Congenital anomaly of the vas deferens is often easily diagnosed by careful examination of the scrotum as well as by present popularity of vasectomy. By the fact that both systems originate from a common urogenital ridge of mesoderm, anomaly of the kidney has long been recognized to coexist with anomaly of the vas deferens. So, inability to palpate the vas deferens in routine examination or absence of vas deferens in vasectomy should alert the physician to study the patient for associated renal anomaly. We report one case of vas anomaly associated with ipsilateral renal hypoplasia with the brief review of literatures.

2.A Case of Reticulate Acropigmentation of Kitamura.

Reticulate acropigmentation of Kitamura, described first by Kitamura and Akam atsu in 1943, is a pigmentary disorder that may be determined by an autasomal dominant gene. Skin lesions are characterized clinically by depressed brownisL pigmented spots, in a reticulated pattern, on the extensor surface of the acral parts of the extremities. Most reported cases are confined to Japanese. We report herein a case of reticulate acropigmentation of Kitarnura in a 28- year-old woman, with a depressed brownish pigmented spots, in a reticulate pattern, on the dorsum of hands and left foot. The authors make the diagnosis of this case a reticulate acropigmentation of Kitamura for the clinical symptoms and histopathologic findings.

3.Cutaneous Plasmacytosis with Multiple Nodular Eruptions and Polyclonal Hypergammaglobulinemia.

We report two patients with multiple peculiar skin eruptions and polyclonal hypergammaglobulinemia. Both patients visited our hospital for the evaluation of asymptomatic multiple nodular eruptions on almost their entire body except for the lower extremities. Histologic examinations disclosed prominent infiltration of plasma cells and lymphoid follicular hyperplasia in the dermis but these plasma cells showed neither a mitotic figure nor atypicalities. Laboratory examinations showed polyclonal hypergammaglobulinemias and increased erythrocyte sedimentation rates. In spite of various investigations, the cause of the hypergammaglobulinemia remained obscure.

4.Biodegradable Internal Fixation For Displaced Non: Comminuted Malleolar Fracture.

Displaced fracture of the ankle are probably the most common skeletal injury managed today by open reduction and internal fixation. But, it is recommended that metallic implant might be removed to restore normal biomechanical forces on hone and to reduce discomfort directly under the skin. in order to reduce the resources needed for the removal of metallic fixation devices, absorbable impiants of biodegradable synthetic polymers were deveioped. However reports of clinical application of such implants have so far been very few. This prospective study represents the good resuits in 15 displaced non-comminuted closed malleolar fractures of the ankle treated using hiodegradahle internal fixation in department of orthopedic surgery of Kwang Myung Sung Ae general hospitai from June I 995 to December 1996. The results were as follows 1. All cases were united and the average time for union was 7.6 weeks. 2. As a results of comparison with normal side, a restriction of 10 degrees or more of dorsiflexion of the ankle joint was present in 4 cases, a restriction of 20 degrees or more of plantar tlexion was present in 2 cases, a restriction of supination was present in 3 cases and a restriction of plantar flexion was present in 4 cases. 3. According to Phillips functional scoring scale, 4 cases were excellent, 7 cases good, 4 cases fair and no cases were poor. 4. There were no complications in all cases.

5.A Case of Charcot-Marie-Tooth 1A Showing Atypical Clinical and Pathological Features.

A 7 year-old girl presented with generalized muscle weakness and delayed motor development. She was able to stand up at 15 months and began to walk at 4 years of age. A nerve conduction study showed severe demyelinating neuropa-thy .There was no family history of peripheral neuropathy, and her parents and younger brother were clinically and electrophysiologically normal. A sural nerve biopsy showed moderate loss of myelinated fibers with onion-bulb forma-tions. Many teased nerve fibers revealed typical tomaculous changes. However the molecular genetic study of the patient confirms the duplication of 17p11.2-p22 on a polymerase chain reaction using D17S261 as a primer but not in her parents.

