2.EFFECTS OF AGING AND THERMAL CYCLING ON THE BIAXIAL RLEXURE STRENGTH OF VENEERING RESIN COMPOSITES FOR CROWN.

This study was performed to evaluate the effect of aging and thermal cycling on the biaxial flexure strength of four commercially available veneering resin composites for crown(Dentacolor : DC, Artglass : AG, Esternia : ET and Targis : TG). Disc specimens were fabricated in a teflon mold giving 12mm in diameter and 1mm in thickness. All samples were divided into 4groups. Group 1 was dried in a dessicator at 25degrees C for 30 days. Group 2 was immersed in distilled water at 37degrees C for 30 days. Group 3 was immersed in distilled water at 65degrees C for 30 days. Group 4 was subjected to 10,000 thermal cycles between 5degrees C and 55degrees C, and the immersion time in each bath was 15 seconds per cycle. Biaxial flexure test was conducted using the ball-on-three-ball method at the cross head speed of 0.5mm/min and fracture surfaces were observed with scanning electoron microscope. The results obtained were summarized as follows ; 1. Weibull modulus values, except for the AG group, decreased after thermal cycling treatment. 2. Biaxial flexure strength values of aging group at 37degrees C were th lowest in all sample groups. Except for the DC group, strength values were significntly decreased for the drying group. 3. After thermal cycling test, the highest value of biaxial flexure strength of 188.8 MPa was observed in the ET group and the lowest value of 73.2 MPa was observed in the DC group. The strength values showed the significant differences in each group (p<0.05). 4. Observation of surfaces after thermal cycling test revealed the ditching in the part f surrounding large fillers.

The application of fine needle aspiration (FNA) cytology to the soft tissue tumors had been neglected. In recent years, however, FNA has been used increasingly in the preoperative diagnosis of these tumors due to its usefulness and accuracy. We present 3 cases of liposarcoma, myxoid, myxoid with round cell, and pleomorphic, diagnosed by FNA cytology with histologic confirmation. Good correlation between his- tologic and FNA cytologic findings was found. Although the cytologic appearances of liposarcomas varied with histologic type, the main criterion was the presence of atypical multivacuolated lipoblast with characteristically scalloped nuclei.

8.Magnetic Resonance Imaging for the Evaluation of Acute Posterolateral Complex Injuries of the Knee.

The purpose of this study is to determine whether magnetic resonance imaging(MRI) can assist in deci- sion-making in the treatment of acute posterolateral complex injuries. We reviewed retrospectively seven patients with acute posterolateral complex injury, among the 79 cases that had taken cruciate ligament recon- struction. MRI findings were then correlated with the results of examination under anesthesia or operi lateral reconstruction. Based on surgical and clinical findings, there were six complete and four partial posterolateral complex injuries. MRI was able to accurately describe the extent of injury in each case. All ten patients had concomitant anterior cruciate ligament tears, and seven were three posterior cruciate ligament injuries. The iliotibial band was avulsed from Gerdys tubercle in seven patients. In knees with complete tears, separation of the coronary ligament of the lateral meniscus was noted on MRI. A characteristic bone contusion on the anteromedial femoral condyle was present in nine patients. Popliteal tendon tearing was observed in eight cases, and biceps femoris was torn in six patients. In knee with complete tears, tears of the lateral collateral ligament complex was noted both on MRI and intraoperatively. A high-quality MRI in patients sustaining these injuries will assist the surgeon in directly applying surgical techniques in a timely fashion.

9.Nephrotic syndrome under 2 years of age.

There had been total 20 patients with early onset(4 months~2 years) primary nephrotic syndrome in the Department of Pediatrics, Seoul National University Children's Hospital, during the period from March 1987 to February 1993. We analysed clinical courses, response to treatment, pathological findings and prognosis of the patients And the results were as follows; 1) The initial responders to steroid treatment were 10(50%), of whom 3 became late nonresponders. Of the 10 initial nonresponders, 8 revealed continuing nonresponsiveness. 2) Incidence of hypertension was significantly higher in the intial nonresponders than in the initial responders. 3) Renal biopsies were performed in 9 initial nonresponders and 2 late nonresponders. And the results were 6 with minimal change lesion, 4 with focal segmental glomerulosclerosis and 1 with mesangiocapillary glomerulonephritis. 4) Six continuing or late nonresponders received methylprednisolone pulse therapy or cyclophosphamide. And partial remission was induced in 4 of them. 5) The overall 3 year maintenance rate of normal renal function was 81%(100% in the initial responders and 64% in the initial nonresponders). In conclusion, the initial steroid responsiveness and prognosis were poorer in patients with early onset nephrotic syndrome than in patients with usual childhood nephrotic syndrome.

10.Therapeutic Effects of Imipramine and DDAVP in Enuretic Children: A Randomized Prospective Study.

PURPOSE: The therapeutic effects of imipramine and DDAVP in primary monosymptomatic enuretic children were compared using a randomized, prospective study. MATERIALS AND METHODS: The study subjects consisted of 23 children, with a mean age of 7.9 years, complaining of monosymptomatic nocturnal enuresis. Baseline evaluations, at the first visit, included voiding diary, enuresis diary, uroflowmetry, urine osmolality, and urine and serum osmolality after a water deprivation test and electrocardiography. After the administration of medication, the changes in the enuretic episodes and adverse effects were evaluated every month. RESULTS: Sixteen children received medication, and after 2 weeks the drug response rate was 57.1% (4/7) in the imipramine group and 33.3% (3/9) in the DDAVP group. Ten of the 16 children completed the study. After 12 weeks of study, the response rate was 75% (3/4) in the imipramine group and 50% (3/6) in the DDAVP group. There were no significant differences in mean urine osmolalities and functional bladder capacities between the response and no-response groups. During the study, side effects developed in only three of the children in the imipramine group. Of these, one demonstrated tachycardia on a follow-up electrocardiography and another complained of irritability, insomnia, a dry mouth and blurred vision. Subsequently, the two children dropped out due to these side effects. CONCLUSIONS: The imipramine and DDAVP had similar therapeutic effects, but the DDAVP had fewer side effects, and was better tolerated, than the imipramine.
