1.Treatment of endometrial hyperplasia with Medroxyprogesterone acetate(MPA).

OBJECTIVE: To investigate the response of hyperplastic endometrium to Medroxyprogesterone acetate according to the histologic types such as simple typical, complex typical and atypical hyperplasia. METHODS: A total of 79 patients with histologically proved endometrial hyperplasia were enrolled into this prospective study between March 1996 and May 1998. Patients without atypia were placed on a regimen of cyclic therapy with 10mg MPA orally, each day for 14days per month for 3 months. In the cases in which hyperplasia did not regress , MPA was increased to 20mg. Patients with atypical hyperplasia received continuous MPA therapy, 20mg orally each day for 3 month. All patients were followed up for a minimum of 3 months and a maximum of 1 year(mean 7 months). RESULTS: In patients with simple typical hyperplasia, 45 patients(80.4%) had regression, 11 patients(19.6%) had persistence and none had progression. In patients with complex typical hyperplasia, 10 patients(83.3%) had regression, 2 patients(16.7%) had persistence and none had progression. But, in patients with atypical hyperplasia 5 patients(45.4%) had regression, 4 patients(36.4%) had persistence and 2(18.2%) patients had well differentiated endometrial adenocarcinoma. There was no recurrence during the follow up. CONCLUSION: This data suggest that most women with typical hyperplasia respond to progestin therapy, but there is high failure rate of response to progestin therapy and risk of endometrial cancer in patients with atypical hyperplasia. If the young patient desires to preserve her fertility, then progestin therapy may be considered as primary treatment in patients with atypical hyperplasia. But older patients in whom fertility is not an issue, hysterectomy should be selected as treatment of choice for atypical lesion.

2.Diagnostic Significance of Serum Tumor Markers in Paitents with Ovarian Tumors.

OBJECTIVE: This study was designed to evaluate the efficiency of tumor markers level for early diagnosis of ovarian malignancy and for differentiation between benign and malignant ovarian tumors. MATERIALS AND METHODS: We determined preoperative serum tumor markers level in patients who were going to have an operation due to ovarian tumor in OB & GY Dept. of Chonnam University Hospital from April 1993 to September 1999. RESULTS: 1) The average values of serum tumor markers in patients with malingnant ovarian tumors were statistically higher than those of benign ovarian tumors. among malignant ovarian tumors, positive rate of all serum tumor markers was highest in epithelial ovarian carcinoma group. and among ovarian tumor markers, and positive rate of CA 125 was highest in epithelial ovarian carcinoma. 2) Dermoid cyst and endometioma were correlated to CA 19-9, CA 125 levels respectively. for malignant tumors, mucinous ovarian adenocarcinoma and non-mucinous ovarian adenocarcinoma were CA 19-9, CA 125 levels respectively. 3) Among ovarian tumor markers, CA 125 was the most in sensitivity and CA 72-4 was the most in specificity and diagnostic efficiency. 4) For postmenopausal women with ovarian tumors, elevated levels of at least one of the 4 tumor markers were present in the serum in 85.7% of the women who developed cancer, 62.5% of women with borderline, 27.8% of women with benign ovarian tumors. Conclusion; It is suggested that determination of serum tumor markers in patient suspected of ovarian tumor may be helpful to clinician for early diagnosis, differentiation between malignant and benign ovarian tumors.

5.Surgical Management of Cecal Diverticulitis Detected during Appendectomy.

PURPOSE: Acute diverticulitis of the right colon is not rare in Korea and the clinical presentation is indistin guishable from acute appendicitis. Cecal diverticulitis has led to a controversy in the management of disease. METHODS: Thirty-one cases of acute cecal diverticulitis who underwent operation for suspected acute appendicitis were reviewed retrospectively from January 1995 to December 1998. RESULTS: There were 17 men & 14 women. Ages ranged from 9 to 69 (mean: 37.5) years. All patients presented with signs and symptoms as acute appendicitis. All patients were explored through a transverse incision in the right lower quadrant under the impression of acute appendicitis. An appendectomy and drainage was performed in 13 patients, and resection of the lesion was performed in 18 patients (12 ileocecal resection, one partial cecectomy including appendix, one partial cecectomy and an appendectomy, 4 diverticulectomy and appendectomy), depending on the location of diverticulitis, severity of inflammation, and surgeon. Staples (TA(R), GIA(R)) were used in all cecal resection cases except for diverticulectomy. Five complications were observed, 3 in cecal resection cases (one wound seroma, one wound infection and one bleeding), and 2 in appendectomy and drainage cases (two wound infections). There was no postoperative mortality. The average length of the postoperative stay was 10.2 days in the drainage group and 8.8 days in the cecal resection group. Two recurrences were observed. One was the patient who had diverticulectomy performed. The other was a patient who had had appendectomy and drainage. CONCLUSION: We concluded that the preferred surgical management of an acute cecal diverticulitis operated for a presumed acute appendicitis is cecectomy using staples depending on its location and severity of inflammation. It was safe, relatively easy to do through the same incision, and could be a definitive treatment.

6.Radical abdominal trachelectomy with pelvic lymphadenectomy: Report of 2 cases.

