1.Fatigue and the Related Factors in Well Women.

Fatigue is the subjective sense which people experience and use commonly in their lives. Although factors regards with fatigue have been explored in specific population, minimal study has been devoted to exploring associated variables generally healthy women. This descriptive study examined the relationship of demographic factors and depressions to subjective fatigue among women in the community who have no critical health problems. From June 15 to July 15, 1996, a convenience sample of 255 adult women completed the questionnaire which was developed by the author on the basis of Yoshitake's(1978) and Zung(1965), respectively. Pearson's correlation, t-test, ANOVA and Mann-Whitney test were used to analyze the data with SPSS/PC+ 5.0 for Windows. The results were as follows ; 1) Average fatigue score of the subjects was 12.57(+/-7.15)(range 0~30). Fatigue scores by area were neuroperceptive fatigue(4.81+/-2.90), physical fatigue(3.99+/-2.46) and mental fatigue(3.71+/-2.91) in order. Subjects with the fatigue scores more than 20 were forty four, 17.2 percent of total subjects. 2) Average depression score was 46.1(+/-6.96)(range 16~64), which was not so high. 3) Statistically significant relationship was noted between fatigue and depression scores(r=-.6747, p<.001) that means the more depressive, the more be fatigued. Correlations with respective fatigue area and depression were mental fatigue(r=-.6833, p<.001), neuroperceptive fatigue(r=-.5293, p<.001) and physical fatigue(r=-.5189, p<.001) in order. 4) Presence of disease revealed as the statistically significant variable affecting fatigue scores(t=-4.31, p<.001). Other variables such as age, marriage and job, however, had no statistically significant effect on the fatigue scores. Fatigue can meaningfully undermine quality of life in women who are accustomed to leading active lives. Fatigue is disabling, and a serious symptom to those who suffer from it. It is important for health providers as well as the women to recognize that there exits the relationship between fatigue and depression. Further investigation is necessary to facilitate adequate resolution of the clients' problem from fatigue leading to diminishment of the significance.

2.Histological Changes in the Skin with Innate Ageing.

BACKGROUND: It is a popular notion that cutaneous ageing includes two distinct phenomenon; true ageing, a universal presumably inevitable change attributable to the passage of time alone, and photoageing, changes attributable to chronic habitual sun exposure that are neither universal nor inevitable. Numerous investigations with experimental animals, in vitro skin models have been conducted, although, few histological studies to date have attempted to announce fundamental morphological changes with innate ageing. OBJECTIVE: We compared skin derived from the breast of old and young persons using light microscopy to discern structural changes in epidermal and dermal morphology with advancing age. METHODS: The histological, immunohistochemical studies were performed with normal skin sections of thirty donors who were diagnosed with breast cancer. They were classified into three age cohort groups; nine into group I (22 to 38), twelve into group II(40 to 52), and nine into group III(54 to 87). We chose the breast as an area that might closely resemble intrinsically aged skin. This region is relatively shielded from photoageing by its anatomical location. Analysis of data was performed using the Kruskal-Wallis and ANOVA test for dermal parameters based on a 5-point rating scale, and a simple regression test for a positive rate of immunoreactants. Results : 1. Light microscopic appearance of aged skin revealed a more flattened epidermis than young skin. There was no trend for an increase in epidermal melanin content per unit area on Fontana-Masson staining. There was an age-associated decrease in the Ki-67 positive rate(p<0.001), the density of Ki-67 positive cells declined approximately 1.16% per decade in photoprotected skin(p<0.001). The number of S-100 positive cells declined approximately 4.4/mm width along the dermo-epidermal juction per decade in photoprotected skin(p<0.001). The expression of differentiation markers(keratin 1, involucrin, filaggrin, loricrin) were not different among the three age cohort groups. 2. With advancing age, there was an attenuation in the number and diameter of elastic fibers in the papillary dermis and an increase in the number and straightness of the same fibers in the reticular dermis. The collagen fibers are arranged in sparse bundles in disarray, and/or aggregates of loosely woven, straight fibers in the aged skin. There was an apparent, age-related decrease in the stainability of ground substances in the papillary dermis on colloidal iron staining. Conclusions : Our data documents semi-quantitative differences among three groups in intrinsically aged breast skin and provide the framework for future research to evaluate the ageing process.

