1.Entrapmental Paralysis of the Deep Branch of the Ulnar Nerve at the Mid-palmar Space caused by a Carpal Ganglion: A Case Report

The ulnar nerve entrapment at the wrist is usually caused by carpal ganglion, occupational neuritis, ulnar artery disease, fractures of the carpal bones, tumors, rheumatioid arthritis, etc. The ganglion is the most common cause of the distal ulnar nerve entrapment. A fort-two years old woman complained of insidious motor weakness of the left hand. The electromyogram revealed distal ulnar nerve palsy. On examination, her sensibility of affected hand was normal; there were no Tinel's sign and palpable mass on the Guyon's canal and palm; there was obvious wasting of all the interossei. On surgical exploration, the deep branch of the ulnar nerve was compressed by a ganglion at the mid-palmar space, not in the Guyon's canal. Four months after removal, the clawing of the 4th and 5th fingers disappeared, and the pinch power of the left hand recovered normally seventeen months later.

2.The Surgical Treatment of The Old Nerve Injury

During recent 3 years, the authors had treated surgically 31 patients who have had old nerve injuries. 19 patients(21 nerves) of them could be followed for more than 1 year after operation. Of these patients, children were 5 and adults were 14. The elapsed time from the injury were from 3 weeks to 20 months(18 cases under 6months and 3 cases above 6 months). The methods of operation were epineural suture(4 cases), grouped interfascicular suture(8 cases), neurolysis(8cases) and nerve graft(1 case). There were satisfactory or good results in 9 nerves of the 21 old nerve injuries. In 4 of 5 children and 5 of 16 adults, good results were obtained. There were 2, 6 satisfactory or good results in 4 epineural sutures and 8 grouped interfascicular sutures and all poor results in 8 neurolyses. One case treated with nerve graft with sural nerve was showed good result. 9 cases of 18 old injuried nerves under 6 months from injury were good results. All 3 cases over 6 months from injury were poor results.

4.Organotypic Culture of HaCaT cells: Use of Dermal Substrate that Combines de-epidermized Dermis with Fibroblast-populated Collagen Matrix.

BACKGROUND: The immortalized human keratinocyte line, HaCaT cells have been widely used as substitutes for normal epidermal keratinocytes. Recently, reconstruction of a skin equivalent using HaCaT cells showed a multilayered epithelium,but somewhat different tissue architecture as compared with normal epidermis. OBJECTIVE: In this study, using HaCaT cells we tried to reconstruct an epidermis resembling more closely to normal epidermis than the previous results. MATERIALS AND METHODS: HaCaT cells were cultured in air-liquid interface on a recently developed dermal substrated in our laboratory, de-epidermized dermis (DED) raised on fibroblast-populated collagen matrix and the result was compared with those on DED or fibroblast-populated collagen matrix alone. RESULTS: HaCaT cells on the new dermal substrate formed a multilayered epithelium with rete ridges, showing rather orderly cellular organization compared with those on fibroblast-populated collagen matrix. However, horny and granular layers were not observed contrary to normal epidermis. Immunohistochemical studies revealed that differentiation markers such as keratin 1, keratin 6 and involucrin showed the similar pattern to those in HaCaT cells cultured on fibroblast-populated collagen matrix. Markers of terminal differentiation, loricrin and filaggrin were not expressed contrary to normal epidermis. CONCLUSION: These results suggest that organotypic culture HaCaT cells on the dermal substrate combines DED with fivroblast-populated collagen matrix results in incomplete differentiation of HaCaT cells contrary to normal keratinocytes.

5.A Study on Volume and Acidity of Gastric Juice Related to Fasting Time.

Background: Gastric juice volume and acidity are influenced by food, drugs, and patient factors such as age, sex, weight, and fasting time. But almost studies were performed without controlling of these confounding variables. This study was designed to determine the change of gastric juice volume and acidity in relation to the fasting time within a constant food intake. Methods: Fifty-seven healthy patient randomly allocated to one of two groups. Group I was instructed to ingest 200 ml of milk up to 8 hours before anesthesia and Group II was instructed to drink 200 ml of water up to 1~2 hours before anesthesia. After the endotracheal intubation the volume and acidity of gastric juice were measured. Relationships between gastric volume or acidity and fasting time were analysed using correlation analysis. The data were analyzed using Wilcoxon rank sum test, comparing Group I with Group II. Results: There is no significant relationship between gastric juice volume or acidity and the fasting time within the same food intake. Gastric juice volume and acidity of the patients who ingested milk at 8~9 hours before anesthesia were similar to that of those patients who drank water at 1~2 hours before anesthesia. Conclusion: It is necessary to individualize the fasting time of the patients and it seems that water ingestion up to 1~2 hours before anesthesia may be safe.

6.A Case Dermal Melanocyte Hamartoma.

A 26-year-old man presented with a bluish speckled patch on the dorsum of the left hand. It had apperaed at birth as a bluish spot. Since adolescence the lesion extended and at the time of visiting hospital it covered the most part of the dorsum of the left hand and even some palmar aspect. The histopathological and ultrastructural examination revealed numerous dermal melanocytes, mainly in the upper dermis. Clinically and pathologically, the patient was diagnosed as having dermal melanocyte hamartoma, a distinct type of dermal melanocytosis.

8.A Case of Lenear IgA Bullous Dermatosis.

We report a 27-year-old male with rnultiple pruritic papulovesicular eruptions on the whole body without any gluten-sensitive enteric syrnpoms. A skin biopsy specimen showed subepidermal vesicles and papillary microabscesses infiltrated with rnany neutrophils and a few lymphocytes and eosinophils. Direct immunofluorescent study of the perilesional skin revealed linear deposits of immunoglobulin A along the dermo-epidermal junction. The lesion responded rapidly to oral dapsone and topical steroid therapy.

9.Two Cases of Epithelioid Sarcoma with Immunohistochemical Study.

Two typical cases of epithelioid sarcoma were examined by immunohistochemical stain using antibodies to epithelial membrane antigen, carcinoembryonic antigen, vimentin and cytokeratin. Both cases showed positive reactivity for the four kinds of antibodies. These results point to the fact that epithelioid sarcoma simultaneously expresses epithelial markers and characteristic mesenchymal phenotypes. Epithelioid sarcoma appears to be a tumor derived from a multipotential mesenchymal cell with multidirectional differentiation.
