lymphadenopathy. Total thyroidectomy with lymph node dissection was performed. Histopathological findings demonstrated high-grade mucosa-associated lymphoid tissue (MALT)-type lymphoma with Hashimoto's thyroiditis. Radiation therapy was performed. We report a case of primary thyroid lymphoma with Hashimoto's thyroiditis.
