1.A Case of Generalized Perforating Granuloma Annulare.

Generalized perforating granuloma annulare is a rare variant of granuloma. annulare, which was first described by Duncan et al 1) in 1973. A 21-year-old female had hundreds of 3 to 5mm papules over the whole body, which were mainly distributed in the extremities and trunk since three months ago. Individual papules developed a central umbilication with a peripheral collarette of scale. Histopathologic examination showed an epidermal perforation and transepidermal elimination of necrobiotic material. The epidermal perforation communicated with areas of necrobiotic collagen surrounded by palisading granulorna in the dermis. We treated her with oral prednisolone and dapsone, but there was no clinical improvement.

2.Orthodontic consideration of cleft lip and palate (Report 1).

The role of the Orthodontist in cleft lip and cleft palate therapy is primarily ill correction of malocclusion which is required by practically every child who has these defects. He can contribute to the assessment of dento-facial growth and development. We may gain the possible limited correction of delayed malocclusion due to cleft lip and palate. The authors have attempted delayed orthodontic treatment of a cleft lip and palate of 12.9 years old girl, who had a cleft lip and palate of surgical closure at 2,3 and 4 years old.

3.A Case of Malignant Melanoma on the Hypothenar Region of Right Hand: Case Report

Most of malignant melanoma of skin had occurred in transformation from benign nevi(Webster, 1944), especially most frequently from junctional nevi of them. which was characterized by flat surface, dark-brown color and hairlessness(Ackermen, 1948) Blumberg (1972) introduced five levels of hietologic invasion of malignant melanoma described by Clark et al. Morton(1970) reported various measures of treatment such as surgery combined with radiotherapy or with chemotherapy or with recent immunotherapy with good result. The authors recently experienced 24-yeas old male soldier patient with malignant melanoma on hypothenar region of right palm. The pathologic investigation including electronmicroscopical aspect was carried out and are presented. Treatment consisted of the resections of right axillary lymph nodes and the lesion in situ on the hypotherar region of right hand, pedicle skin graft to right hand from abdomen, and immediate postoperative immunotherapy with intradermal injection of 1.0 cc. of B.C.G. daily on extensor surface of both thighs in turn, which lasted for about 3 months duration to find good result.

5.A Study on Circulating Immune Complexes in Leprosy Sera.

Recent studies indicated that cellular and humoral immune reactions against Afycobacterium leprae may be involved in the development of some of the pathological manifestations. In lepromatous leprosy one might anticipate the formation of immune complexes from concomitant occurrence of large amounts of mycobacteria and corresponding antibodies. Indeed, there are some evidences of an involvement of immune complexes in the pathogenesis of erythema nodosum leprosum(ENL), mainly based on clinical and pathological studies. Some of numerous immune cornplex tests have been applied for the study of leprosy, But no single test has yet been shown to be the rnost sensitive and the most specific for the entire spectrum of circulating immune complexes. Furthermore. Leprosy has been found to differ in its clinical manifestations and immunological behaviors in different geographic areas and races. The authors considered it is necessary to determine the circulating immune complexes in Korean patients with leprosy to evaluate the immune status and the differences in the immunologic reactivity between the lepromatous and the tuberculoid spectrum of Ieprosy.

6.Long-term Follow-up Astigmatic Changes in Cataract With or Without Suture.

We performed an analysis of corneal astigmatism of 123 eyes with suturing(sutured group) and 56 eyes without suturing(sutureless group) after phacoemulsification and PCL implantation through 7.0 mm straight incision wound 2.0-2.5 mm from superior limbus by use of algebraic and vector methods for two years following surgery. In both groups, the amount and axis of astigmatism were continued to change for 2years(p>0.05). The amount of astigmatisms in sutureless group was less than in sutured group until 2 months postoperatively(p<0.01), no significant difference from 3 months to 6 months postoperatively(p>0.05). Astigmatic changes in sutureless group were stable and had lower against-the-rule shift than the sutured group after 6 months postoperatively(p<0.01). Surgically induced astigmatism was stabilized after 1 month postoperatively in sutured group and after 2 weeks psotoperatively in suture less group(p<0.01), and it was lower in suture less group than sutured group during the two years postoperatively.(p<0.01).

7.Clinical Comparison of Neonatal Urinary Tract Infection Caused by Klebsiella pneumoniae Versus Non - klebsiella pneumoniae.

PURPOSE: The purpose of this study was to describe the clinical characteristics of neonatal urinary tract infection (UTI) caused by Klebsiella pneumoniae and non- Klebsiella pneumoniae UTI. METHODS: We compared clinical characteristics of 84 neonatal patients with UTI caused by Klebsiella pneumoniae who were hospitalized at the Department of Pediatricsat Han Dong University, Sunlin Hospital during the period between May, 1994 and August, 1998. The cases were divided into two groups depending upon causative organisms' Klebsiella pneumoniae UTI vs non-Klebsiella pneumoniae UTI, and the clinical characteristics of these groups were compared. RESULTS: Escherichia coli was the most common bacterial pathogen causing neonatal UTI, followed by Klebsiella pneumoniae. There was no significant difference in the sex distribution of Klebsiella pneumoniae UTI, but non-Klebsiella pneumoniae UTI showed male predominence. There were no significant differences in the incidences of hematologic, urologic, radiologic findings and perinatal complications in between these 2 groups. CONCLUSION: Klebsiella pneumoniae is the second most common pathogen causing neonatal UTI. There were no specific differences in the laboratory, symptomatologic, and radiologic findings in these two groups.

Sacral agenesis is a rare condition. This syndrome was first described by Hohl in 1852 and is usually associated with other anomalies such as vertebral deformities, deformed hypoplastic legs, urologic and gastrointestinal problems. We have experienced two cases of this deformity; one with partial sacral agenesis, and the other with partial lumbar agenesis and complete absence of sacrum.

10.Angular Positioning of Stent Increases Bowel Perforation after Self-Expandable Metal Stent Placement for Malignant Colorectal Obstruction.

BACKGROUND/AIMS: Some factors like stent wires, balloon dilatation and use of guide wires seems to increase perforation after self-expandable metal stent (SEMS) placement, but few studies mentioned about the relationship between angulation of malignant stricture and perforation. The present study aimed to confirm that more angular positioning of stents increases perforation. METHODS: This study was conducted with retrospectively evaluation at Digestive Disease Center, CHA Bundang Medical Center, CHA University. Between January 2002 and August 2011, SEMS was inserted in 130 patients with malignant colorectal obstruction. We studied the difference in the angle of stenosis between perforation and non-perforation groups using fluorography images. RESULTS: SEMS insertion was performed in 130 cases of obstruction due to colon cancer. Perforation occurred in eight patients (6.2%) of them. Thirteen cases were excluded from the analysis due to poor fluoroscopic images. Among the eight patients with perforation, the mean stenosis angle was 109.9degrees compared to 153.1degrees in the nonperforation group, indicating that the angle was more acute in the perforation group (p=0.016). CONCLUSIONS: This study shows that more angular positioning of stent increases bowel perforation after SEMS placement for malignant colorectal obstruction.
