1.Cloning and Sequence Analysis of Gene Encoding an Outer Membrane Protein ( OmpTL ) of Treponema Strain PFB4G Isolated from Periodontitis Patients.

Treponema strain PFB4G is a novel oral spirochete and one of the most frequently detected organisms in subgingival plaque samples from rapidly progressive periodontitis and adult periodontitis patients. In this study, a genomic library of Treponema strain PFB4G was constructed in lambdaZAP expression vector. One positive clone that carried a 2.6-kb fragment was identified by screening with chicken Ig Y (immunoglobulin yolk) antibody raised against whole bacterial sonicates. Nucleotide sequencing of the subclones revealed an open reading frame (ORF) lacking the 5'-end. This region was obtained by PCR amplification using a degenerative and a specific primer. A complete open reading frame of 1,770 bp was identified and the deduced polypeptide consisted of 590 amino acids with a molecular mass of 65 kDa. The polypeptide, designated as OmpTL, had a typical prokaryotic signal sequence (19 amino acids) with a potential cleavage site for signal peptidase I and showed a significant level of homology with the outer membrane proteins of other oral treponemes, especially with that of Treponema maltophilum. The isolation of the gene encoding an outer membrane protein may allow the study of their roles in future, possibly as adhesion, pore forming or induction of proinflammatory cytokine synthesis.

2.A case of palatally impacted incisor treated by onlay technique with surgical exposure.

The patient, a girl of 19 years in good health, had a class I malocclusion. The maxillary left centra1 incisors and both lateral incisors had already erupted. But the space for the right central incisor was partially closed by the mesial drifting of the neighboring teeth. The caused a shift in the midline and a cross-bite relation on the incisors. X-ray examination revealed the presence of the right central incisor in the alveolar bone and odontoma just above the crown of the right central incisor. After enough space for the impacted incisor was created in the dental arch with a open-coil spring the rectangular incision was made. Removing the odontoma uncovered the flat surface of the labial aspect of the incisor. During the tooth had erupted of its own accord, any unnecessary force had been imposed on the tooth. When it was decided that the tooth should be brought out by the mechanical device, the gold cast onlay with hook was used and run a light elastic between this hook and the main arch wire. Finally the tooth was brought down to the arch level. The result was excellent. Fortunately the esthetic problem and any detrimental effects on the psychological make-up could be avoided.

4.Clinical Evaluation of Intravenous Regional Aneetheeia using Bupivacaine .

We prospectively studied the clinical effect of intravenous bupivacaine for regional anes-thesia in the lower & upper extremities in sixty consecutive patient from September, 1985 to truly 1986. 0.2% bupivacaine was used and dosage was varied between 1.0mg/kg and 2.5mg/kg. according to the 7ype of surgery, site, body weight and the size of the limb. The amount of bupivaeaine required was rather depend upon the size of the limb thats the patient's body weight. The effect of the anesthesia for a variety of extremity procedures was rated as exoellent, good or fair in 98.3% of the patient.

5.Clinical Study of Epinephrine Administration to Pediatric Patients under Halothane Aneathesia for Closure of Cleft Palate .

A retrospective review was performed of 72 cases of cleft palate operated at the Nati-onal Medical Center from March 1985 to June, 1986. We evaluated the frequency of cardiac arrythmia and changes in the heart rates when epinephrine is used with halothane anesthesia in pediatric patients for the closure of palate. 21 children (29.2%) developed several types of ventricular dysrrhythmias and one child had ventricular fibrillation. 41 children(65.37) had increase in heart rates, and 23 children had no change after epinephrine injection. All children restored sinus rhythm after 100% oxygen inhalation lidocaine(1-1.5 mg/kg) , and demerol (0.5-1 mg/kg) were injected.

6.Expression of matrix metalloproteinase-1 and -2 mRNA in retrodiscal tissue of the temporomandibular joint.

Matrix metalloproteinases (MMPs) play an important role in the normal morphogenesis, maintenance, and repair of matrix and also have important functions in pathologic conditions characterized by excessive degradation of extracellular matrix, such as rheumatoid arthritis, osteoarthritis, periodontitis and in tumor invasion and metastasis. In this study, expression of MMP-1 and -2 mRNA in retrodiscal tissue of the temporomandibular joint (TMJ) was examined and compared with magnetic resonance imaging (MRI) and surgical findings. MMP mRNAs in the retrodiscal tissue samples were detected by reverse transcription - polymerase chain reaction. TMJ internal derangement (ID) was categorized as normal disc position, disc displacement with reduction, early stage of disc displacement without reduction (DDsR) and late stage of DDsR. TMJ osteoarthrosis (OA) was classified with normal, mild and advanced OA. The amount of synovial fluid collection was divided into not detected, small, large and extremely large amount on MR T2-weighted imaging. Perforation and adhesion were examined during open surgery of the TMJ. Six out of 37 samples were excluded because of little amount of extracted total mRNA. MMP-2 mRNA was detected whole joints, and so the MMP-2 mRNA seems to be expressed normally in retrodiscal tissue. However, MMP-1 mRNA was expressed in 8 of 31 joints. Frequencies of MMP-1 mRNA expression according to the TMJ IDs, amount of synovial fluid and surgical findings made no significant difference. MMP-1 mRNA was detected more frequently in OA groups (7/16 joints, 43.8%) than in normal bony structure group (1/15,joints, 6.7%). Expression of MMP-1 mRNA in retrodiscal tissue might be related with OA of the TMJ.

9.The distance between the upper canine and the orbital plane in gnathostatic model of Koreans with normal occlusion.

In orthodontic field, the study on gnathostatic model is very important to the diagnosis and analysis of malocclusion. The purpose of this study is to observe that relation of orbital plane and upper canine in Koreans. We selected 230 cases with normal occlusion, 124 male and 106 female cases aged from 12 years to 25 years, to prepare 230 gnathostatic models, and measured the distance from midpoint of mesio-distal distance of upper canine to orbital plane in gnathostatic model. The results obtained were as follows: 1) The mean value of the distance from upper canine to orbital plane showed no significant change with age. 2) The mean value of the distance from the upper canine to orbital plane was -3.11mm in male and -3.84mm in female. 3) The orbital plane passed through the upper canine in normal occlusion of Koreans.
