2.Fluorescein Fundus Angiography in Behcet's Disease.

Since its introduction, fluorescein angiography has been widely used in diagnosing and evaluating treatment of diseases of the retina, uveal tract, and optic nerve. A case with Behcet's disease was studied by means of fluorescein fundus angiography. This, pre summed a retinal angiopathy, has been diagnosed Behcet's disease after developing symptoms of hypopyon iritis, intermittent fever, aphthous stomatitis, and genital ulceration. The fluorescein angiography in Behcet's disease is typically shown a specific finding. Extravasation of the dye waS most conspicuous in capillaries over-lying the optic nerve head, the radial peripapillary capillaries, retinal capillaries proper and retinal veins. Retinal arteriols generally lacked extravasation phenomenon. Edematous swelling of the disc, diffuse turbidity of the posterior fundus and macular cysts are regarded as consquences of the fluorographically confirmed increased permeability of the retinal vessels with resultant edema of the neuropithelium. Pathogenesis of the retinal angiopathy in Behcet's disease and diseases with retinal edema and hyperpermeability were discussed with references. The fluorographic findings thus indicate that the pathological involvement of retinal vessels play a major factor in the diagnosis and prognosis of ocular manifestation of Behcet's disease.

3.A clinical study of cellulitis.

We reviewed the 43 medical records of patients with cellulitis or erysipelas who had been admitted at the depar1ment of dermatology of Seoul National University Hospital and followed them up for mean 21.6 months. The results were as follows. 1. The sex ratio of patients with cellulitis was 1:1.4 and the average age was 49 years. 2. The lower extremity as the most frequently invloved site of cellulitis with the frequency of 58.1% (25 cases), with the head and neck being involved in 16 cases(37.2%) and trunk and upper extremity in 1 case each(2.3% ). 3. The portals of infecticin were suspected in 33 cases(76% ), where tinea pedis was detect,ed in 18 cases(41.9%), previous skin infeection 10 cases(23.3%) and trauma in 9 cases(20.9%). 4. Initial systemic symptoms were fever(76.7%), chill(65.1%) and headache(37.2%). Erythema(100%), local heating(90,7%), tenderness(88.4%), swelling(86.0%), ulcer(16.3%), bulla(14.0%), lymphangitis(9.2%) and lymphadenit,is(9.2%) were found in the skin lesions. 5. The laboratory findings revealed leukocytosis in 35.7%, elevaed ESR in 86.8% positive 6. ASO titer in 63.0% and positive CRP in 80%, Microorganism. were detected in only 1 of 15 blood cultures, in 2 of 4 bulla fluid cultures and in 7 of 9 bus cultures. There was no growth of causative microorganisms in 3 tissue cultures and 9 saline needle aspiration cultures. Penicillin was used as primary antibiotics in 19 cases, which changed to another antibiotics due to lack of improvement in 3 cases and cefazolin was used in 21 cases, which changed in 2 cases due to the same reason. 7. Average admission priod was 11 days(range from 3 to 20 days) and 30.2% of patients wit.h cellulitis experienced recurrence, but 48% on low extrernity in contrast with 6.7% on head and neck.

4.The Ocular Parasitic Inflammations due to Cysticercus Larva.

Localized subconjunctival abscess, episcleritis, recurrent lid abscess, and chorioretinitis are not uncommon diseases of the eye due to various causes such as infective emboli, buried cilia, trauma, autoimmune reactions, or specific infection. The cysticercus cellu]osae has been found in all parts of the eye and the lid, but there are less common presentation of the infection foci than of the cysticercus cellulosae alone. A subconjunctival abscess which is located in the upper fornix of the right eye near the caruncle, episcleritis which is localized in the upper half of the conjunctiva of the left eye, a recurrent abscess of the lid which is located in the right lower lid, a chorioretinitis which is not good visualization because of many cells and flares in the vitreous body, are reported due to a cysticercus larva. No systemic manifestation of cysticercus is found at all. The literature for ocular cysticercosis is referred.

5.A Clinical Investigation of Pustulosis Palmaris et Plantaris.

A clinical investigation of the patients with pustulosis palmaris et plantaris (PPP) was made on 36 patients who were seen at the Department of Dermatology, Seoul National University Hospital, from January 1986 through June 1987. The results were summarized as follows : 1. In this series of 36 patients, 14 patients(38.9%) were male and 22 patients (61.1%) were female. Females are more subject to the affection than males. 2. The mean age of onset in male was 30.6 years and in female it was 38.4 years, indicating a earlier onset in male. The peak age of incidence in male was between 20 and 29 years and in female, between 40 and 49 years. 3. The sites of involvement were mostly bilateral hands and feet. 4. the season of the year did not influence the severity of the skin lesions in 68.9%. 5. Nine patients(28.1%) had psoriasis-like skin lesions and 14 patients(43.1%) had nail changes out of 32 patients with PPP. 6. No direct relationship could be found between pustular eruptions and focal infections, such as tonsilitis, otitis, dental abscess and sinusitis. 7. Remmission of the pustular eruptions could be achieved in 5 out of 10 patients treated with etretinate and in 4 out of 11 patients treated with ketoconazole for more than 4 weeks' treatment

6.Ectopia Cordis Completa Thoracica-Anesthetic Experience with a Case .

