1.A Case of Darier' s Disease Treated by the Combined Treatment with Oral Etretinate and Topical Tretinoin Cream.

We report a case of Dariers disease which showed a more effective result using a combined treatment with oral etretinate and topical tretinoin cream than treatment with oral etretinate alone in a 17-year old male patient. Oral etretinate 40mg was given daily and topical therapy with tretinoin cream was applied to lesions of neck and face. Two weeks later, lesions of neck and face showed good improvement compared with the not applied lesions. Topical therapy was then extended to other lesions. Six weeks after treatment, there was a marked improvement. Three months after treatment, most of lesions were nearly gon.

5.A Laboratory Study of Diathermy Capsulorhexis.

This study was performed to evaluate histopathologic and physical characteristics of anterior capsule during capsulorhexis with a new high-frequency diathermy.The authors investigated the histopathologic characteristics of anterior capsule according to coagulation time and anterior capsular diameter in diathermy capsulorhesxis(DC),comparing with those in continuous curvilinear capsulorhexis(CCC). Thermally induced histopathologic alter-ation was localized at anterior capsule with coagulation for 0.5 second, while it was extended into cortex with coagulation for 2 seconds.When capsulorhexis was performed with diameter of 6 mmand the margin was pulled with iris retractor,anterior capsular edge was fully extended both in CCC and DC.But with diameter of 3.5 mm,DC mostly led to tear of capsular edge on its extension,while CCC permitted full extension of anterior capsular edge.These results suggest that strength of anterior capsular edge in DC is inferior to that in CCC.However diathermy capsulorhexis is a useful alternative technique in cases,which CCC is difficult to perform,such ashypermature or congenital cataracts.

6.The Relationship of Positive Antimicrosomal Antibody and Postpartum Thyroiditis.

Postpartum thyroiditis is a common but frequently unrecognized disorder, affecting approximately 5% of women during the first 12 months after delivery. It is characterized by transient hyperthyroidism occurring about 14 weeks postpartum followed by transient hypothyroidism at 19 weeks postpartum. Our aim is to investigate the prevalence of positive antimicrosomal antibody in women 3 months postpartum and an association of antimicrosomal antibody with symptomatic and biochemical thyroid disorders. We used data collected from 205 women who visited Dankook University Hospital in 3 months postpartum, Our results showed that the rate of positive antithyroid microsomal antibody was 12.7% in women 3 months postpartum and the prevalence of biochemical hyperthyroidism and biochemical hypothyroidism with positive antithyroid microsomal antibody women 3 months postpartum were 26.9% and 19.2%, respectively. The prevalence of postpartum biochemical thyroid dysfuncion 3 months postpartum was 13.7%. There was no relationship between any of the following factors and thyroid antibody status: fetal distress, birth weight and infant sex, matemal age, experiences and mode of delivery, previous medical disease(such as pstrointestinal and psychotic diseases), experiences of previous abortions, gestational age and past history of thyroid diseases.

8.Toxicity of Benzalkonium Chloride on Corneal Endothelium of Rabbits.

An experimental study was performed on 16 rabbits to evaluate the toxicity of benzalkonium chloride(BAK) on the corneal endothelium. Each rabbit received two drops of 0.01% BAK in the right eye and BSS in the left eye as control. The rabbits were divided into 4 groups: instillation of BAK 20 times at 6-minute intervals in normal cornea(group 1), instillation of BAK 40 times at 3-minute intervals in normal cornea (group 2), instillation of BAK 20 times at 6-minute intervals in de-epithelized condition with the size of 6mm diameter(group 3) and instillation of BAK 40 times at 3-minute intervals in deepithelized cornea(group 4). After the last instillation of BAK, histopathologic examination was performed with electron microscope. Group 1 showed nearly normal corneal endothelial findings, but group 2, 3 and 4 showed enlarged rough endoplasmic reticulum, partially distrupted Goigi apparatus and mitochodria, the presence of vacuoles and phagosomes. Group 4 showed severe destruction of subcellular structures. The results of this study indicate that an exaggerated use of topical drug containing 0.01% BAK may induce corneal endothelial damage, especially when the epithelium was already damaged.

9.Two Cases of Generalized Syringoma.

We experienced two cases of generalized syringoma in a 21-year and a 34-year- old healthy woman. A 34-year-old woman have had numerous yellowish to pink papules on the face, anterior chest, upper part of the back and right buttock for the past twelve years. A 21-year-old woman have had numerous yellowish to brownish papules on the face, neck and anterior chest for the past ten years, and her mother had syringoma on the eyelids. Diapnosis was confirmed by clinical and histopathological findings as generalized syringoma.

10.A Case of Type IIa Hyperlipoproteinemia with Xanthoma Planum , Xanthoma Tendinosum and Xanthoma Tuberosum.

We report a case of a 59 year-old female with hyperlipoproteinernia who had xanthoma tuberosum, xanthoma tendinosurn and xanthoma planum. Yellowish nodules appeared on the right elbow 26 years ago and the lesions gradually increased in size and spread to the elbow, hand, knee, foot and achilles tendon. Except for the skin lesion, she was normal on physical examination. The laboratory findings showed that the serum cholesterol, beta lipoprotein and total lipid were increased. The beta band was rnarkedly elevated on electrophoresis, which suggested type IIa hyperlipoproteinemia. Histopathological findings shc wed many aggregates of foam cells and foreign body giant cells were seen in the dermis.
