3.A Case of Yellow Nail Syndrome.

We report a 58 year old man who complained that his nails didn't grow for a year and developed yellowish discoloration. On physical examination his nails showed yellow greenish discoloration, increased corvexity, loss of lunulae and cuticles. C]iest X ray revealed atelectasis on right lower lung field and bronchiectasis on left lower lung field. Mild restrictive pattern was observed in pulmoniry function test. We gave him 800IU of Vitamin E claily for 3 months, but rio remarkable changes have been observed as yet..

4.A Case of Corrosive Gastritis Caused by Hydrochloric Acid.

Recently the use of hydrochloric acid as a toilet disinfectant has become more common, and the risk of its ingestion is though to be increasing. We experienced a case of 67 year-old man who accidentally ingested hydrochloric acid and as a result developed a postprandial epigastric fullness end pain 3 weeks thereafter, The patient underwent an UGI series, fiberoptic gastroscope and abdominal ultrasonography and was found to have a gastric midbody stricture. A total gastreetomy and Roux-en- Y esophagojejunostomy was performed. We report this case with brief review of the literature.

6.11 Cases of Pseudomyxoma Peritonei.

Pseudomyxoma peritonei is a rare disorder chracterized by abdominal distension resulting from the accumulation of a mucinous, gelatinous, translucent material which arises from rupture of pre-existing mucinous cystadenomas and cystadenocarcinomas of the ovary and mucocele of the appendix. We report 11 cases of pseudomyxoma peritonei which were diagnosed by surgical and pathological findings. The results were as follows. 1) The mean age of 11 cases was 51 years and 5 cases were male and 6 cases were female and male to female ratio was 1 to 1.2. 2) Clinical manifestations in 11 cases were abdominal distension in 5 cases (45.5%), palpable mass in 4 cases(36.6%), abdominal pain in 4 cases(36.6%), nausea in 3 cases(27.3%), weight loss in 3 cases(27.3%), diarrhea in 2 cases(18.2%), constipation in 1 case(9.0%), anal bleeding in 1 case(9.0%) and defecation difficulty in 1 case(9.0%). The duration of symptoms was 2 days to 1 year. 3) The primary sites of pseudomyxoma peritonei were ovarian cancer in 3 cases(27.2%), appendiceal cancer in 2 cases(18.2%), gastric cancer in 2 cases(18.2%), rectal cancer in 1 case(9.1%), cecal cancer in 1 case(9.1%) and unknown origin in 2 cases(18.2%). 4) Following surgical interventions were performed; right hemicolectomy in 2 cases, palliative gastrojejunostomy in 2 cases, oophorectomy in 1 case, omentectomy in 1 case and right hemicolectomy with gastrojejunostomy in 1 case. 5) The adjunctive anti-cancer chemotherapy was done in 4 cases(36.4%) by combination of actinomycin D, adriamycin, CCNU, cisplatin, cytoxan and fluorouracil, And chemotherapy only was done in 2 cases and hyperthermia was done in 1 case.

7.A Blue Nevus Associated with a Pilar Sheath Acanthoma on the Neck.

Blue nevus is a neoplasm composed of pigmented dendritic dermal melanocytic cells in the reticular dermis. The lesion clinically presents as a well dermacated cerulean-blue or bluish black colored papule or plaque that usually measure less than 1 cm in diameter. Pilar sheath acanthoma is a rare, benign follicular hamartoma commonly affecting middle-aged and elderly individuals. It is characterized by a small, solitary, skin colored papule situated on the head and neck, particularly around the upper lip. We report a rare case of a blue nevus associated with a pilar sheath acanthoma in a 26 year-old man presented with a bluish papule on the neck.

8.A Clinicohistopathological Study on the Lesion Resembling Seborrheic Keratoses of the Face.

BACKGROUND: The most of typical seborrheic keratoses (SKs) can be clinically differentiated with ease from other dermatoses. However, its differential diagnosis can sometimes be difficult and recently reported cases clinically suggest that SKs with the skin biopsy result being some other diseases can bring on the need for further evaluations. OBJECTIVE: The aim of this study is to examine the diagnostic yield in submitting clinically diagnosed SKs which showed single discoid lesions of the face for routine microscopic examinations. METHODS: We conducted a retrospective review of cases for histological examinations with a clinical diagnosis of SKs or with a differential diagnosis that includes SKs which shows single discoid lesion of the face. A total of 94 pathology reports containing a diagnosis of SKs in the clinical information field were identified. The accuracy of the clinical diagnosis and clinic histopathological correlations were studied. RESULTS: In this study, 45.7% of patients was clinically assumed to have SKs but turned out to be some other diseases according to the histopathology test results - a total of 19 cases (44.2%) with some considered to be precancerous (actinic keratosis 25.6%, benign lichenoid keratosis 16.3% or cancerous (basal cell carcinoma 9.3%, squamous cell carcinoma 4.7%). CONCLUSION: Lesions suggested to be SKs need more examinations and lesions of atypical clinical appearances which cannot completely exclude the possibility of precancerous or malignant lesions are strongly recommended for skin biopsies.

10.Healing of the Bone around Hydroxyapatite-Coated Implants without Primary Bone Contact.

Implant stability is the key to long-term successful outcome for osseointegrated implants. To evaluate the initial healing response of bone around HA-coated implants without primary bone contact. 21 HA-coated thread type implants(STERI-OSS?) were placed in the femurs of 5 mongrel dogs, about 1-year old. Implants, 8 mm in length and 3.8mm(experimental 1group), 5.0mm(experimental 2group) and 6.0mm(control group) in diameter, were inserted after 3 holes of 6.0mm in diameter and 10mm in depth were prepared in the surgical sites each dog. Implants were supported by only non-resorbable membrane(Teflon(R)), in order to prevent the ingrowth of upper soft tissue into the gap between bone and implant, and to maintain each implant to be positioned in the center of the drilled hole. 9 implants with different diameters were inserted in 3 dogs for histologic observation, and 12 implants were inserted in 2 dogs for mobility test and removal torque test. Fluorescent dyes were injected for the observation of new bone formation in order of Terramycin(R), Arizarin Red(R), and Calcein(R) at an interval of 2 weeks. 3 dogs were sacrificed for histologic observation at 4, 8, and 12-week after placement. Light microscopy and confocal laser scanning microscopy were used to qualitatively characterize the bone around HA-coated implant. 2 dogs were sacrificed for mobility test(Periotest(R), Simens AG, Bensheim, Germany) and removal torque test (Autograph(R) AGS-1000D series, Japan) at 8 and 12-week after placement The results were as follows: 1. Histologic observation showed that osseointegration occurred to both control and experimental groups as time lapse, but delayed bone healing was revealed in 3.8mm group (experimental 1group), compared to contrtol group and 5.0mm group (experimental 2group). 2. The mobility test showed that the experimental groups had no distinguishable movement during experimental periods of 8 and 12-week, and there was no difference in mobility depending on the gap between bone and implant, and time lapse. 3. The removal torque forces were increased depended on the gaps decreasing between bone and implant, and time lapse. The results suggest that HA-coated implant without primary bone contact, based on guided bone regeneration could obtain its stability in all experimental groups as time lapse, but bone healing was delayed in experimental group of 3.8mm. And the results suggested that studies on correlationship between mobility test and removal torque test for implant stability would be necessary.
