1.Effects of calcipotriol(MC 903), a novel synthetic derivative of vitamin D3 on the growth of cultured human keratinocytes and melanocytes.

The cutaneous synthesis of vitamin D in response to ultraviolet radiation exposure is the most important factor in maintaining vitamin D balance in Man. The skin is not only the site of vitamin D synthesis, but also a target organ for calcitriol(1, 25-dihydroxy-vitamin D) which is naturally occuriag, hormonally active form of vitamin E. It is now known that calcitriol inhibits the proliferation of epidermal cells and induces her differentiation. In this study, epidermal keratinocytes and melanocytes were isilated from the neonatal foreskin, and were culturod using a MCDB 153 and modified TIC media, respectively. And then various concentratioris of calcipotriol(MC 903), a synthetic aralogue of calcitriol, were added to each culture. The effects of calcipotriol on the growth of human keratinocytes and melanocytes were investigated. The results were as follows : 1. The addition of calopotriol to human keratinocyte and melalocyte cultures inhibited their proliferation in a dosdependent manner. 2. Calcipotriol had no effects on the melanization process of the melanocyte. 3. Calcipotriol was found to inhibit the proliferation, however it induced the terminal differentiation of cultured keratinocytes, as judged by morphologicai changes. The decreased density of kerationcytes, The formation of cornified cells, and the cellular destruction in a concentration of 10 M of calcipotriol were observed. 4. By using the light atid the electron microscope, we observed that the epidermal thickness was decreased and terminal differentiation was facilitateir. Living Skin Equivalent (LSE) according to the increasing concentration of calcipotriol. A]i)parent cytotoxic effects were observed in 10 M, 10 M of calcipotriol. In summary, the above results indicate that the addition of calcipotriol to the in vitro culture system of human keratinocyte and melanocyte induces the biologic process of terminal differentiation of keratinocytes and inhibits proliferation of keratinoytes and melanocytes in a dose-dependent manner.

5.A Case of Epidermodysplasia Verruciformis Associated with Bowen's Disease.

We present a case of epidermodysplasia verruciformis a.ssociated with Bowen's disease. Patient was 33 year-old male and his skin lesions showed diffuse flat papules on face and neck, many brownish papules on trunk and papillomatous tumors also appeared un scalp and post auricular area, Skin biopsy findings from the flat papular lesion showed histopathologic features of verruca plana and from the papillomatous lesion showed the feature of Bowen's disease. Electron microscopic finding from the flat papular lesion showed marked aggregates of viral particles around the nuclei of epidermal keratinocytes.

6.Flow Cytometric DNA Analysis of Prostate Adenocarcinoma :Correlation with histologic grade and DNA ploidy.

Nuclear DNA content of 32 cases of prostate adenocarcinoma diagnosed 1986-1991 was determined by flow cytometry, with the use of paraffin-embedded archival tissue. The present study was done to define the relationship between clinical stage, histopathological grade, and DNA ploidy. Aneuploidy was found in 10(31.3%) cases including 7 cases of near-tetraploidy. Among diploid tumors, 36.4% were localized disease(stage A and B), 13.6% were characterized by invasion outside the prostate(stage C), and 50.0% showed distant metastasis(stage D). Among aneuploid tumors, 10.0% were stage B, 50.0% stage C, and 40.0% stage D. The degree of glandular differentiation was characterized by the Gleason score and the percentage of sampled tissue involved by carcinoma was graded by Dhom's method. Apparent correlation was found between Gleason grade and Dhom grade(P<0.05). All 13 tumors with a Gleason grade I(score of 2 to 5) were diploid. Four of 9 tumors with a Gleason grade II(score of 6 to 7) were aneuploid(near-tetraploidy 33.3%, aneuploidy 11.1%) and 60.0%, of tumors with a Gleason grade III(score of 8 to 10) were aneuploid(near-tetraploidy 40.0%, aneuploidy 20%). The percentage of aneuploid cases increased with advanced clinical stage, but the relationship between aneuploidy versus clinical stage was not significant. However, it can be concluded that DNA ploidy correlates well with Gleason grade(p<0.05), which may have predictive prognostic value for prostate adeno-carcinomas.

