2.Cytogenetic Analysis in 543 cases of amniocentesis.

OBJECTIVE: To analyze 543 cases of amniocentesis with indications, cytogenetic results. METHOD: This study includes 543 cases cytogenetic study results which amniocentesis to detect fetal chromosomal abnormality in the Cytogenetic Laboratory at Eun Hospital in Kwang-Ju from August 1996 to December 2000, as gestational ages, indications of amniocentesis, maternal age distributions, chromosome aberrations with cytogenetic results. The cytogenetic results of chromosome aberration was identified by parents inheritance and de novo karyotypes as parents periperal blood cytogenetic study. RESULTS: Amniocentesis performed mostly from 15 weeks to 20 weeks of gestaional ages. Requested indication of amniocentesis presents abnormal maternal serum screening (37%), infertility (23%) and maternal old age (> or =35) (17%). Chromosome aberration according to amniocentesis indication was suspected fetal anomaly by ultrasonogram (8.6%), previous family history (5.7%). Chormosome aberration following maternal ages, more 40 years old women were found highly 7.9%. The frequency of chromosome aberration was 5.5% but de novo chromosome aberration was 2.2%. Numerical aberration was overall new karyotypes (1.3%). Structural aberration was inheritance karyotypes (3.3%) and de novo karyotypes (0.9%). CONCLUSION: Amniocentesis is a effective diagnostic tools in fetal chromosome aberration. Indication of fetal chromosomal anormality by ultrasonography and advanced maternal ages is an important diagnostic method with chromosome aberrations. Prenatal fetal chromosome aberrations included inheritance and de novo karyotypes. Especially, identification of de novo chromosome aberrations may predict fetal anomaly and counsel the fetus for pregnant parents.

Osteoma cutis is a rare disorder characterized by compact bone formation in the dermis and subcutaneous tissue. It is classified in primary and secondary forms according to the presence or absence of previous cutaneous lesions. Multiple miliary osteomas of the face is a form of primary miliary osteoma cutis that generally occurs in middle-aged and older women. We report a typical case of multiple miliary osteomas of the face in a 63-year-old woman.

4.Inhibitory Effect of Continentalic Acid from Aralia continentalis on Streptococcus mutans Biofilm.

In our present study, we investigated the effects of continentalic acid on Streptococcus mutans (S. mutans) biofilm. Methanol extract of Aralia continentalis (A. continentalis) was suspended in water and sequentially partitioned with CHCl3, ethyl acetate (EtOAc), and n-butanol (n-BuOH). The CHCl3 fraction showed the highest activity and an antibacterial compound against S. mutans was isolated from this preparation through various chromatography methods by bioassay guided fractionation. MS, 1H-NMR and 13C-NMR analysis showed that the active principle was continentalic acid which was confirmed to show significant inhibitory effects against S. mutans biofilm. These results may provide some scientific rationale for the traditional use these extracts for the treatment of dental diseases.

6.Effects of Several Natural Medicines on Alkaline Phosphatase Synthesis in MC3T3-E1 Cells.

Several growth factors and polypeptides are not commonly yet used for regenerators of bone tissue or alveolar bone because of the insufficiency of studies on their side effects, genetic engineering for mass production and stability for clinical application. Recently, many natural medicines, which have advantage of less side effects and possibility of long-term use, have been studied for their capacity and effects of anti-bacterial, antiinflammatory and regenerative potential of periodontal tissues. Olibanum, Myrrha, Phlomis Radix, and Cimicifugae Rhizoma have been traditionally used as a drug for treatment of bone disease in oriental medicine. The objective of this study was to examine the ability of alkaline phosphatase(ALP) synthesis of rat calvarial osteoblast(MC3T3-E1) when several natural medicines were supplemented. MC3T3-E1 cells were cultured with alpha-MEM(negative control), dexamethasone(positive control), and each natural medicines for 3 and 5 days. And then ALP synthesis was measured by spectrophotometer for enzyme activity and by naphthol AS-BI staining for morphometry. All of the natural medicines induced higher activity of ALP synthesis than the negative controls. Especially Olibanum induced the higher activity than the positive controls(p<0.05). In the aspects of culturing time, except Cimicifugae Rhizoma, the natural medicines induced higher activity of ALP synthesis at 5 days than at 3 days(p<0.05). In morphometry, all of the natural medicines showed statistical significance compared to the negative control(p<0.05). Myrrha and Phlomis Radix showed larger positively stained area at 5 days than at 3 days, whereas the others did not showed the difference between at 5 and at 3 days(p<0.05). These results indicate that several natural medicines have an inducing ability of ALP synthesis in MC3T3-E1 cells.

