1.A Clinical Analysis of Supination-outward-rotation Injuries of the Ankle

78 cases of ankle fracture are experienced and especially 25 cases of supination-outward rotation injuries treated during the period from January 1959 to October 1970 were analyed. 1) 17 cases of male and 8 cases of female, and 11 caces in right side and remaider in left side. 2) According to the Lauge-Hansens classification, 5 cases in type II, and 20 cases in type IV. 3) Conservative treatment was given in 8 cases and open reduction in 17 cases. 4) A comparison has been made between functional results treated by conservative treatment and open reduction; satisfactory results were obtained in 3 cases of type II and I case of type IV by conservative treatment. I case of type II and 14 cases of type IV by open reduction. 5) Time of complete healing by bony union were 7. 8 weeks in conservative treatment and 9. 3 weeks in open reduction. 6) Wound infection, necrosis of wound edge and pseudoarthrosis were noted as post-operative complications.

2.Experiences on Idiopathic Aseptic necrosis of Femoral Head

6 cases of idopathic asceptic necrosis of femoral head were experienced at the Department of Orthopedic Surgery. Age distribution ranged from 19 to 58 years and the ratio of sex was 3:3. Two of them had bilateral involvement of necrosis of femoral head. Two of them were primarily treated by means of arthrotomy, joint debridement and drilling of the necrotic area. Symptoms only temporarily relieved and required further surgical procedure. As a whole, arthrodesis in 2, cup arthroplasty in 1, Austin-Moor hip prosthesis in 2 and total hip prosthesis, McKee-Farrar type, in 1 case were performed. Four of the mobile operation of the hip restored satisfactory range of hip motions despite the types of surgery.

3.A Case of Secondary Glaucoma combined with Mesodermal Dysgenesis of Anterior Segment.

The authors experienced a case of secondary glaucoma combined with mesodermal dysgenesis of anterior segment, which disclosed extensive peripheral iridocorneal adhesion of congenital origin while his fellow eye revealed a sclerocornea. This case is extremely rare and entirely different from the previously reported Axenfeld's and Rieger's anomalies. We report this interesting case with brief review of the literatures.

4.Human Vitreous Proteomes in Idiopathic Macular Hole.

PURPOSE: To investigate the vitreous proteomic profiles of macular hole (MH) patients compared to donor eyes and epiretinal membrane (ERM) patients. METHODS: Ten donor eyes, 25 MH patients (15 MH without posterior vitreous detachment (PVD) and 10 MH with PVD), 10 ERM patients and 4 patients with vitreomacular traction syndrome (VMTS) were enrolled in the present study. The undiluted vitreous samples were obtained from all eyes. Proteins were identified using the micro-RPLC/MS/MS technique and using mass spectrometric data analysis from the IPI database. Proteins were searched for the function, subcellular location and tissue specificity using the Uniprot database. RESULTS: The mean vitreous protein concentrations were the highest (p < 0.01) in MH patients without PVD. There were no qualitative changes according to the age of donor eyes. However, the authors newly-identified 12 proteins (i.e. vasorin) in donor eyes. Ten out of 136 proteins (7.3%) (i.e. retinoschisin) were exclusively detected in the MH samples. S100A8 was exclusively detected in eyes without PVD, whereas CD59 was exclusively detected in eyes with PVD. CONCLUSIONS: The results suggest there is a small difference in the vitreous proteomic profiles in MH patients compared to donor eyes or ERM patients, and that the expression of vitreous proteins is closely associated with the presence of PVD.

5.A Case of Fungal Granuloma Caused by Trichophyton violaceum.

We report a patient with an unusual manifestation of Trichophyton violaceum infection, who lives in Seoul. An 8-year-old male presented an erythematous nodule on the lower eyelid of the left eye which had persisted for 10 weeks. On physical examination, there was no abnormality except a skin lesion on the eyelid. The skin biopsy of the nodule showed epidermal hyperplasia and granulomatous change in the upper dermis. The biopsy specimen yielded Trichophyton uiolaceum on culture. The skin lesion disappeared 2 months after administration of ketoconazole.

6.Eosinophilic Granuloma of the Adult Spine: Report of a Case.

Eosinophilic granuloma of bone well-documented in the literature. This lytic bone disease most commonly occurs in children, but a fraction of the cases does occurs in adult. When the disease occurs in adults, the bone most often affected include the skull, ribs, mandible, clavicle, pelvis, and long bone of the extremities. We report the case of a 31-year old woman with isolated eosinophilic granuloma of the thoracic spine. She had a 1-month history of progressive back pain that radiated to the right anterior chest. Authors performed resection of vertebral body lesion and soft tissue lesion and anterior fusion from T7 to T9. Immunochemical staining showed a positive S100 protein stain. EM demonstrated characteristic Birbeck granule. Postoperatively, the patient underwent a 5-day course of RT to the T6-T10 spine with 200cGY fractions (total dose 1000cGY). Postoperatively, the patient's back and chest pain had disappeared. There is no evidence of local recurrence or distant metastasis at 18 months follow-up. Though rare, eosinophilic granuloma of the adult spine could be considered in the differential diagnosis of osteolytic lesions.

7.A Case of Relapsing Polychondritis.

Relapsing polychondritis is a rare inflammatory disorder affecting the cartilaginous structures throughout the body. Although there is no pathognomonic laboratory tests, clinical features, when coupled with histopathologic findings, enable one to make a diagnosis of relapsing polychondritis. A 26-year-old man presented with purplish, swollen, and tender auricles and injected eyes. His symptoms had developed eight months ago and fluctuated irrespective of various medications such as corticosteroids and antibiotics. Histopathologic examination revealed loss of basophilic staining of the cartilage with some dissolution of its structures. Treatment was initiated with dapsone, However, the remission was too immediate to be considered as a effect of dapsone.

8.Artificial intelligence in obstetrics

This study reviews recent advances on the application of artificial intelligence for the early diagnosis of various maternal-fetal conditions such as preterm birth and abnormal fetal growth. It is found in this study that various machine learning methods have been successfully employed for different kinds of data capture with regard to early diagnosis of maternal-fetal conditions. With the more popular use of artificial intelligence, ethical issues should also be considered accordingly.

10.Spontaneous Rupture of Hemorrhage Renal Cyst into the pyelocaliceal System.

We report on a patient with spontaneous rupture of hemorrhagic renal cyst into the pyelocaliceal system. He had no history of trauma, bleeding disorders, urinary tract infection or urinary tract obstruction. Gross hematuria followed by left flank pain occurred 1 day prior to admission. Although radiologic findings were suggestive of rupture of renal cyst into the calyx or calyceal diverticulum. exclusion of malignant disease or renal carbuncle was difficult. He was managed by partial nephrectomy. Microscopic findings revealed ruptured cyst lined with low cuboidal epithelium without evidence of malignancy.
