1.Alcoholic Type Cirrhosis Following Side to Side Ileo-Transverse Colon Anastomosis.

A case of micronodular cirrhosis of the alcoholic type developed following an intestianl bypass surgery in a 47 year-old nonalcoholic male patient is presented. The patient denied any drug use of a long duration and had no diabetes mellitus. Five years before, a side to side ileo-transverse colon anastomosis had been performed for perforated intestinal tuberculosis at 1 m proximal to the ileocecal valve, bypassing a short segment of ileum (about 1.5 m) and transverse colon. The ileum distal to the perforated site had been found completely stenosed. He was severely lean with evidences of nutritional deficiency such as low serum levels of the albumin and vitamin B12. The liver biopsy showed a fatty change, Mallory bodies and perihepatocellular collagenosis within the cirrhotic nodules. The present case suggests that, when there are blind loop formation and nutritional deficiency, hepatic changes identical to those following jejunoileal bypass could develop even by reduction of a shore segment of the small intestine.

4.Immunohistochemical Study on Cytokeratin Expression in Tracheal Epithelium of Human Fetus.

Trachea is lined by a pseudostratified epithelium which usually expresses a complex mixture of stratified as well as simple epithelial-type cytokeratins. In the present work, the cytokeratin expressions was studied immunohistochemically in the tracheal epithelium and gland of human fetus at 14, 26 and 32 weeks of gestation. The primary antibodies used were CK7, 8, 10, 14, 18, AE8, 5D3, MNFl16 and AE3. In PAS-hematoxylin stain, the tracheal eithelium was composed of pseudostratified ciliated columnar type and consisted of surface, intermediate and basal layers regardless of gestational ages. The PAS positive cells, however, were decreased in number in proportion to gestational ages. The tracheal gland was not fully differentiated at 14 weeks of gestation, and had well differentiated secretory portions consisting mucous and serous cells at 26 and 32 weeks of gestation. The mucous cells and luminal border of the duct were positive for PAS stain. The tracheal eithelium showed different immunoreactivity between cartilageous and membranous portions. In general, CK7 and 5D3 were expressed in surface cells, AE8 in intermediate cells, and MNFl16 and AE3 in the cells of all layers. At 14 weeks of gestation, the tracheal epithelium immunoreacted for CK7, AE8, 5D3, MNFl16 and AE3. The premordium of tracheal gland was positive for 5D3, MNFl16 and AE3. The tracheal epithelium at 26 and 32 weeks of gestation showed same staining properties to those at 14 weeks of gestation. The duct cells at 26 weeks of gestation were immunoreactive for CK7, 8, 14, 18, AE8, 5D3, MNFl16 and AE3, and those at 32 weeks of gestation were immunoreactive for CK7, 14, 5D3, MNFl16 and AE3. The acinar cells at 26 and 32 weeks of gestation were positively stained for CK7, 8, 18, 5D3, MNFl16 and AE3. These results suggest that CK7 and 5D3 may serve as useful markers for mature cilated cells, AE8 (CKl3) for immature ciliated cells, and CKl4 for duct cells in tracheal epithelium and gland.

5.Spectral Analysis of REM Sleep EEG in Narcolepsy and REM Sleep Behavior Disorder.

INTRODUCTION: It has been proposed that narcolepsy and REM sleep behavior disorder (RBD) have overlapped symptom profile and pathophysiology. This study was aimed at measuring and comparing changes in EEG frequency band of REM sleep in narcolepsy and RBD, applying EEG spectral analysis method. METHODS: Nine patients diagnosed as narcolepsy and the same number of RBD patients were studied. Spectral analysis of the REM sleep EEG was performed in each patient on 9 epochs selected evenly from the first, second, and third REM periods. Then, we compared frequency band percentages of REM sleep EEG in narcolepsy and RBD. RESULTS: Narcolepsy patients had significantly higher delta frequency ratio than RBD ones (p=0.00). In alpha and beta2 frequency bands, RBD patients showed higher percentage than narcolepsy ones. Slow wave sleep was more prevalent in narcolepsy patients. But, no difference of REM sleep percentage was found between the two groups (p=0.93). CONCLUSION: Higher delta frequency ratio in REM sleep of narcolepsy patients than RBD ones reflects that sleep-promoting mechanism is more dominant in narcolepsy than in RBD.

7.A Case of Generalized Perforating Granuloma Annulare.

Generalized perforating granuloma annulare is a rare variant of granuloma. annulare, which was first described by Duncan et al 1) in 1973. A 21-year-old female had hundreds of 3 to 5mm papules over the whole body, which were mainly distributed in the extremities and trunk since three months ago. Individual papules developed a central umbilication with a peripheral collarette of scale. Histopathologic examination showed an epidermal perforation and transepidermal elimination of necrobiotic material. The epidermal perforation communicated with areas of necrobiotic collagen surrounded by palisading granulorna in the dermis. We treated her with oral prednisolone and dapsone, but there was no clinical improvement.

9.A Study on the Limb Lengths Following Femoral Shaft Fracture in Children

Growth acceleration following femoral shaft fracture occasionally results in a significant leg-length inequality with limp. The ability to predict subsequent overgrowth would enable the surgeon to compensate for growth acceleration by providing the appropriate overriding of the fragments before the time union. The purpose of this study was to establish principles which would aid in predicting over growth. Between 1972 May and 1983 September, 115 inward patients who were 16 years old or the younger were treated in the Department of Orthopaedic Surgery of Hanyang University Hospital. In these cases, we analysed the causes of fractures, associated injury and methods of treatment. And also, by use of mentgenogram, evaluated on site, shape and degree of overriding of fractures. And then, the degree of overgrowth of bone was compared with unaffected site and analized by Bell Tompson's split orthoroentgenogram. 1. The average tibial overgrowth after fracture of femoral shaft is 2.2 mm in length. 2. The average femoral overgrowth after fracture of femoral shaft is 9.4 mm in length.
