1.A Case of Darier' s Disease Treated by the Combined Treatment with Oral Etretinate and Topical Tretinoin Cream.

We report a case of Dariers disease which showed a more effective result using a combined treatment with oral etretinate and topical tretinoin cream than treatment with oral etretinate alone in a 17-year old male patient. Oral etretinate 40mg was given daily and topical therapy with tretinoin cream was applied to lesions of neck and face. Two weeks later, lesions of neck and face showed good improvement compared with the not applied lesions. Topical therapy was then extended to other lesions. Six weeks after treatment, there was a marked improvement. Three months after treatment, most of lesions were nearly gon.

2.A Case of Type IIa Hyperlipoproteinemia with Xanthoma Planum , Xanthoma Tendinosum and Xanthoma Tuberosum.

We report a case of a 59 year-old female with hyperlipoproteinernia who had xanthoma tuberosum, xanthoma tendinosurn and xanthoma planum. Yellowish nodules appeared on the right elbow 26 years ago and the lesions gradually increased in size and spread to the elbow, hand, knee, foot and achilles tendon. Except for the skin lesion, she was normal on physical examination. The laboratory findings showed that the serum cholesterol, beta lipoprotein and total lipid were increased. The beta band was rnarkedly elevated on electrophoresis, which suggested type IIa hyperlipoproteinemia. Histopathological findings shc wed many aggregates of foam cells and foreign body giant cells were seen in the dermis.

3.Propranolol Hcl in Patients with Angina Pectoris.

A double-blind trial of the efficacy of propranolol hydrochloride a new beta adrenergic blocking agent, in the prophylactic treatment of angina pectoris was conducted in 9 patients. Through the trial period of 8 weeks to more than 26 weeks, the patients received propranolol and placebo for equal period. Following are the results. 1. The effective dose of propranolol hydrochloride in the prophylactic treatment of angina pectoris was 80 mg to 120 mg per day. 2. The effectiveness of propranolol hydrochloride was manifested by a reduction in frequency of pain to 40.7%, in nitroglycerin comsumption to 38.8% and by an increase in exercise tolerance. 3. Obseved side effects were transient nausea, vomiting and bradycardia, which were improved with reduction in drug dosage. 4. Further clinical and experimental information may be necessary in order to assess the long-term effects of propranolol hydrochloride.

6.Iron Deficiency Anemia and Iron Nutrition in Adolescent Female Athletes.

PURPOSE: Iron deficiency, particularly iron deficiency anemia(IDA) often not only decreases athletic performance but also impairs immune function and leads to other physiologic dysfunctions. This study was designed to assess and compare iron status and the IDA prevalence in female adolescent athletes with those of general adolescent students. METHODS: Physical measurements and hematologic examinations were performed for adolescent female runners(n=32), non-runner athletes(n=78) and apparently healthy students(n=511). Iron deficiency was defined as a serum ferritin concentration <12 ng/mL. The diagnosis of IDA was established when hemoglobin level <12 g/dL was associated with ferritin <12 ng/mL and/or transferrin saturation <16%. RESULTS: The prevalence of ID in the runners' group(40.6%) was significantly higher than that of the non-runner athletes' group(23.1%) and the control group(25.4%). The prevalence of IDA in the runners' group was 31.3%, which was significantly higher than that of the non-runners' group and the control group(7.7% and 16.4%, respectively). CONCLUSION: The prevalence of IDA of Korean adolescent females was relatively high, especially, in female adolescent runners. Screening for anemia and iron status is warranted. It is important to measure for iron nutrition and to enforce effective nutritional education in these groups.

7.Effect of Nicardipine on Left Ventricular Mass in Hypertensive Patients.

BACKGROUND: Systemic hypertension produces varying degree of LVH which is associated with increased cardiovascular morbidiity. Previous studies have documented regression of LVH with various antihypertensives including calcium channel blockers, except diuretics and vasodilators. Recently echocadiographic assessment of the change of left ventricular mass(LVM) after antihyertensive therapy have been reported to offer prognostic cardiovascular information. The aim of this echocardiographic study is determining the influence of nicardipine, a calcium channel blocker, on the change of LVM in patients with essential hypertenison. METHODS: Left ventricular mass(LVM) and left ventricular mass index(LVMI) were measured by M-mode echocardiography in 15 patients with essential hypertension. Nicardipine, a calcium channel blocker, was administered for 6 months and two echocardiographies were done before and after administering, respectively. RESULTS: In the 15 patients treated for 6 months, systolic and diastolic pressure remained very significantly decreased compared with pressure before before therapy(135+/-15mmHg vs 168+/-26mmHg, and 86+/-7mmHg vs 105+/-16mmHg, both p<0.01). Concomitantly both LVM and LVMI decreased significantly(209+/-49g vs 235+/-71g, and 116+/-6g/m2 vs 131+/-38g/m2,both p<0.05). And no change was noted in left ventricular cavity size, demonstration that LVM reduction was due to regression of hypertrophy. CONCLUSION: This study showed that nicardipine produced a significant decrease in blood pressure, LVM, and LVMI over the 6 months period. And large and longterm controlled studies are needed for the clarification of the association between nicardipine and regression of LVH in hypertensive patients.