7.A Case of Fox-Fordyce Disease.

Fox-Fordyce disease is a chronic pruritic papular eiruption, usually appearing in women and presenting a strict localization to the areas bearing apocrine sweat glands. More than 90g, of the cases were seen in women, the majority being in the age group of 13 to 35 years. The present report deals with unusual case of 14-year-old boy who developed Fox-Fordyce disease 6 years ago. To our knowledge, it is extremely rare case in Korea.

8.Electrophysiological Classification of Guillain-Barre Syndrome: Sequential Changes in Electrophysiologic Findings.

BACKGROUND: Guillain-Barre syndrome (GBS) is subclassified into acute inflammatory demyelinating polyneuropathy, acute motor or motor-sensory axonal neuropathy, and the other variants. While most studies from Western countries report demyelinating type as predominant form of GBS, some reports from Asian countries suggest the opposite result. Because they are not easily discriminated clinically, electrophysiological study is the most practical method for their subclassification. METHODS: Among the patients who had been admitted to our hospital with typical clinical features of GBS over the past 4 years, we analyzed 32 patients, who had undergone at least 2 serial nerve conduction studies (NCS) throughout the course. In each case, conventional NCS in at least 3 limbs were performed and the criteria of Hadden et al., and 2 other criteria were applied. RESULTS: Using the criteria of Hadden et al., 91% of the patients were classified as primary demyelination, and none as primary axonal pathology. Among the cases of primary demyelination, 21% showed electrophysiological features of axonal GBS at least in one occasion during the serial studies. CONCLUSIONS: Our study suggests that the primary demyelinating type is by far more predominant in our population. Secondary changes of electrophysiological findings in some of our cases suggest that primary demyelination could be misinterpreted as primary axonal pathology without timely serial studies. For the epidemiologic study of larger scale in conjunction with clinical features and immunologic investigations, the protocol and criteria of consensus is necessary, with the qualified normal limit values of each laboratory.

9.Clinical outcome of intrauterine adhesion after Resectoscopy.

OBJECTIVES: From February 1995 to April 1998, those patients who had visited for evaluation of infertility, shown abnormal endometrial pattern on hysterosalpingography underwent resectoscopic operation. We evaluated about its therapeutic effect, recurrence rate of uterine adhesion and effectiveness of its assisted method. METHOD: We have reviewed 45 cases of intrauterine adhesion, classified as a central type, marginal type and multiple type. We used 26F resectoscope made in Storz for operation and inserted Lippes loop or pediatric foley catheter for prevention of readhesion. For promoting reepithelialization, conjugated estrogens(premarin) 5mg daily for 30 - 50 days were given and then 10mg of medroxyprogesterone acetate(provera) were added daily for the last 10 days. We evaluated the uterine cavity condition preoperatively and postoperatively by using hysterosalpingography. RESULT: In 45 cases, 41 cases were followed up postoperatively. 21 cases were markedly improved, 12 cases were improved and 8 cases were not improved or recurred on hysterosalpingography. In 41 cases, for prevention of readhesion 20 cases were used pediatric foley catheter and 5 cases(25%) were recurred. 21 cases were used Lippes loop and 3 cases(14%) were recurred. Pregnancy outcome was as follows;15 cases were pregnant and 10 cases delivered a viable infant, 3 cases aborted spontaneously, 1 case was ectopic pregnancy and laparoscopic salpingectomy was done, 1 case was ongoing pregnancy. CONCLUSION: This report suggests that resectoscopic operation is very effective in the treatment of intrauterine adhesion(therapeutic rate of 81%) and that the use of Lippes loop seems like to have the less recurrence rate than the use of pediatric foley catheter in prevention of postoperative readhesion, but more experience and further follow-up are necessary to obtain more detailed conclusions.
