We have experienced a case of congenital esopageal atresia which was confirmed by autopsy. A 3 days old female infant, birth weight 2,300gm, length 48cm, was admitted to Dept. of Pediatrics, Chonnam University Medical school because of poor sucking power, fever and vomiting. On admission, she was kept in an incubator and whole milk was giver per oral, followed by immediate vomiting and regurgitation through mouth and nose, accompanied by dyspnea and cyanosis. A Nelaton catheter was inserted through the esophagus but met a solid obstructed region at 15cm from the nares. A plain abdomen showed no air in the upper gastrointestinal tract, and in esophagogram with air and Dionocil, esophageal atresia without tracheoesophageal fistula was shown. Gastrostomy was performed but 6 days after operation, she expired. The autopsy findings revealed blind pouched upper esophagus with dilatation at 2nd thoracie spine level and blind pouched lower esophagus just below the diaphragm. A brief review of related literature is also noted.

Severe recurrent attacks of acute febrile neutrophilic dermatosis shortly followed multiple keratoacanthoma in a 43-year-old man with myelodysplastic syndrome. Besides the typical clinicopathological findings of the syndrome(fever, leukocytosis and neutrophilic erythematous plaques of the skin), the patient showed orolingual and conjunctival ulceration and severe cutaneous pathergic reactions. Moreover, to the best of our knowledge, multiple keratoacanthoma has not yet been reported with relation to the acute febrile neutrophilic dermatosis.

It has been well known that successful anesthesia is provided by rendering patients free from apprehension and fear prior to operation. This report is concerned with a new sedative compound which appears to reduce anxiety-tension and relax skeletal muscles. Preanesthetic usefulness of diazepam, which is a derivative of benzodia-zepine and has a high safety index as a preanesthetic medication agent, has been clinically assessed by a standard double blind study on 288 patients in this hospital. They received diazepam(0.3mg/ kg), meperidine hydrochloride (1.0~1.5mg/kg) or saline (1.0ml) via intramuscular route. The following are the results: 1) Diazepam was effective in producing a sound sleep during the preoperative night. 2) Diazepam was effective in reducing apprehension and fear immediately prior to induction of anesthesia. 3) Less nausea and vomiting as well as better sedation were produced by diazepam than by meperidine hydrochloride.

The survival rate and motility recovered after cryopreservation of testicular spermatozoa in testicular sperm extraction (TESE)-ICSl program is low. The purpose of this study was to assess the availability and efficiency of mouse empty zona pellucida in cryopreserving human TESE spermatozoa. Mouse empty zonae pellucidae were obtained by extraction of cytoplasm with or without cytochalasin B treatment. Motile sperm from proven-fertile donor and two azoospermic patients after TESE were individually inserted into empty zona pellucida and cryopreserved. Two to five days after cyropreservation, the frozen sperm were thawed and the rates of recovery and motility were observed. The ooplasmic extraction rates of control (N=80) and cytochalasin B treated oocytes (N=80) were 94.0% and 96.2%, respectively (p>0.05). The post-thaw recovery rates of spermatozoa and rates of motility recovery of ejaculate (N=70) and testicular (N=70) sperm were 97.1%, 97.1% and 95.7%, 94.3%, respectively (p>0.05). The results of this study showed that the mouse zone pellucida is useful for cryostorage of single testicular spermatozoa.

Exercise-induced ischemic ST responses were analyzed in 36 patients who presented with chest pain and had exercise test and the results were compared with their coronary angiographic findings. Among 36 exercise test positives, the incidences of one-, two- and three vessel disease, and left main disease were 25%(9 cases), 30%(11 cases), 25%(9 cases) and 9%(3 cases), respectively. The incidence of multivessel disease(i.e., two-to three vessel disease or left main disease) in patients with ST depression > or =2.0mm was 72% and that in those with ST depression of 1.0-1.9mm was 45%. In patients with downsloping ST depression, the incidence of multivessel disease was significantly higher than that of one vessel disease(86% vs 14%, p<0.001). But both incidences of one vessel disease and multivessel disease were similar in patients with flat and slowly upsloping ST depression. More than two thirds of patients with ischemic ST depression appearing in the first 6 minutes of exercise or those lasting past 7 minutes in recovery were associated with multivessel disease. It is concluded that attention to depth, type, appearance time and duration of ST depression during exercise test is particularly helpful in detecting patients with advanced coronary disease.

Endotoxin is a component of bacterial outer membrane. Endotoxin is generally a heat-stable toxin that has the various biological activities such as pyrogenicity, macrophage and complement activation. To determine the source of endotoxin contamination in our water processing and dialysate-delivery system in Korea, endoxin was monitored in tap water at the inlet of water softening, in processed water, at the inlet of dialysate proportioner and in the dialysate at the inlet and outlet of the dialyzer. The endotoxin levels were 24.9ng/ml at the inlet of water softening and 2.3ng/ml in processed water. The removal rate of endotoxin during water processing is 90.7%. Endotoxin concentration were 3.58ng/ ml at the inlet of dialyzer and 12.4ng/ml at the outlet of dialyzer. There was no statistically significant change between the inlet of dialyzer and the outlet of dialyzer. These result suggested that a reverse-osmosis system appeared necessary for decreasing endotoxin in tap water to a certain concentration, so the level of endotoxin in the dialysate would be minimal.
