2.Soft Tissue Change After Single Jaw(mandible) Surgery in Skeletal Class III Malocclusion.

The purpose of this study was to evaluate the amount and interrelationship of the soft and hard tissue change after mandibular setback surgery in skeletal Class III malocclusion. The sample consisted of 25 adult patient (12 male and 13 female) who had severe anteropostrior skeletal discepancy. These patient had received presurgical orthodontic treatment and surgical treatment which is bilateral sagittal split ramus osteotomy. The presurgical and postsurgical lateral cephalograms were evaluated. The computerized statistical analysis was carried out with SPSS/PC program. The result were as follows: 1. After mandibular bilateral sagittal split ramus osteotomy, lower facial soft tissue horizontal posterior changes were high significance value. but vertical soft tissue changes were low significance value. 2. After mandibular bilateral sagittal split ramus osteotomy, relative upper lip protrusion increased(p<0.01) and relative lower lip protrusion decreased(p<0.01) and lower facial soft tissue thickness increased(p<0.01).

3.A Case of Darier' s Disease Treated by the Combined Treatment with Oral Etretinate and Topical Tretinoin Cream.

We report a case of Dariers disease which showed a more effective result using a combined treatment with oral etretinate and topical tretinoin cream than treatment with oral etretinate alone in a 17-year old male patient. Oral etretinate 40mg was given daily and topical therapy with tretinoin cream was applied to lesions of neck and face. Two weeks later, lesions of neck and face showed good improvement compared with the not applied lesions. Topical therapy was then extended to other lesions. Six weeks after treatment, there was a marked improvement. Three months after treatment, most of lesions were nearly gon.

4.Localization of Keratinolytic Proteinase in Skin Tissue Sections of Guinea Pigs with Microsporum canis Infection by Immunoperoxidase Technique in Electron Microscopy.

An immunoperoxdase technique in electron microscopy was used to investigate the ultrastructural site of keratinolytic proteinase (KPase) of M crosporum cans in sections of skin from guinea pigs infected with the same organism. Ultrastructurally, the KPase was present only in the cell walls of the invading dermatophytes as a continuous deposition of the electron-dense reaction product on the inner and outer aspects of the cell wall of the fungal hyphae without deposition in the keratin surrounding the invading hyphae. Our results suggest that the KPase may not play an absolute role in the invasion of dermatophytes into keratinized tissue in vivo.

5.A Comparative Clinical analysis of Arthroscopic Meniscectomy and Arthroscopic Meniscus Repair

There have been controversies on the rationale, surgicl indication, technique and results of meniscus repair.. Authors comparatively studied 100 cases of meniscus injury in 88 patients (arthroscopic total menisectomy: 16 cases, arthroscopic partial menisectomy: 68 cases, arthroscopic meniscus suturing: 16 cases) from February 1985 to March 1992. The results were as follows: 1. Male (61.4%) was much more affected than female. 2. The third decade group was most commonly affected and the most common causes was sports injury (43.2%) 3. The lateral meniscus was more injured (71%), than the medial (29%) 4. In arthroscopic finding, longitudinal tear (37%) was the most common. 5. According to the Tapper and Hoover's criteria, satisfactory result was 56.3% in arthroscopic meniscus repair group.

6.Clinical Significance of Arthrography in Legg-Calve-Perthes' Disease

Legg-Perthes' disease, as avascularity of the femoral head that often arises in the child of a particular age group, is a serious disease of which the frequency is more than the osteochondroses of the other part, and the cause of Legg-Perthes disease is obscure up to now, but the pathogenesis and, pathology are more clearly understood. Twenty patients with Legg-Perthes disease were managed at the department of Orthopaedic Surgery of Hanyang University hospital from May, 1972 till September, 1977 and were studied according to the method of Catterals classification arid arthrogram. The following conclusion were made. 1. The age group with the highest frequency was that from 6 to 9 years (40%) and the ratio of male: female was 6: 1. 2. The stage of disease s progress when patient went to hospital was almost fragmentation stage as 17 persons of the whole. 3. By arthrogram Spherical form of femoral head was 15%; Round form 45%; Flatten form 40%. 4. The longer the period from onset to beginning of treament was the more femoral head flattened. 5. There was the most number of patient as 13 persons of the whole wearing abduction internal rotation brace. 6. From the view point of arthrogram the more spherical the femoral head was the better the prognosis was; otherwise the worse.

