2.An Experimental Study and Clinical Investigation on Rotational Deformity in Supracondylar Fracture of the Humerus in Children

In children, the supracondylar fractures of the humerus are common in elbow region, and often associated with various complications and problems in treatment (ie,Volkmann's ischemic contracture, cubitus valgus or varus deformity, rotational deformity). The results of reduction can be measured by Baumann's angle, carrying angle, trochlea-capitellum angle, and displacement of distal radial styloid process on radiological study. So we have made experiments on the process which various three dimensional displacements reflected into two dimensional Baumann's angle and humerocapitellar angle. And we investigated the process of rotation, through various radiological studies by means of molded models of humerus. The results were as follows:l. In experiments, AP & lateral X-ray studies showed no change in rotation of proximal segment of fracture with fixed distal segment of fracture. 2. On the other hand, AP & lateral X-ray showed significant change, in rotation of proximal fracture segment with distal segment of fracture. 3. Supracondylar rotation angle(S.R.A.)depicted as the ratio of the length of proximal fracture width minus that of the distal fracture width in lateral view, to the length of distal fracture width in AP view minus that of the distal fracture width in lateral view. 4. According to the modified Mitchell result, S.R.A. to the result of treatment was average 7.6 degree in excellent group, 15.2 degree in good group and 29.3 degree in unsatisfactory group.

4.Initial Clinical Experience of Extracorporeal Shock Wave Lithotripsy using the Northgate SD-3 Lithotriptor.

Extracorporeal shock wave lithotripsy monotherapy was performed in 72 renoureteral units from 66 patients with the Northgate SD-3 lithotriptor using ultrasound stone localization between June 21, and August 25, 1989. Of 72 cases renal stones were 46 cases( 63.9% ) and ureteral stones 26 cases ( 36.1% ). No regional or general anesthesia were necessary but sedatives or analgesics were required in most patients. Among 67 cases who completed extracorporeal shock wave lithotripsy, the success rate of treatment(rate free of calculi plus that of clinically insignificant residual stone fragments) was achieved 100% below 20mm, 87.5% in 21-30mm and 100% over 31mm and the total mean success rate was 97%. 23 cases( 31.9% ) were required auxiliary procedures such as Double-J stent in 14 cases, push up procedures 6 cases and stone basket 3 cases. Poet-lithotripsy complications were transient macroscopic hematuria in 90%, renal colic 20.8%, steinstrasse 13.9%, fever 6.9% and subcapsular hematoma 2.8% and these complications were controlled with conservative management. We suggest that extracorporeal shock wave lithotripsy monotherapy with the Northgate SD-3 lithotriptor was considered to be effective and safe procedure for the initial treatment of urinary stone.

5.Revitalization of Osteonecrosis of the Femoral head by Vascular Pedicled Iliac Bone Grafting: A Preliminary Report

Idiopathic osteonecrosis of the femoral head is an important and unresolved problem in orthopaedic surgery. Generally the progressive collapse of the femoral head and ultimate osteoarthritis of the hip will ensue requiring joint replacement. But longterm results of total hip joint replacement surgery are unfavorable especially in young patients. Therefore, if alternatives to the toal joint replacement are to be effective, they must be applied especially in young patients with osteonecrosis of the femoral head. Recently we attempted the vascular-pedicled iliac bone graft (VPIBG), using the deep circumflex iliac vessel as a vascular pedicle, in 13 hips of 10 patients with idiopathic osteonecrosis of the femoral head and followed up 9–19 months after operation. Aims of this procedure are to decompress the femoral head hence allowing better circulation, to revitalize the dead head by insertion of live bleeding bone and to give a mechanical support preventing further collapse of the femoral head. Though more follow up studies are required, the preliminary results seen during 9–19 months after surgery are encouraging enough to be reported.

7.Masticator Space Lesions: MRI and CT Findings.

PURPOSE: We evaluated the MR and CT findings of the masticator space lesions in order to identify the differences among the malignant and benign tumors and infectious conditions. MATERIALS AND METHODS: MR and CT findings in 46 cases with proven masticator space lesions were reviewed retrospectively. We analysed the involvement of masticator muscles, adjacent spaces, orbit and intracrahium, homogeneity, necrosis, cystic changes, growth patterns, calcifications, enhancement patterns, MR signal intensity, and CT attenuation. RESULTS: Among the 29 cases of malignant tumors, seven cases were mandibular tumors including four chondrosarcomas, and 22 cases were extramandibular tumors. Malignant tumors of mandibular origin showed large masses with severe bone destruction and epicenter of mandible. Extramandiblular malignant tumors showed the epicenter out of the mandible and less severe bone destruction than mandibular tumors: Among the nine benign tumors, four cases were ameloblastomas which showed the well-defined masses and the expansion of the mandible, and four cases were extramandibular tumors which showed well-marginated extramandibular masses with no bone destruction. Among the eight infectious conditions, five cases were mandibular osteomyelitis with or without abscess formations, and the other three cases were infections from adjacent soft tissue or limited to the soft tissue. CONCLUSION: By careful observations of growth patterns, involvement of the masticator and adjacent spaces, bone changes, and epicenter of the lesions, one can discriminate a mandibular lesion from an extramandibular lesion. With this approach, it is thought to be easier to suggest a dignosis among a wide spectrum of masticator lesions.

9.A Clinical Study of Dupuytren's Contracture

Dupuytren's contracture is characterized by progressive fibroplasia and contracture of palmar fascia with resultant deformity of hands and fingers. Recently it has been shown that the cellular element, called myofibroblast, contributes significantly in the process of contracture. Among the 6 patients we have seen during the last few years, there was a total of 9 hands with Dupuytren's contracture. Seven of these were treated by a partial fasciectomy or by a dermofascietomy with a full thickness skin graft and the results of these two procedures were compared. Electron microsconic studies were done in 3 of the 6 patients and myofibroblasts were found in 2 of them.

10.Prognostic Significance of p53 Overexpression after Hepatic Resection of Hepatocellular Carcinoma.

BACKGROUND/AIMS: p53 mutation is the most common genetic abnormality in human cancers. However, although it has been reported that p53 overexpression in hepatocellular carcinoma (HCC) is associated with the aggressive behavior of tumor, the prognostic significance of p53 overexpression in HCC remains controversial. The aims of the present study were to examine the correlations between p53 overexpression and the clinicopathologic parameters of HCCs, and to determine the prognostic significance of p53 overexpression in HCC. METHODS: Immunohistochemical analysis of p53 overexpression was performed in 105 consecutive cases of HCC who underwent curative hepatic resection. Survival curves were calculated using the Kaplan-Meier method and multivariate analysis of outcome predictors for HCCs was assessed by logistic regression analysis. RESULTS: p53 overexpression was observed in 20 of 105 HCCs (19.0%). Multivariate analysis identified significant correlations between p53 overexpression and microvascular invasion (p=0.027), liver cirrhosis (p=0.035), 1-year survival rate (p=0.016), multiple tumors (p=0.014), and the presence of tumor capsule (p=0.010). The 2-year survival rate was poorer in patients without tumor capsule (p=0.043). CONCLUSIONS: Our results show a positive association between p53 overexpression and microvascular invasion in HCC, and indicate that p53 overexpression is a poor prognostic factor of survival, especially within 1 year after liver resection in HCC patients.