7.A Case of Congenital Eruptive Vellus Hair Cyst.

An eruptive vellus hair cyst is an uncommon, benign, epithelial tumor with hair differentiation. Clinically, it presents as asymptomatic, flesh-colored to yellowish, follicular papules, 1 to 4 mm in diameter, most commonly on the chest. It is usually seen in children and young adults but may be congenital. Histologically, it shows a middermal cyst lined by squamous epithelium containing keratinous material and vellus hairs. It has a tendency to clear spontaneously but may be persistent. We report a case of congenital eruptive vellus hair cyst in an 11-year-old girl who presented multiple pinhead-sized brown to black papules on her anterior chest since birth.

8.A Case of Anticonvulsant Hypersensitivity Syndrome thought to be Caused by Lamotrigine.

Anticonvulsant hypersensitivity syndrome (AHS) is a rare and potentially life-threatening drug reaction, which has been associated with aromatic anticonvulsants such as phenytoin, carbamazepine, and phenobarbital. It is characterized by the triad of fever, rash and internal organ involvement, which mostly includes hepatitis. Histopathological findings usually show characteristic erythema multiforme. Lamotrigine is a new antiepileptic drug, chemically distinct from other anticonvulsant medication, however, AHS has recently been documented in patients treated with lamotrigine. We report a case of AHS in a 29-year-old man, thought to have been caused by the use of lamotrigine.

Gingival fibroblasts are embedded in an extracellular matrix. The matrixs have influence on the development, polarity, and behavior of nearby cells. The major component of periodontal extracellular matrix is a glycosaminoglycan. The glycosaminoglycan are large carbohydrates that are composed of repeating disaccharide units and exist in three main form: dermatan sulfate, chondrotitin sulfate, heparan sulfate. The purpose of present study is to examine the biologic effects of glycosaminoglycan on human gingival fibroblast. Human gingival fibroblasts were supplemented with each glycosaminoglycan, and cellular attachment and proliferation was determined by MTT assay. Dermatan sulfate and chondroitin sulfate did not stimulate the attachment and proliferation of human gingival fibroblasts, but heparan sulfate increased the proliferation and attachment in a time- and dose- dependent manner. These results indicated that heparan sulfate seems to have a high potential for gingival regeneration and root surface attachment.

10.A Case of Zosteriform Cutaneous Metastasis from Transitional Cell Carcinoma of the Urinary Bladder.

The usual sites of the bladder cancer metastases are lymph nodes, liver, lungs and bone. The incidence of metastases to the skin from bladder cancer has been reported to be between 0.2 and 2%. Cutaneous metastases from transitional cell carcinoma (TCC) of the bladder occur infrequently. Metastatic skin cancers vary in type, in rare instances, zosteriform metastases may occur. A 77-year-old man with transitional cell carcinoma of the bladder presented with multiple grouped erythematous papuloplaques and nodules which had developed on the right abdomen, flank and back in zosteriform distribution. Histopathologic examination revealed dermal pleomorphic cellular infiltration and tumor cells in lymphatic vessels. It represents a case of zosteriform cutaneous metastasis from transitional cell carcinoma of the urinary bladder.
