1.Dysplasia Epiphysealis Hemimelica: A Case Report

A case of dysplasia eplphyseaalis hemlmelica (Fairbank, 1956) in a 8 years old boy, showlng typical clinical and roentgenographical appearance, is reported together wlth a brief review of literature in this paper. This condition is a rare developmental disorder of childhood in which there is a asymmetrical cartilaginous overgrowth of one or more than one epiphysis, or of a tarsal or carpal bone. Thls disorder is usually Iimited to elther the medial or lateral half of a single extremity. The disease is characterised radiographically by delay in the appearance of the epiphyseal nucleus, which is mottied and of irregular density. The most common deformities were genu valgum or varum and valgus or equinus deformity of the ankle. These deformities tended to increased wlth growth but the size of the lesion increased in proportion to the growth of the normal bone. Concomitant involvement of the knee and ankle is the rule, and in those cases the affected limb Is usually longer than Its mate. The lesion Is often microscopically indistingulshable from an osteochondroma.

2.Recurrent Annular Erythema in A Patient Presumed to Have Lupus Erythematosus.

Annular erythema associated with lupus erythematosus/ Sjogrens syndrome has recently been described in Orientals. We present a patient with recurrent annular erythema who partially demonstrated features of lupus erythematosus. A 32-year-old man was referred to us for recurrent annular erythema. Laboratory findings revealed mild leukopenia and the presence of antinuclear antibodies at a titer of 320 with a finely speckled pattern on Hep-2 substrates. Anti-Ro/La antibodies were also detected. A skin biopsy specimen revealed the findings of perivascular and periappendageal lymphocytic infiltration without prominent hydropic degeneration of the basal layer. Skin lesions subsided with hydroxychloroquine (400-200mg/day).

3.Clinical and Histopathologic Study of the patients with Lichen Striatus.

Nineteen patients with lichen striatus were investigated by clinical data, clinical photographs and histopathologic study by the pattern analysis method of Ackermsn. Results were as follows '. 1. The male to female ratio was 1: 1.1, and average age was 7.8 years. Seven- teen patients (about 90%) visited hospitals during summer season. 2. Nine cases showed superficial perivascular dermatitis, 9 cases interface dermatitis (vacuolar type : 7, lichenoid type, 2), 1 case nodular dermatitis and 2 rases interface dermatitis with nodular dermatitis. 3. In the epidermis, 9 cases showed variable nurnber of dyskeratotic cells. One case showed finding of perforating lichen striatus. 4. Two cases showed inflammation around acrosyringium, 4 cases around hair follicle, 6 cases around eccrine sweat gland and 1 case showed perineural inflamrnation. Summarizing above results, histopathology of lichen striatus characteristically shows superficial perivascular dermatitis, interface dermatitis, and infiltration of inflammatory cells around the sweat gland, which helps to diagnose the disease.

A localized area of alopecia of the scalp can be a challenge in diagnosis. We report two patients with alopecia which was found to be associated with underlying congenital melanocytic nevus. Congenital melanocytic nevus should be taken into consideration in the differential diagnosis of alopecic plaques of the scalp.

8.A Study of Food Allergy in Atopic Dermatitis Using CAP-RAST FEIA and Open Food Challenge Test.

BACKGROUND: The role of dietary factors in atopic dermatitis has been the subject of debate for several decades. Recently it has received convincing support from a double-blind controlled trial of an exclusion diet, showing benefit from careful avoidance of egg and milk in a group of atopic children. OBJECT: The purpose of this study was to determine the prevalence of specific food allergies in Korean atopic dermatitis and the potential utility of the Pharmacia CAP-RAST FEIA in the diagnosis of food allergy. METHODS: One hundred ten patients with atopic dermatitis were evaluated for food hypersensitivity by history, skin prick test, CAP-RAST FEIA, and open food challenge test(OFCT). RESULTS: The prevalence of specific food allergies in the study population was 28.2% in history and 18.2% in CAP-RAST FEIA and OFCT. The frequent food allergens were milk, egg, peanut, and wheat. There was no correlation between the clinical severity of atopic dermatitis and CAP-RAST FEIA and OFCT results. Plasma eosinophil and IgE levels were correlated with CAP-RAST FEIA results, but not OFCT results. According to the history, 31 atopic dermatitis patients answered that their dermatitis was related to food, and of the 31 patients, positive reactions to prick test and CAP-RAST FEIA were observed in 8 and 22 patients, respectively. Among the other 79 patients who answered that there was no relation between atopic dermatitis and food, 14 patients reacted positively to prick test and 47 patients showed positive result to CAP-RAST FEIA. CONCLUSION: The CAP-RAST FEIA provides standardized measurement of food specific IgE antibodies which may be useful to diagnose food allergy in atopic dermatitis.

9.Isolation of MLL1 Inhibitory RNA Aptamers

Mixed lineage leukemia proteins (MLL) are the key histone lysine methyltransferases that regulate expression of diverse genes. Aberrant activation of MLL promotes leukemia as well as solid tumors in humans, highlighting the urgent need for the development of an MLL inhibitor. We screened and isolated MLL1-binding ssRNAs using SELEX (Systemic Evolution of Ligands by Exponential enrichment) technology. When sequences in sub-libraries were obtained using next-generation sequencing (NGS), the most enriched aptamers—APT1 and APT2—represented about 30% and 26% of sub-library populations, respectively. Motif analysis of the top 50 sequences provided a highly conserved sequence: 5′-A[A/C][C/G][G/U][U/A]ACAGAGGG[U/A]GG[A/C] GAGUGGGU-3′. APT1, APT2, and APT5 embracing this motif generated secondary structures with similar topological characteristics. We found that APT1 and APT2 have a good binding activity and the analysis using mutated aptamer variants showed that the site information in the central region was critical for binding. In vitro enzyme activity assay showed that APT1 and APT2 had MLL1 inhibitory activity. Three-dimensional structure prediction of APT1-MLL1 complex indicates multiple weak interactions formed between MLL1 SET domain and APT1. Our study confirmed that NGS-assisted SELEX is an efficient tool for aptamer screening and that aptamers could be useful in diagnosis and treatment of MLL1-mediated diseases.

10.Influence of microthread design on marginal cortical bone strain developement: A finite element analysis.

PURPOSE: The present study was aimed to evaluate the level of cortical bone strain during the placement of an implant. The primary concern was to investigate if the extent of overloading area near the marginal bone could be affected by microthread fabricated at the cervical 1/3 of an implant. MATERIALS AND METHODS: Three dimensional finite element analysis was used to simulate the insertion of 3 implants. Control model was 4.1 x 10 mm implant (Submerged model, Dentis Co,, Daegu, Korea) equipped with a main thread only. Type I was with main thread and microthread, and Type II had similar thread pattern but was of tapered body. A PC-based finite element software (DEFORM 3D ver 5, SFTC, Columbus, OH, USA) was used to calculate a total of 3,600 steps of analysis, which simulated the whole insertion. RESULTS: Results showed that the strain field in the marginal bone within 1 mm of the implant wall was higher than 4,000 micro-strain in the control model. The size of bone overloading was 1-1.5 mm in Type I, and greater than 2 mm in Type II implants. CONCLUSION: These results indicate that the marginal bone may be at the risk of resorption on receiving the implant for all 3 implant models studied. Yet, the risk was greater for Type I and Type II implants, which had microthread at the cervical 1/3.
