2.A Case of Sebaceous Adenoma.

Sebaceous adenoma is a benign organoid tumor of proliferating, incornpletely differentiated sebaceous glands. We report a case of sebaceous adenoma in a 15-year-old male. He had a 1.7 x 2.2 cm sized, asymptomatic, srnooth, round, raised, firm, reddish tumor on the left quadrant of abdomen. Histopathologically, the tumor was composed of lobules of irregular size and shape, and sharply demarcated from the surrounding tissue, The most lobules showed undifferentiated germinative and mature sebaceous cells in about equal number. Sebaceous cells in lobules were stained for prekeratin by means of avidin-biotin-peroxidase complex method. Ten months after surgical excision there was no evidence of recurrence.

3.Gallbladder Ascariasis: A case report.

Migration of roundworms, Ascaris lumbricoies, into the biliary tract is a relatively common cause of biliary symptoms in endemic areas, but the number of reported gallbladder ascariasis was scanty even in these areas. Some diagnostic methods might provide suggestive informations, but there are no pathognomonic clinical features for biliary ascariasis, so it is discovered usually after surgical exploration, which is especially the case of gallbladder ascariasis. A young female patient with 4 months' history of right upper quadrant pain which did not respond to medical control and was aggravated recently, presented normal level of peripheral blood leukocyte count with no eosinophilia, normal liver function test, normal serum amylase level and subjectively no febrile sense. Abdominal ultrasonogram disclosed a long, thick, linear, non-motile echogenic structure, not accompanied by acoustic shadowing, within the gallbladder. The intra-and extra-hepatic bile ducts were not dilated and the pancreas was revealed as normal echogenecity and contoru. Endoscopic retrograde cholangiopancreatography was performed immediately thereafter, and it showed a thin linear filling defect in the gallbladder. Cholecystectomy was performed under the impression of gallbladder ascariasis. A mummified Ascaris lumbricoides was found in the lumen of the gallbladder. There was no stones and the histologic examination of the gallbladder revealed chronic cholecystitis.

7.Community screening for stress by using General Health Questionnaire.

This study investigated the stress of community residents in Kwang-Ju and Chonnam areas by using the General Health Questionnaire(GHQ-60) as a instrument of stress measurement. The number of subject were 445 residents who lived in three areas (large city, middle city, and rural area) and they were individually interviewed in March, 1994. The result of study showed that the degrees of stress measured by GHQ-60 were statistically significant in the residents' area, age. sex variables: (a) the residents in middle city among three area had the highest level of stress: (b) the resident who were more than 60 in age had the highest level of stress: (c) the female resident had more stress than male residents: (d) particularly, the residents who were more than 60 years old in the middle city had the highest level of stress. Further, the results of factor analysis showed that there were three factors of social dysfunction, depression and anxiety, and psychosomatic symptom. The social dysfunction factor was statistically significant in both age and resident area variables. The depression and anxiety factor was statistically significant in the residents' area, age. sex variables. The psychosomatic symptom factor was statistically significant in both age and sex variables. The study suggested that they should give a special attention to solve the old people's stress because stress was closely related to residents age.

8.Two Case Reports of Bone-invading Basal Cell Carcinoma.

Basal cell carcinoma is the most common skin cancer, especially on the eyelid and nose. As it rarely invades to a underlying bone or metastasizes an distant site, and is usually found at an early stage, it is regarded as a curative disease. However, basal cell carcinoma on the eyelid and nose may be resected incompletely due to efforts to preserve important structures or as a result of esthetic considerations. We experienced two cases of basal cell carcinoma with local invasion to underlying bone. One was a recurred case on the nasal area extending to the nasal bone arts medial wall of the ethmoid sinus. The lesion was widely resected and covered with a radial forearm free flap. The other was on the eyelid extending to the orbit. It was treated with orbital exenteration and resection of the involved eyelid. The defect was reconstructed with the temporalis muscle flap with split-thickness skin graft. These patients were followed up for 7 months with no evidence of recurrence. Since basal cell carcinoma can invade to the bone and metastasize to a distance site, it should be resected radically in the regions of the eyelid and nose.

10.Obstruction of the lower ureter by obliterated umbilical artyery: report of a case.

The congenital hydronephrosis due to lower ureteral obstruction is not common. In case of congenital obstruction of the lower ureter. the aberrant blood vessels. obliterated vessels or fibrous tissue bands can be the cause of it infrequently. We observed a case of lower ureteral congenital obstruction by obliterated umbilical artery in a 12-yr-old boy. A retrograde pyelogram showed left hydroureteronephrosis with evidence of partial obstruction near the ureterovesical junction. At operation a obliterated left umbilical artery was found compressing the ureter 2 cm. proximal to the ureterovesical junction.
