2.A Case Dermal Melanocyte Hamartoma.

A 26-year-old man presented with a bluish speckled patch on the dorsum of the left hand. It had apperaed at birth as a bluish spot. Since adolescence the lesion extended and at the time of visiting hospital it covered the most part of the dorsum of the left hand and even some palmar aspect. The histopathological and ultrastructural examination revealed numerous dermal melanocytes, mainly in the upper dermis. Clinically and pathologically, the patient was diagnosed as having dermal melanocyte hamartoma, a distinct type of dermal melanocytosis.

3.Systemic Lupus Erythematosus Showing Vesiculobullous Eruptions: Report of Two Cases.

l6-year-old male and 48-year-old female with systemic lupus erythematosus (SLE) and renal disease presented with vesiculobullous eruptions during severe attack of the disease. They showed hypocomplementernia, and high ANA titers. Histopathologic findings revealed subepidermal blister and leukocytoclastic vasculitis, Direct immunofluorescence demonstratedlinear deposits of IgG, IgA and lgM at the dermoepidermal junction. The vesiculobullous eruption of SLE may be an important marker reflecting disease activity and prognosis.

4.A Clinical Study of Doppler Examination for Carotid Artery Occlusive Disease.

To determine the comparative accuracy of two Doppler tests, the findings of 38 patients who had undergone a complete evaluation by supraorbital Doppler test, frontal Doppler test and angiography were studied. When stenosis of 60 percent or more in diameter was considered as a true positive finding, the Doppler tests of 76 internal carotid arteries of the 38 patients showed the following results. Supraorbital Doppler test: eight false positive, three false negative, and 65 correct findings, with an overall accuracy of 85%. The authors, therefore, concluded that the suprorbital Doppler test was more sensitive and accurate than the frontal Doppler test in screening the patients with possible carotid artery occulusive diseases.

6.A Case of Tinea Versicolor of the Scalp and Face in Infancy.

Tinea versicolor is a superficial mycosis caused by Malassezia furfur. Tinea versicolor in infancy is rare and the distribution of the lesions differ from that of adult. The predilection sites for tinea versicolor are the chest and back in adults, whereas the face and scalp are commonly involved in childhood tinea versicolor. We report a case of tinea versicolor in a 5-month-old male infant. He had fine scaly whitish macules on the scalp, face and posterior neck. The lesions were successfully treated with topical ketoconazole for 3 weeks.

7.Histologic and Immunohistochemical Study of Cutaneous Vascular Disorders.

There is a wide array of designation for cutaneous vascular disorders based on clinical characteristics, histology, embryology, cellular biology, and hemodynamics. The cutaneous vascular disorders can be divided into hemangioma and vascular malformation according to the biologic classification based on cell kinetics i.e. endothelial hyperplasia. There are clinical and histologic differences between them. In this study, clinical, histologic and im-munohistochemical evaluations were attempted on 40 cases of cutaneous vascular disorders diagnosed the period between 1985 and 1993. The results are as follows: 1) Twenty-three out of forty cases were immunoreacive for proliferating cell nuclear antigen(PCNA). The lesions composed of capillary-sized blood vessels with endothelial hyperplasia were diffusely reactive, whereas those composed largely of dilated blood vessels with or without focal endothelial hyperplasia were only focally reactive. 2) Each groups of the classic classification contained both reactive and nonreactive cases except nevus flammeus and juvenfle hemangioma. 3) In contrast to the cases nonreactive for PCNA, those reactive for PCNA contained areas of proliferating small vessels, which showed reactivity for PCNA. In conclusion, the cutaneous vascular disorders diagnosed by the classic classification are heterogeneous in the pattern of the endothelial hyperplasia and the PCNA staining. Therefore it should be classified by the clinical and the histologic characteristics.

10.Assessment of Congnitive Disorders in Alcoholics Using the 7 Minute Screening Battery.

OBJECTIVES: Chronic alcohol consumption has been known to result in various neurocognitive deficits. Many neuropsychological studies revealed that the major disturbances occurred in the executive function learning and short-term memory. visuospatial performance function, perceptuo-motor skills and abstraction and problem solving abilities. This study was done to identify which cognitive areas might be mainly affected. METHODS: The cognitive disturbance was evaluated using the Korean Version of the Mini Mental State Examination(MMSEK) and the 7 Minute Screen(7MS) in male inpatients with alcohol dependence(N=31) as well as in age and education level matched healthy male controls(N=30) Four individual tests of the 7MS were consisted of the Benton Temporal Orientation Test the Enhanced Cued Recall, the Clock Drawing and the Category Fluency. RESULTS: 1) The average scores of four individual test of the 7MS for the alcoholics were 2.77+/-4.38 for the Benton Temporal Orientation Test. 13.90+/-2.02 for the Memory Test(the Cued Recall 6.77+/-1.94, the Uncued Recall 7.10+/-2.45), 5.84+/-1.86 for the Clock Drawing and 12.58+/-3.29 for the Category Fluency. Except the Benton Temporal Orientation Test, there were statistically significant differences between test scores of alcoholics and those of controls(p<0.01). 2) The alcoholics who had MMSE-K score<24 were 9.68%. The average(+/-S.D) score of the MMSE-K for the patient group(27.23+/-2.62) was significantly(p<0.001) lower than that of the healthy controls(29.20+/-1.24) There were no statistically significant differences between four individual test scores of the 7MS of alcoholics with the MMSE-K score<24(N=3) and those of alcoholics with the MMSE-K score > or =24(N=28) 3) Four individual test scores of the 7MS seemed to have statistically significant association with such variables as MMSE-K, duration of alcohol drinking blood magnesium concentration liver function and thyroid function. CONCLUSION: Mild deficits of cognitive areas such as orientation, memory, visuospatial abilities and verbal fluency could be found in alcohol dependence.
