1.The Effect of a Education regarding Women's Breast Self-Examination Teaching Methods.

The purpose of this study was to investigate the effects of breast self-examination teaching methods which are printed material, videotape, demonstration used BSE model on women. The subjects for this study were nonmedical professional 88 women in Kwangju. The data was collected from October 16 to November 24, 1995 by means of a structured questionnaire. For measuring BSE knowledge, Kim's(1994) instrument was used ard modified by researcher. For measuring BSE attitude based on health belief, Lee's(1982) & Kim's(1994) instrument was used and modified by researcher. Printed material from literature were reviewed by researcher, "A woman's guide to breast self-examination" videotape by Nurse Practitioner Creation and BSE simulator by Narco were used 21 attitude scale and 13 knowledge scale were checked before and after teaching methods. The data was analyzed by frequency, paired t-test, ANOVA using the SAS program. The results were as follows : 1. The BSE total knowledge score after having teaching was higher than before, different significantly(t=-14.05, p=0.000). The most effective teaching method on BSE knowledge change was demonstration used BSE model in three teaching methods and was different significantly(F=8.44, p=0.000). 2. The BSE total attitude score after having teaching was changed to more positively, different significantly(t=4.59, p=0.000). The most effective teaching method on positive BSE attitude change was demonstration used BSE model in three teaching methods and was different significantly(F=3.21, p=0.045). 3. The HB score after having teaching was higher than before, and benefits, barriers and health concerns were different significantly(t=-2.51, p=0.014 ; t=-4.53, p=0.000 ; t=-3.71, p=0.000). Susceptibility and seriousness were not different significantly. 4. There were different significantly among knowledge, religion(F=3.92, p=0.013) and child(F=3.08, p=0.08). There were different significantly among attitude, age(F=4.65, p=0.05), menstruation state(F=4.39, p=0.016), and artificial abortion experience(F=5.18, p=0.026). There were different significantly among health concern, age (F=2.50, p=0.068), delivery experience(F=3.22, p=0.076), artificial abortion experience(F=5.46, p=0.023), and feeding(F=6.24, p=0.003). Printed material, videotape, and demonstration used BSE model may be valuable for changing positively knowledge and attitude related to BSE. Demonstration used BSE model was most effective teaching method in this study. Therefore, this study revealed that BSE teaching methods were a useful nursing intervention for promoting practice regularly and exactly on women.

2.Expression of Bovine Growth Hormone Gene in a Baculovirus, Hyphantria cunea Nuclear Polyhedrosis Virus.

Bovine growth hormone (bGH) gene was expressed in an insect spodoptera frugiperda cell line using a Baculovirus, Hyphantria cunea nuclear polyhedrosis virus (HcNPV). The bGH gene in pbGH plasmid was sequenced and amplified by PCR technique with two primers containing NcoI sites. The bGH gene consisted of 654 bp (217 amino acid residues), the 5'-untranslated region of the cloned bGH cDNA contains 56 bp, and the 3'-untranslated region contains 145 bp and two pallindromic regions. The amplified bGH gene DNA fragment (654 bp) was inserted into the NcoI site of the pHcEVII vector, which was named pHcbGH. The pHcbGH transfer vector DNA and the wild type HcNPV DNA were cotransfected into s. frugiperda cells to construct a recombinant virus. Eight recombinant viruses were selected and named HcbGH. One clone, HcbGH-4-1 showed largest plaque size, therefore the recombinant virus was further studied. The multiplication patters of the recombinant HcbGH-4-1 was similar to that of the wild type HcNPV. The bGH gene DNA in the HcbGH-4-1 recombinant was confirmed by Southern lot hybridization. The amount of the bGH (217 amino acid residues, 21 kDa) produced in S. frugiperda cells infected with the HcbGH-4-1 recombinant was approximately 5.5 ng per ml (106 cells) by radioimmunoassay.

Cystic entrapment of infraspinatus branch of suprascapular nerve is a rare cause of shoulder pain with infraspinatus atrophy. We discuss the ultrasonographic diagnosis and ultrasonography- guided aspiration of the cyst as a treatment. Case: A 37-year-old, ship-yard laborer had experienced left shoulder discomfort and pain without trauma history. Physical examination showed profound selective atrophy of left infraspinatus muscle. Simple X-ray of shoulder showed normal findings. Electrodiagnostic examination revealed delayed motor latency of infraspinatus branch of suprascapular nerve and denervation potentials of infraspinatus muscle exclusively. Ultrasonographic finding showed 13.5X21.5 mm sized cystic mass around left infraspinatus notch. Ultrasonography-guided aspiration was done, and then electrical stimulation therapy and strengthening exercise of left shoulder were performed. Symptoms were improved. We report a rare case of cystic entrapment of infraspinatus branch of suprascapular nerve in a patient without traumatic insult.

8.A Case of Chronic Neutrophilic Leukemia with Multiple Myeloma.

Chronic neutrophilic leukemia(CNL) is a rare disorder characterized by persistent neutrophilia of mature cell type without evidence of infection, hepatosplenomegaly, elevated neutrophil alkaline phosphatase score and the absence of Philadelphia chromosome We report a typical case of CNL with multiple myeloma(IgG-lambda type) in 68 years old female. Many neutrophils had toxic granule and vacuole. The peripheral blood smear showed neutrophlic leukocytosis(50,940/microliter, neutrophil 94%) with rare immature form. The cytogenetic study showed normal karyotype without Philadelphia chromosome. She was diagnosed as multiple myeloma with paraproteinemia, lytic bone lesion of skull, and immature plasma cells in bone marrow.

The migratory thrombophlebitis and thromboembolic disorders of the venous and arterial systems in the setting of malignancy are termed Trousseau's syndrome. The overall incidence of clinical thromboembolic events in patients with cancer has been reported to vary between 1~11%. Pancreatic carcinoma has been associated with the greatest risk of thromboembolic events. Other tumor type also prone to an increased risk of thromboembolic events, including lung, prostate, stomach, acute leukemia and colon cancer. Hypercoagulability associated cancer may result from activation of coagulation, injury to the endothelium, or alteration of blood flow. Unlike other coagulopathies, Trousseau's syndrome may manifest with thromboses in unusal areas, including the upper extremities, face and visceral organs. Neck vein thrombosis associated with distant cancers have been rarely reported. Jugular vein thrombosis associated with gastrc cancer is very rare. We report one case of gastic adenocarcinoma presented as internal jugular vein thrombosis.

10.A Case of Cutaneous Horn Associated with Acquired Digital Fibrokeratoma.

Cutaneous horn is a relatively rare tumor, and is characterised as a protrusion of keratinized material from the skin in the shape of a horn. The lesion can be derived from many primary underlying lesions which may be benign, premalignant or malignant, so it is important to determine an accurate condition at the base of the lesion. We report a case of cutaneous horn on the right middle finger of a 63 year old woman. A 0.8 X 0.5 X 0.4 sized, dark-brownish protruding papule was observed. Histopathologic findings showed prominent orthokeratotic hyperkeratosis and findings at the base of the lesion were consistent with acquired digital fibrokeratoma.