7.A Clinical Study on Fracture of Shaft of Forearm Bones

To achieve full pronation and supination after fracture, many authors have pointed out the importance of the length of the bones, the absence of the angulatory deformity and rotational deformity, maintenance of the interosseous space and the curves of the radius, especially the lateral bowing of the radius. Over the years many methods of open reduction and internal fixation or the closed method have been advocated and comparisons have been made between the groups treated by the conservative method and open reduction and internal fixation. The authors have reviewed 108 cases of the fracture of the shaft af the forearm bones from 1 January 1971 to 31 March 1979 which were treated in Department of Orthopedic Surgery, Severance Hospital, Yonsei University College of Medicine. The following results were obtalned from analysis of the cases studies. 1. There was no difference between the conservative treatment group and the open reduction and internal fixation group for fractures of the radius or ulna alone in fracture heallng time, but there was delay in fracture healing time in the open reduction and internal fixation group compared to the conservative treatment group for fractures of both forearm bones. 2. Satisfactory functional results were shown in the open reduction and internal fixation group rather than in the conservative treatment group. 3. Angulatory deformities were shown in 47.2% of the conservative treatment group and 14.8% of the open reduction and internal fixatlon group. 4. In selected cases, an Evan's tuberosity view was taken and rotational deformities were shown in 30.0% of the conservative treatment group and in 8.8% of the open reduction and internal fixation group. 5. There was no difference between the conservative treatment group and the open reduction and internal fixatlon group in non-union rate and delayed union rate.

8.Expression of E-cadherin, Matrix Metalloproteinase, and Vascular Endothelial Growth Factor in Squamous Cell Carcinoma and Adenocarcinoma of the Lung.

E-cadherin is a calcium-dependent epithelial adhesion molecule which plays a role in the initial step of invasion of cancer cells. The step that follows the migration of separated tumor cells is a proteolytic lysis of basement membrane and extracellular matrix by protease of epithelial and endothelial cells such as matrix metalloproteinase (MMP). Vascular endothelial growth factor (VEGF) is known to be an endothelial cell-specific powerful mitogen as well as a vascular permeability factor. This study is aimed to evaluate the correlation between expression of these factors and pathologic or clinical variables and the roles and prognostic significance of those factors in squmous cell carcinoma and adenocarcinoma of the lung. Immunohistochemical stains were performed for E-cadherin, MMP-2, and VEGF in surgically resected specimens from 90 patients with squmous cell carcinoma and adenocarcinoma of the lung. Mean age of the patients was 59.7 years. Histologic type was categorized into 56 cases of squamous cell carcinoma and 34 cases of adenocarcinoma. Mean survival period of the 35 patients was 54 months. The immunohistochemical stains for E-cadherin, MMP-2, and VEGF revealed positive reaction in 67 cases (74.4%), 43 cases (47.8%), and 34 cases (37.8%), respectively. The expression of E-cadherin was higher in adenocarcinoma (82.4%) than in squamous cell carcinoma (69.6%). MMP-2 was expressed in the tumor cells, especially those invading into the surrounding stroma. The expression of MMP-2 was significantly correlated with the survival rate (p<0.05). The expression of VEGF in the tumor cells was significantly higher in cases with lymph node metastasis (p<0.05). In conclusion, these findings suggest that the expression of MMP-2 and VEGF predict poor prognosis of patients with squmous cell carcinoma and adenocarcinoma of the lung and that VEGF may play a role in tumor metastasis.

9.An Experimental Study about the Influence of TGF-β1 upon Fracture Callus Formation

The formation, maintenance, and regeneration of bone is a complex precess involving the interactions of many cellular elements with systemic and local regulators. TGF-β is one of growth factors that play an important role in the formation and remodeling of bone. In vitro studies have suggested that TGF-β regulates chondrogenesis and possibly osteogenesis by affecting replication, gene expression, and structural protein synthesis in bone formation. We investigated the effect of TGF-β1 upon fracture callus formation and maturation in mature rate. Closed femoral shaft fracture was made consistently by three point stress technique after percutaneous intramedullary nailing. TGF-β1 was injected subperiosteally at the fracture site daily for 2 weeks. We examined the effect of TGF-β1 on the fracture healing process with the radiographic, densitometric, histologic, and immunohistochemical methods. The following results were obtained. 1. Radiographic examination demonstrated that TGF-β1 injection group appeared to have more abundant callus formation and earlier callus maturation as compared to the control group. 2. Bone densitometric examination revealed that TGF-β1 injection group had higher bone mineral density and content that the control group. 3. Thermographic examination revealed that TGF-β1 injection group had higher local temperature at the injection area than the control group. 4. Histologic examination suggested that TGF-β1 stimulates and accelerates fracture callus formation and endochondral bone formation. 5. Immunohistochemical examination revealed that chondrocytes at the fracture site in the TGF-β1 injection group seemed to produce type I collagen.
