8.The Clinical Follow-up Study on Total Hip Replacement

Total Hip Replacement is well documented and widely used procedure for painful arthritic hip in past two decades and each year still seems to be increased in numher of total hip replacement. And recently, there is a few reports of follow-up study on total hip arthroplasty in our country. The aim of this study was to present the clinical material which was performed on 27 hips of 21 patients who were treated at Dept. of Orthopedic Surgery. College of Medicine, Chungnam university from jan, 1979 to Jan. 1982. The longest follow up was 4 years, shortest was 8 months, average time was 2 years and 8months The results were obtained as follow; 1. The age incidence ranged from 26 to 69 years old, and average were 47 years old. 2. There were 17 males (81%) and 4 females (19%). 3, The causes of hip disease were mostly avascular necrosis of the femoral head (67%). 4, The most commonly used prosthesis were Muller type with trochanteric ostcotomy approaches. 5. Several complications were found: perforation of acetabulum with bone cenent in pelvis, postperative pneumonia wire breakage and painful bursitis around the trochanteric area which was on osteotomy sites. 6. Postoperative functional evaluation was evaluated by method of d'Aubigne and Postel, and improved from 10.0 scores to 15.8 scores. Relief of the pain is the most effect of the total hip replacement in this study, But case are not enough to more detail evaluation, especially, the problem of loosening of stem, acetabular cup wearing and heterotopic bone formation after total hip replacement.

9.A Clinical Study of closed Flexible IM Nail for Fractures of Distal one

Fractures of the tibia are frequently encountered in mordern civilized society. Especially, those in distal 1/3 of the tibia have so many problems, such as angular deformity, osteomyelitis and nonunion because of anatomically scanty soft tissue. Many modalities of the treatment of fracture of distal 1/3 of the tibia were introduced. Among them, interlocking IM nailings were considered as one of the best modality. But, in interlocking IM nail, reaming of the medullary canal which destroys the endosteal circulation and causes thermal necrosis of the inner aspect of the cortical bone results in delayed union. And also, procedure for distal screw fixation is difficult and time-consuming. As for using flexible IM nails for fractures of distal 1/3 of the tibia, it was considered as not so satisfactory method due to lack of stability of fixation. However, the stability of fixation can be strengthened with fanning of the nails in distal fragment, using more than 3 nails and delay the protected weight bearing. Authors studied 25 cases of fractures of distal 1/3 of the tibia treated with closed flexible IM nailing at Sung Ae General Hospital, from July., 1987 to July., 1989, and obtained following results: 1. Among 25 cases, 4th decades were most common and males were more commonly involved. 2. Traffic accidents were the most common causes of injury and majority of 16 open fractures and 19 comminuted fractures, were caused by high energy mechanism. Among open fractures, type II were most common. 3. Two angular deformities less than 10 degrees, one proximal migration and one checkrein deformity were observed as complications, but, no clinically significant complications were noted. 4. The time for radiological union were 14.5 weeks in group without fibular fracture and 15.1 weeks in group with fibular fracture, and 14.0 weeks in closed fracture group and 17.2 weeks in open fracture group, and average in 15.6 weeks.

10.Experimental Study of Nerve Regeneration by Inside-in and Inside-out Vein Graft in the Nerve Defect of Rat.

PURPOSE: Autogenous vein graft as an alternative to autogenous nerve graft for bridging the gap of the peripheral nerve has been widely accepted. The purpose of this study was to find the comparative superiority between the two grafting techniques of inside-in and inside-out vein graft in rat model. MATERIALS AND METHODS: Thirty-three Sprague-Dawley rats were divided into three experimental groups: inside-in vein graft (group A), inside-out vein graft (group B) and control group. In group A, 15 mm jugular vein was grafted to a 10 mm gap created in the sciatic nerve. In group B, the same procedure was performed with group A, however, the vein was pulled through itself and its normal orientation was inverted before grafting. To compare the results of the two groups, electromyography and histologic studies were performed at 4, 8 and 12 weeks postoperative. RESULTS: In the electromyographic study, the mean of peak amplitude of compound motor action potential in group B was significantly higher than in group A at each experimental week. In the light and electron microscopic study, group B had more abundant regenerated axons and myelinized axonal fibers than group A at 4 weeks. At 8 and 12 weeks, there were no significant differences in the numbers and extents of myelinization of regenerated axons in both groups. CONCLUSION: This study demonstrates that inside-out vein grafting technique induces more faster axonal regeneration and earlier reinnervation to the denervated distal muscles in rat model.
