1.Women's Practice and the Result of Pap Smear.

BACKGROUND: Pap smear is well known as a useful method for early detection of the cervical cancer and known to be widely performed by women. But the cervical cancer is the most common cancer in Korean women until now. So we surveyed to find out the status of womens practice and the result of Pap smear. METHODS: A questionnaire was filled up by the 270 women who had taken Pap smear from June to July in 1994 at the department of family medicine of Kyung Hee Medical Center. The family physicians gynecological findings at Pap srnear and the results of Pap smear were obtained. Af ter then we analyzed the data by SAS(Statistical Analysis System). The data which were not answered at each item were excluded in the analysis of each item. RESULTS: Among the 270 women, thirties were 98 persons(36.3%), forties were 115 persons(42.6%). The women who had never been taken Pap smear before were 34.6%. At the age performed Pap smear first, 30-39 years old was most common as of 39.8%. In the interval of Pap smear among the performed, 2 years interval was 36.8%. The medical facility usually being performed Pap smear was obstetrics and gynecological clinic as of 49.7%. At the opinion of int,erval which they think they should take Pap smear, the women answered that 1 year interval was appropriate were 50.7% and the opinions of the experienced group were statistically different from those of the non-experienced group(P<0.01). Gynecological findings showed normal in 74.8%, the cytological results of Pap smear showed class I as of 88.9%, class II 6.3%, class III 4.4% and class IV 0.4%. CONCLUSIONS: Almost of them knew correctly the appropriate interval of Pap smear they should take, but the practice rate was very low. They had good results in the final cytological results of Pap smear, but the cervical cancer is the most common cause of cancer death in Korean women until now. So they should continuously get public informations and be educated to take Pap smear at the appropriate interval for the reduction of death due to cervical cancer.

3.Interleukin-1 beta , -2, -6 Production, Serum Concentration and Hypothalamic-Pituitary-Adrenal Axis in Patients with Major Depression.

The present study was carried out in order to investigate the relationship between immune function and the activity of hypothalamic-pituitary-adrenal(HPA) axis in patients with major depression. The subjects were 16 female major depressives and 16 female healthy controls. We measured mitogen-induced production of IL-1 beta, IL-2, IL-6 and serum level of IL-1 beta, IL-2, IL-6 and basal plasma cortisol levels at 8 00 a.m. We measured post-DST(dexamethasone suppression test) cortisol levels in 16 major depressives. The result were as follows : 1) Basal cortisol level was significantly higher in the patients with major depression than in the healthy controls(14.4+/-4.6 microgram/dl, 10.1+/-5.2microgram /dl, respectively, p<0.05). 2) IL-2 production was significantly lower in the patients with major depression than in the healthy controls(1747.3+/-387.9 pg/ml, 2520.2+/-884.1 pg/ml, respectively, p<0.05). There were no significant differences in IL-1 beta and IL-6 production between the patients with major depression and the healthy controls. 3) Serum level of IL-2 was detectable in 12 of 16 patients with major depression and in 10 of 16 healthy controls. There was no significant difference in serum level of IL-2 between two groups. Serum level of IL-1 beta was detectable in 3 of 16 patients with major depression and of 16 healthy controls. We could not detect serum level of IL-6 in both groups. 4) There was significant negative correlation between IL-2 production and post-DST cortisol level(r= -0.89) in the 16 patients with major depression. There was significant negative correlation between serum level of IL-2 and post-DST cortisol level(r= -0.97) in the 12 patients with major depression. There was significant negative correlation between serum level of IL-2 and basal cortisol level(r= -0.65) in the 12 patients with major depression. But there was no significant correlation between IL-2 production and basal cortisol level in the 16 patients with major depression. These findings suggest that immune function is decreased in major depression and the decreased immune function is highly related to the hyperactivity of the HPA axis.

6.A case of palatally impacted incisor treated by onlay technique with surgical exposure.

The patient, a girl of 19 years in good health, had a class I malocclusion. The maxillary left centra1 incisors and both lateral incisors had already erupted. But the space for the right central incisor was partially closed by the mesial drifting of the neighboring teeth. The caused a shift in the midline and a cross-bite relation on the incisors. X-ray examination revealed the presence of the right central incisor in the alveolar bone and odontoma just above the crown of the right central incisor. After enough space for the impacted incisor was created in the dental arch with a open-coil spring the rectangular incision was made. Removing the odontoma uncovered the flat surface of the labial aspect of the incisor. During the tooth had erupted of its own accord, any unnecessary force had been imposed on the tooth. When it was decided that the tooth should be brought out by the mechanical device, the gold cast onlay with hook was used and run a light elastic between this hook and the main arch wire. Finally the tooth was brought down to the arch level. The result was excellent. Fortunately the esthetic problem and any detrimental effects on the psychological make-up could be avoided.

8.A Clinical Study of Congenital Dislocation of the Hip

The congenital dislocation of the hip is one of the most common congenital disease of the infants. The diagnosis and treatment of comgemital dislocation of the hip has been developcd since Lorenz published the methods of the closed reduction and immobilization in 1895. By replacing the displaced femoral head in the acetabular socket in early time, the normal development of acetabulum and femoral head were expected. So the early diagnosis and treatment were very important for good results. The authors studied 19 cases of 18 patients of congenital dislocation of the hip at the Department of Orthopedic Surgery of Hanyang University Hospital from June, 1972 to June, 1977. As results of this study, the following concolusion were result reached: 1. The proponderance of girl to boy was 13: 5. 2. The ratio of left side to right was 12:7. 3. The chief complaints were shortening of lower extremity, limitation of hip motion(esp. abduction) below 1 year old age and limping in over 1 year old age. 4. The associated congenital anomaly was abserved in a case of internal tibial torsion and metatarsus varus deformity. 5. In birth history, there was 1 case of breech presentation. 6. The acetabular index was much decreased in involved hip than the sound hip in treatment. 7. The arthrogram was valuable to detect any obstacles of closed reduction. 8. In 16 cases of 15 patients, 13 cases were treated by closed reduction, 1 case was open reduction, 1 case was derotation osteotomy, 1 case was varus and derotation osteotomy. 9. The results of treatment was Excellent:8 Good:8.

10.Arch-First Technique in Aortic Arch Aneurysm.

To minimize the period of brain ischemia and the potential for neurologic damage during aortic arch replacement, we used the arch-first technique. First case was a 28-year-old female with extensive aneurysm involving ascending, arch and descending thoracic aorta. Exposure was obtained via a bilateral via a bilateral thoracotomy (clamshell incision) in the anterior 4th right and 3rd left intercostal space with oblique sternotomy. To prepare for arch perfusion, the side-arm graft(10mm) was anastomosed to the aortic graft, opposite the site of the planned anastomosis to the arch vessels. After completing the arch anastomosis under total circulatory arrest(37min) and retrograde cerebral perfusion(12min), aortic graft was clamped on either side and the arch was perfused via side-arm graft for 36min. When distal aortic anastomosis was finished, distal clamp of aortic graft was released and arch vessels were perfused via common femoral artery, and the proximal aortic anastomosis was accomplished. The patient was discharged with no event. Second case was a 48-year-old male with extensive aneurysm involving ascending, arch, and aortic regurgitaiton(grade III/IV). This case was also done using the clamshell incision. Aortic valve replacement was done by valved-conduit(Vascutek 30mm), both coronary artery anastomosis using Cabrol's procedure. Last operation procedure was the same as the 1st case.