3.Parapharyngeal Masses: CT and MR Finding.

PURPOSE: Authors reviewed CT and MR findings of patients with parapharyngeal masses, and their incidence and characteristic findings were studied. MATERIALS AND METHODS: We reviewed 27 CT and 9 MR of pathologically proven 29 patients with primary parapharyngeal mass Neurogenic tumor (17 cases) and other rare masses (3 carotid body tumors, 2 pleomorphic adenomas, 2 cavernous hemangiomas, 1 cavernous lymphangioma, 2 carotid artery pseudoaneurysms, 1 non-Hodgkin's lymphoma, 1 idiopathic cervical fibrosis) were included and the characteristic CT and MR findings were studied. RESULTS: Neurilemmoma was seen as a well-demarcated, solid mass of heterogeneous attenuation or intensity, so that pleomorphic adenoma of minor salivary gland origin could not be differentiated by demarcation, attenuation, or signal intensity of the tumor itself. The direction of carotid artery displacement was not constant in cases of neurilemmomas while the styloid process was constantly displaced laterally in all cases. Characteristic CT and MR findings of other rare tumors were described. CONCLUSION: Neurogenic tumor is the most common parapharyngeal space mass and the direction of displacement of styloid process might be helpful in differential diagnosis from pleomorphic adenoma. Other rare tumors showed very characteristic CT and MR finidings and could be differentiated easily.

4.Immunohistochemical Analysis of Cutaneous Pseudolymphoma.

BACKGROUND: In the skin, it is often difficult to differentiate lymphomas from reactive lymphoid lesions by light microscopic examination. OBJECTIVE: Our purpose was to determine whether immunologic data obtained from mutine-processed specimens could be used to further objective morphologic interpretations. METHODS: We conducted an immunohistochcmical staining in 44 cases of benign and malignant cutaneous lymphoproliferative lesions using nine antibodies, including anti-CD3, UCHL1, MT1, MT2, L26, MB2, BerH2, 123C3, and MIB1. RESULTS: 1. Immunophenotyping with anti-CD3, UCHL1, MT1, L26, and MB2 was useful for the diagnosis of T cell or B cell lymphoma. However, these antibodies showed a lack of specificity for neoplastic cells, 2. Antibody to CD56, 123C3 showed positivity in 4 cases of angiocentric lymphoma, but negativity in 8 cases showing angiocentric lymphoma-like pathology. 3. Antibody to CD30, BerH2 showed positivity in all 6 cases of CD30 positive large cell lymphoma, but negativity in 6 cases showing diffuse lymphoma-like pathology. 4. Antibody to Ki-67, MIB1 showed positivity in more than 30% of infiltrating cells in 6 cases of angiocentric lymphoma, 4 cases of diffuse B cell lymphoma, and in more than 60% of infiltrating cells in 6 cases of CD30 positive large cell lymphoma. CONCLUSION: These observations suggest that immunostaining may provide useful adjunctive information in distinguishing benign from malignant cutaneous lymphoproliferations in paraffin sections.

6.A Case of Cystic Hygroma.

Cystic hygroma, or cystic lymphangioma, is a true, benign, congenital rnulticystic tumor arising from sequestrations of embryonic ly mphatic iissue. Most often it occurs. At birth or early in life, and it is found most commonly in the neck, and its distribution coincides with that of the prirnitive lymph sacs. We report herein a case of huge cystic hygroma on the face and neck in a 2 month-old male infant. The swelling was softly cystic, and was partially compressible, and it was brilliantly translucent. The cystic mass contained a serous fluid. Biopsy findings showed simple endothelium-lined lymphatic vessel and loose, lace-like fibrous tissue stroma, and dilated muscle space filled with serous fluid.