9.The Effect of Anoxia and Reoxygenation on the Production of Prostaglandin I2 from the Endothelial Cells of Umbilical Vein

The successful revascularization and reperfusion of ischemia are still associated with high systemic complication rates and severe local tissue injuries. The morality rates after revascularization have been reported to range from 10% to 20% and the amputation rates from 12% to 22%. It is well recognized that the microvasculature is highly sensitive to ischemia-reperfusion (I/R) and that the initial damage of endothelial cells contributes to I/R-induced tissue injury. In an effort to define the mechanisms responsible for reperfusion-induced vascular injury number of in vitro models have been developed to stimulate the responses of endothelial cells to I/R. Because of its simplicity, many investigators have used monolayers of cultured endothelial cells exposed to anoxia and reoxygenation as a model system to minic I/R-induced vascular changes in vivo. The endothelium serves as an important modulator of vascular homeostases by secreting various levels of both thrombotic and antithrombotic agents. One of the important product of endothelial cells, prostaglandin I2 or prostacyclin (PGI2) helps to maintain hemostasis through its involvement in coagulation, platelet activation, leukocyte migration and adhesion, vascular tone regulation and growth control. PGI2 synthesis is a readily quantifiable index of endothelial cell perturbation and thus serves as a marker for the identification of injurious stimuli. Endothelial cells were isolated from human umbilical vein and cultured in M-199 medium plus 20% fetal calf serum. Purity of culture was determined by immunological fluorescent staining of factor VIII related antigen, phase-contrast microscopy. TRK 790 radio-immunoassay kit was used for the measuring of 6-keto-PGF1 alpha released by endothelial cells. The results were as follows: 1) The concentration of PGI(2) released from the cultured endothelial cells was 33.44 +/- 2.26 pg/1 105 cells/mL 2) Incubation of endothelial cells with anoxia and reoxygenation resulted in PGI(2) release of 42.98 +/- 2.29 pg/1x10(5) cells/ml and 62.44 2.11 pg/1 105 cells/ml, respectively. 3) Incubation of endothelial cells with allopurinol (20 mumol/L) decreased the PGI(2) release to 40.68 +/- 2.99 pg/1x10(5) cells/ml. In conclusion, our data showed that the damage of endothelial cells in reoxygenotion group was significantly increased comparing anoxia group (p<0.005) and that allopurinol can inhibit reoxygenation-induced injury of endotheial cells.

10.Sulfidoleukotrienes Production in Blood Leukocytes Stimulated by Mite Allergen.

PURPOSE: Although asthma is the most common chronic disease in childhood, accurate diagnosis in infants and young children remains challenging clinicians. Allergen challenging tests in vitro have been used productively as an investigative tool in studies of the pathophysiology and diagnosis of asthma. Therefore, we compared the sulfidoleukotrien (sLT) concentration according to allergen leukocyte stimulation test, in normal versus asthmatic patients, to find better diagnostic tools. METHODS: From May through August, 1998, nine children were enrolled who presented positive skin reaction in Dermatophagoides pteronyssinus (D.p.), Dermatophagoides farinae (D.f.) as patient groups. We measured total eosinophil count, serum IgE, sLT concentration of three different allergen stimulation (100 ng/ml, 10 ng/ml, 1 ng/ml). RESULTS: sLT concentration in three different D.p., D.f. stimulation showed significant differences (P<0.01). Allergen concentration of 10 ng/ml was fit for stimulating peripheral leukocyte. The sLT concentration is correlated with IgE, total eosinophil count, but not with age. Actual concentrations of sLT was not measured in allergen stimulation test. Its interpretation of test results was complicated by the fact that several variants were involved in determining sampling time and appropriate sampling volume. Most importantly, the diagnostic sensitivity of the sLT concentration tests varies directly with the magnitude of IgE antibody and total eosinophil count. CONCLUSION: We emphasizes the role of allergen challenge in understanding the pathophysiology of young children asthma. It focuses on more accurate diagnosis with objective techniques for analyzing the leukocyte sLT release to antigen.