8.A case of malignant acanthosis nigricans associated with gastric adenocarcinoma.

The new onset of acanthosis nigricans in an adult-especially when the lesions appear on mucosal membranes such as the lips, periocular areas, and anus-may be indicative of an underlying tumor. Adenocarcinoma of the gastrointestinal tract is the most common malignancy-most often cancer of the stomach. A 72 years old female had disseminated, symmetrically distributed, brownish black pigmentation and papillary hypertrophy on the intertriginous and flexural areas for 12 months, and gastric adenocarcinoma was confirmed during the evaluation of internal malignancy. The cutaneous finding of acanthosis nigricans was a significant cutaneous marker of internal malignancy in this case.

9.Cementless Acetabular Revision Performed with Morselized Bone Grafts:A Ten-Year Minimum Follow-Up.

Purpose: We evaluated the results of cementless acetabular revisions performed with morselized bone grafting and screw-fixed hemispherical cups with different surface treatments. Materials and Methods: Forty hips, which had been followed for more than 10 years, were included in this study. Reconstruction was performed with 10 hydroxyapatite (HA)-coated cups and 30 porocoated ones. The mean followup time was 12 years and 1 months (range, 10 years to 15 years). Re-revision or radiographic loosening was considered as an endpoint of follow-up. Results: The average Harris hip score improved from 52 points to 75 points. During the follow-up period, radiographic loosening was observed in 17 hips. The loosened implants were HA-coated cups in 8 hips and porocoated ones in 9 hips. In 14 of these, re-revision of the cups was performed. The re-revision rate was 20% for the porocoated cups and 80% for the HA-coated cups. There were 2 hips with liner wear, which had undergone liner and head changes. Bone grafts were united in all the hips. The average time to union was 5.2 months (range, 2 to 9 months), and the average time to incorporation was 12 months (range, 5 to 18 months). Conclusion: Our results imply that HA-coated cups have a significantly higher failure rate compared with the porocoated ones (P<0.05) after a minimum follow-up of 10 years. Morselized bone grafting with use of a porocoated cup is an effective modality, which can restore the bone loss of the acetabulum in revision total hip arthroplasty.

10.Mercury Concentration in Air and in Urine of Workers in A Fluorescent Lamp Manufacturing Factory.

In order In interpret the relationship among many variables, urinary mercury and at the same time mercury concentration in air were measured for 254 workers of a fluorescent lamp manufacturing factory. And monthly mercury consumption amounts, numbers of monthly producing fluorescent lamps, numbers of inferior lamps and numbers of breakage lamps, numbers of vacuum exhaustion pumps and frequencies of mercury infusion were also investigated from January 1988 to March in 1989. The results were as follows; 1. On the mercury concentration in air by producing line and by working site, all of them were exceed the threshold limit value (0.05mg/m(2)). 2. The highest mercury concentration in air by sampling point was found at the floor of workplace (0.334mg/m(2)) and next were at vacuum exhaustion pimp (0.183mg/m(2)), and breathing zone of workers (0.103mg/m(2)) in order. 3. The highest mean of the mercury concentration in urine by producing line was the automatic exhausting line (80.8microgram/1) and next were high speed exhausting line (72.4microgram/1), and Manual exhausting line (35.8microgram/1) in order. Distribution of workers of the mercury concentration in urine more than 200microgram/1 by working site was the highest (10%) in the vacuum exhaustion pump part and next were sealing part (6.6%), packing part (4.6%), and stem part (4.1%) in order. 4. The correlation coefficient between mercury concentration in urine and in air was the highest (0.302) and next were numbers of breakage lamps (0.223), frequencies of mercury infusion (0.223), numbers of inferior lamps (0.205) in order.
