Fracture of the coracoid process are rare and those associating with a complete acromioclavlcular separation is even more rare. The mechanism of injury may be attributed to either direct or indirect trauma. Most coracoid fractures are minimally displaced because It is by maintained by the coracoclavicular ligament and coracoacromial ligaments. Usually good result is obtained In the coracoid fracture by non-operative treatment. On the other hand open reduction Is rarely Indicated. The case we are reporting was treated by open reduction and Internal fixation of the acromloclavicular joint using two Kirschner wires, and screw fixation was done for the fracture of the base of the coracoid process. Review of the literature ls also done.

The present study is an observation of 678 cases of children's fractures; which was treated in the Dept. of Orthopedic Surgery, inchon Christian Hospital, for the past ten years, from 1968 to 1978. The peak age was in from 5 years to 6 years. The sex ratio was 2.9 male to 1 female. The ratio between the right and left extremity was nearly equal. The frequency of fracture of the upper extermity was silghtly dominant then lower extremity. The cause of fracture was fall down Injury in nealy 70% of all cases. The epiphyseal Injury was noted as 16.8% of total orthapedic cases. The most common type of Salter & Harris Classiflcation was type II or 56.1%. Mostly all of children's fractures were treated by closed method, and others were treated by open reduction. Complication Included 29 cases of changed carring angle 18 cases of traumatic arthritis and 8 cases of malunlted supracondylar fracture and 3 cases of postoperative Infections and 2 cases of radial nerve palsy and 3 cases of delayed ulnar nerve palsy of the elbow. In fractures of the femur, 3 cases of the peroneal nerve palsy and 2 cases of leg longth discrepancy was noted. Brief discussion and review of Iiterature is presented.

Eosinophilic granuloma of bone well-documented in the literature. This lytic bone disease most commonly occurs in children, but a fraction of the cases does occurs in adult. When the disease occurs in adults, the bone most often affected include the skull, ribs, mandible, clavicle, pelvis, and long bone of the extremities. We report the case of a 31-year old woman with isolated eosinophilic granuloma of the thoracic spine. She had a 1-month history of progressive back pain that radiated to the right anterior chest. Authors performed resection of vertebral body lesion and soft tissue lesion and anterior fusion from T7 to T9. Immunochemical staining showed a positive S100 protein stain. EM demonstrated characteristic Birbeck granule. Postoperatively, the patient underwent a 5-day course of RT to the T6-T10 spine with 200cGY fractions (total dose 1000cGY). Postoperatively, the patient's back and chest pain had disappeared. There is no evidence of local recurrence or distant metastasis at 18 months follow-up. Though rare, eosinophilic granuloma of the adult spine could be considered in the differential diagnosis of osteolytic lesions.

Glomus tumor is a kind of vascular tumor that arises from the glomus body, which regulates skin temperature and is placed in the skin and the subcutaneous area. It is a benign tumor that usually presents in the subungal area. It is relatively common in areas other than the fingers, but its occurrence in peripheral nerves is known to be comparatively rare. We report our experience with a case of glomus tumor arising from the brachial plexus, a rare site of occurrence for glomus tumors.

There are two contradictive opinions of the therapeutic administration of Ginseng in the conditions of cerebrovascular accident and/or hypertensive heart diseases. This experimental study was performed to observe the effects of the Ginseng extract on the blood pressure(BP) and intracranial pressure(ICP) at the same time. Seven mongrel cats were used. ICP was measured with epidural ballon and pressure transducer, and BP with intraarterial catheter via female routs. Both were continuously recorded by polygrapher. 1. In a normal condition, a single intravenous injection of Ginseng 100mg/kg induced immediate reduction of BP for 30~45 second. Intracranial pressure, however, showed delayed elevation for 50~60 second. 2. Even in a condition of increased ICP or induced cardiac hypertension, the results were the same as in above-described normal condition.

Recurrent episode of meningitis in infants and children frequently constitute a frustrating and distressing, both in determination of course and treatment. The authors are reporting a infected case of congenital dermal sinus with dermoid cyst in the subarachnoid space of cauda equina and conus medullaris which was cured after complete removal of cyst and sinus tract.

We recently noticed four cases of Vibrio(V.) vulnificus infection from July, 1985 to September, 1985. The V. vulnificus was isolated by culture from the necrotizirig skin bullae in three cases, blood culture in two cases, and cerebrospinal fluid(CSF) in one case. The clinical characteristics of V. vulnificus infection in our cases was as follows: 1)All patients were men with their age over forties and the outbreak of the disease was during the summer season. 2) It was suspected that all patients had the previous hepatic problems. 3) The skin lesions showing bullae in three patients and subcutaneous nodules in one patient were noticed. 4) Two patients were showed positive in blood culture and one of thern also showed positive in lesional skin, urine and CSF. Lesional skin culture showed positive in three patients. 5) The isolated v. ulnificus was sensitive to chloramphenicol, erythromycin, gentamycin, kanamycin and cefobid. 6) Two patients died due to sepsis within 48 hours after liospitalization and one patient died due to hepatic failure.

PURPOSE: Fragile X syndrome is an X-llinked genetic disorder and is characterized by mental retardation, learning disability, behavior disorder, and autism with typical elongated face, large ears, and macro-orchidism. Recent reports have focused attention on the EEG finding of this syndrome, which is a particular paroxysmal pattern during sleep (mono or diphasic centrotemporal spikes) and awake state (background slowing). In this study, we analyzed the clinical characteristics of fragile X syndrome patients and observed whether a particular EEG pattern is associated with this syndrome or not. METHODS: 7 cases of fragile X syndrome, diagnosed at Sowha Children's Hospital and Cha General Hospital from August 1993 to February 1995, were analyzed retrospectively in terms of typical phenotypes and clinical & EEG characteristics. The patients were diagnosed by Southern blotting and polymerase chain reaction (PCR) method. RESULTS: 1) The subjects were all male and the mean age was 5.8 years old (2Y-11Y). 2) Typical phenotype of long elongated face, macro-orchidism, large ears, and large head are noted in 2/3 of the subject. 3) Developmental delay, mental retardation, learning disability, attention deficit, hyperactivity, and autism are noted in 2/3 of the subject. 4) Seizure is noted in one case and EEG was performed in 6 cases, regardless of the presence of seizures. Abnormal findings including centrotemporal sharps and background slowing are noted in one case, each. 5) By molecular diagnostic methods including Southern blotting and PCR, 3 cases of affected male and 4 of normal transmitting male were diagnosed. CONCLUSIONS: 1) The typical phenotype of fragile X syndrome is long elongated face, macro-orchidism, large ears and large head. 2) The non-physical characteristics of fragile X syndrome are developmental delay, mental retardation, learning disability, attention deficit, hyperactivity, and autism. 3) The characteristic EEG findings of fragile X syndrome known by literature are noted in 2 among 6 cases, which means the specificity is high even though the sensitivity is low. This allows us to propose this EEG pattern as an important "marker" in the diagnosis of fragile X syndrome. However, the number of the subject is too small to conclude now. Further accumulation of cases is reguired.