A six-hour-old female infant with a complete, thoracic ectopic heart underwent left antero-lateral thoracotomy, division of left superior vena cava, left lower lobectomy, and reduction of heart into the lobectomied space under endotracheal fluothane-oxygen anesthesia employing infant circle absorber. Despite all supporting measures available, her condition progressively deteriorated with increasing severity in bradycardia and cyanosis resulting in cardiac arrest three hours after the completion of surgery. Anesthetic management and cause of death have been discussed together with the case report.

7.Experimental Study on the Effect of Unilateral Cryptorchidism on the Contralateral Scrotal Testicles.

An experimental study was made to investigate the fertility of unilateral cryptorchidism in rat, Twenty-six male Sprague-Dawley rats were treated to create unilateral mechanical cryptorchidism at 5 weeks of age and both testicles were examined microscopically at 13, 19 and 25 weeks of age. Testicular volume, tubular diameter and spermatogenic index were subjected to statistical analysis. No significant differences were found in testicular volumes of the contralateral scrotal testicle (P>0.05 ), whereas that of the intraabdominal testicle was greatly decreased (P <0.001). Found was considerable decrease in contralateral scrotal testicles in terms of spermatogenic, index and tubular diameter (P <0.01), in addition to significant changes of intraabdominal testicle (P<0.001). Severe histological change depicted by thickening of the basement membranes of seminiferous tubules and peritubular hyalinization were noted in the intraabdominal testicles while contralateral scrotal testicle showing a mild to moderate changes. In conclusion, one can state that spermatogenesis is decreased in both intraabdominal as well as contralateral scrotal testicles, the severity of the former being greater than the latter.

8.Antinociceptive Effects of Intrathecal Morphine and NMDA Receptor Antagonists, MK 801 and CPP, on the Formalin Test in Rats.

BACKGROUND: It is emphasized that repetitive stimulation of small diameter afferent fibers produces a progressive increase in the action potential discharge and a prolonged increase in the excitability of neurons in the spinal cord following the stimulus and that this facilitatory component has a unique pharmacology. To investigate the behavioral parallels of this spinal facilitation, we evalusted the antinociceptive effects of intrathecal morphine, N-methyl-D-aspartate(NMDA), (+)-5-methyl-10,11- dihydro-5H-dibizo(a,d) cycloheptene-5, 10-imine hydrogen maleate(MK801) and (+/-)-3-(2-carboxy- piperazine-4-yl)-propyl-I-phosphonic acid(CPP), on the formalin test in rats. METHODS: Four to six days after chronic lumbar intrathecal catheterization, normal saline, morphine(0.1 to 30 ug), MK801(0.1 to 10 ug), CPP(0.1 to 5 ug) or NMDA(10 or 100 ng) were administered intrathecally before formalin injection. Spontanesous flinches were observed at 1-2 and 5-6 min(phase 1) and at 10 min intervals thereafter for 50 min(phase 2) after subcutaneous formalin injection into the dorsum of the right hind paw for each drug treated rats. RESULTS: Intrathecal morphine produced dose dependent inhibition of the phase 1 and phase 2 response(ED50=0.63 ug and 0.37 ug, respectively). Intrathecal MK801(0.1 to 10 ug) and CPP(0.1 to 5 ug) inhibited the phase 2 response more strongly than phase 1 response and inhibition of the phase 2 response(P<0.05 at any dose) was dose dependent(ED50=0.54 ug for MK801 and 0.15 ug for CPP). 1ntrathecal NMDA(10 or 100 ng) produced augmented responses in the intermediate and phase 2(P<0.05), but had no effect on the phase 1 response(P>0.7). Relative potencies of MK801 and CPP when compared with morphine were 1.34 and 0.41, respectively. CONCLUSIONS: This study suggests that intrathecal morphine and NMDA receptor antagonist(MK801 and CPP) have an antinociceptive effect on pathological pain mediated by central sensitization and that NMDA receptor antagonists can be utilized selectively in the treatment of components of central sensitization.

10.Hypospadias: Recent Experience of 17 Cases.

There is no one good method for all hypospadias repairs. A technique must be adapted for each individual patient: therefore, the urologist ought to be proficient in performing a number of procedures in order to be prepared for all possible eventualities. And one-stage operations are preferable whenever they are judged to have a reasonable chance for success. Herein we report hypospadias repairs on 17 patients between August, 1985 and July, 1986. In 7cases of two stage operations such as Belt-Fugua and Thiersch-Duplay tube with glans channel, the success rate was 57%. In 10 cases of one stage operations such as Mathieu, King and Duckett, the success rate was 50%."