Traditionally, radical hysterectomy is the main surgical method for the treatment of early cervical carcinoma and always results in the loss of fertility. But, large numbers of young women are recently being diagnosed with cervical carcinoma and fertility preservation has become a concern. So, there has been a move towards more conservative approaches for the treatment of cervical carcinoma in recent years. Radical trachelectomy, which allows preservation of uterus but removes the cervix, parametrium and upper one third of the vagina, is a conservative but locally radical procedure. We performed radical abdominal trachelectomy with pelvic lymphadenectomy for two cases of invasive cervical carcinomas, which may be the first report in korea. One patient was 37 years old single women who had stage Ib cervical cancer and was disease free for 17 months after treatment. The other patient was 19 years old student who was also single and had stage IIa cervical cancer and was disease free for 14 months after treatment. We report the first two cases and review the literature on radical trachelectomy.

7.A Case of Unilateral Submandibular Glandular Aplasia.

The congenital absence of the major salivary glands is an uncommon disorder, although it is not always symptomatic. Their etiopathogenesis is poorly understood. Aplasia of the salivary glands may occur either in isolation or in association with other developmental abnormalities. Some authors have reported familial salivary gland aplasia. Aplasia may be partial or total. Severely affected patients suffer from dry mouth and an increased rate of dental decay. Following clinical exclusion of the cause of these symptoms, the diagnosis of submandibular gland aplasia can be confirmed by computed tomography and a Tc-99m pertechnate scintiscan. The authors experienced a case of incidentally detected, unilateral submandibular glandular aplasia in a 38-year old female. So we present this case with review of literatures.

8.Irradiation Alone in Stage IB, IIA, and IIB Cervix.

PURPOSE: We analyzed the survival and failure patterns of cervix cancer patients treated with irradiation alone to evaluate our treatment method and to compare with the others. METHODS AND MATERIALS: Two hundred and twenty cervical cancer patients, Stage IB, IIA, and IIB who completed the planned treatment between May 1987 and December 1991 were analyzed retrospectively. The Stage IB patients were restaged to the Stage IB1 and IB2 by the recently revised FIGO classification. Patients were treated with a combination of external irradiation and the intracavitary brachytherapy. Determination of the tumor control was done at the time of 6 months postirradiation. The follow-up time was ranged from 3 to 115 months and the mean was 62 months and the follow-up rate was 93.6%(206/220). RESULTS: The overall 5-year survival rate of Stage IB1(N=50), IB2(N=15), IIA(N=58), and IIB(N=97) was 94%, 87%, 69%, and 56%, respectively. In the univariate analysis of prognostic factors,stage(0.00), initial Hg level (p=0.00), initial TA-4(tumor-associated) antigen level(p=0.02), initial CEA level(p=0.02), barrel-shaped tumor(p=0.02), whole cervical involvement (0.00), pelvic lymphadenopathy(LAP) in CT(p=0.04), and post-irradiation adjuvant chemotherapy(p=0.00) were statistically significant in survival analysis. In a while, multivariate analysis showed that the stage was the most powerful prognostic indicator and the post-irradiation chemotherapy factor also showed the statistical significance. The overall local control rate was 81% and by the stage, 100% in Stage IB1, 86.7% in Stage IB2, 84.5% in Stage IIA, and 68.1% in Stage IIB, respectively. The overall tumor recurrence rate was 15.5%(27/174) and by the stage, 8%(4/50) in Stage IB1, 0%(0/13) in Stage IB2, 22.4%(11/49) in Stage IIA, and 19.4%(12/62) in Stage IIB, respectively. CONCLUSIONS: We obtained the similar treatment results to the other's ones in early stage cervical cancer patients. But in Stage IIB, the local control rate was lower than that of the other institutes and also the survival was poorer. So it seems to be necessary to reevaluate the treatment method in advanced cervical cancer patients.

9.A Case of Squamous Cell Carcinoma Originating from Oral Lichen Planus.

Oral lichen planus (OLP) is a relatively common chronic inflammatory disease of uncertain etiology. OLP lesions rarely undergo spontaneous remission and have the potential to undergo malignant transformation. The risk of malignant transformation in OLP is estimated to be between 0.4% and 5.3%; two clinical studies in Korea reported the frequency of malignant change as 2.9% and 2.85%. However, to the best of our knowledge, there has been no individual case report in the Korean dermatological literature to date regarding the malignant transformation of OLP. Herein, we report the case of a 77-year-old Korean woman with long-standing OLP that underwent transformation to oral squamous cell carcinoma. This is the first well-documented case report of squamous cell carcinoma that developed from OLP.

10.Parkinsonism Associated with Frontal Lobe Meningioma: A Report of Two Cases.

Parkinsonism is not commonly associated with intracranial tumors. The most common brain tumor causing parkinsonism is meningiomas. We are presenting two cases of meningiomas whose major manifestations included parkinsonism. In one, parkinsonian symptoms initially partially responded to L-dopa and bromocriptine. A CT scan taken for a stroke-like episode revealed a meningioma of left frontal convexity. After neurological sequelae of CO poisoning, the patient poorly responded to antiparkinsonian drugs. Surgical treatment of meningioma did not result in satisfactory control of extrapyramidal symptoms. In the other with a parasagittal meningioma, parkinsonian symptoms were postoperatiYely improved. Because no curative treatment is available for the majority of parkinsonian patients, early detection of an underlying neoplasm may result in a most rewarding outcome. It would be desirable to perform CT or MRI in cases of parkinsonism with other associated neurological manifestation. Unilaterality of parkinsonian symptoms may also be indication.