7.MR imaging of oral cavity malignancy.

We retrospectively analyzed the Magnetic Resonance (MR) imaging findings in 41 cases of histologically proved malignant tumors of oral cariey. The diagnostic value of MR imaging in detection and delineation of the lesions was assessed. The value of MRI was compared with that of Computed Tomography (CT) in 14 patients. Thirty-four cases of 41 malignant tumors were squamous cell carcinomas. Adenoic cystic carcinomas (2 cases), malignant melanomas (2 cases), non-Hodgkin lymphomas (2 cases) and mucoepidermoid carcinoma(1 cases) were also included in this study. Most of the lesions were isointense or slightly hyperintense to muscle on T1 weighted images and showed variable degrees of high signal intensity on T2 weighted images. Two cases of malignant melanomas showed characteristic hyperintensity on T1 weighted images. T2 weighted image was better in detection and delineation of tumor in most of the cases. In 6 cases, Gd-DTPA-enhanced T1 weighted image was better than T2 weighted image. T2 weighted image was useful for the evaluation of deep tissue infiltration and T1 weighted image was useful for the evaluation of bone invasion and superficial tissue plane invasion. The lesion was detected only by MR in 3 cases of 14 in which CT was also performed. MR imaging was more sensitive in the evalustion of bone marrow involvement. MR imaging is very useful modality in evaluating oral cavity malignany and is superior to CT in delineation of the as well as in the evaluation of mandible invasion.

8.Effect of Cobalt-Chromium Alloy Surface Treatment When Bonding With 4-Meta/mma-Tbb Resin.

The effects of pretreatment of Co-Cr alloy, including two adhesive primers that contain either MDP or MAC-10, and silicoating on the bond. The results obtained as follows : Strength of 4-META/MMA-TBB resin were investigated using FT-IR SEM, and EDAX. in the SEM observation of surface morphologies, the sandblasted specimen exibited a very rough surface, whereas the surfaces of the two groups primed with either MDR or MAC-10 were covered with a layer of primer, and the surface morphology of the silicoated specimen remained almost the same after sandblasting. Before the thermocycling tests, the group treated with MDP demonstrated the highest mean tensile bond strength and the sandblasted group showed the lowest bond strength. After 20,000 themocyling, the mean tensile bond strength of the sandblasted group exhibited a 50% reduction in bond strength, while the other showed a 20~30% reduction. Observation of the metal-resin interface revealed that in all groups the resin permeated the rough surface formed by sandblasting thereby producing a mechnical bond between the metal and the resin, It was also found that thermocycling resulted in a gap formation at the metal-resin interface of the specimens, and the sandblasted group exhibited a larger gap width than the other groups. in fracture mode, all specimens indicated a cohesive fracture within the resin before thermocycling. However, thermocyling produced adhesive failure at the edge of the resin-metal interface in most specimens, The sandblasted group, which exhibited the lowest bond strength after thermocycling, also demonstrated the largest area of adhexive failure.

9.Multiple Glomus Tumor in Brachial Plexus: A Case Report.

Glomus tumor is a kind of vascular tumor that arises from the glomus body, which regulates skin temperature and is placed in the skin and the subcutaneous area. It is a benign tumor that usually presents in the subungal area. It is relatively common in areas other than the fingers, but its occurrence in peripheral nerves is known to be comparatively rare. We report our experience with a case of glomus tumor arising from the brachial plexus, a rare site of occurrence for glomus tumors.

10.A Case of Phakomatosis Pigmentovascularis Associated with Ocular Abnormalities.

Phakomatosis pigmentovascularis characterized by the association of cutaneous hemangioma and pigmentary nevi was first recognized as a syndrome by Ota in 1947. We describe herein a case of phakomatosis pigmentovascularis alsoiated with ocular abnormalities in a 22-year-old female. According to Hasegawas criteria, this case is lassified as type lIb.
