2.Endovascular Treatment of Proximal Superior Mesenteric Artery Pseudoaneurysm after Stab Injury.

Superior mesenteric artery (SMA) injuries remain a challenge to most trauma surgeons and continue to result in significant mortality despite aggressive management. We report successful management of a proximal SMA injury through endovascular treatment in a 56-year-old man. The patient presented with hypotension due to a stab wound after the epigastrium. He underwent emergency laparotomy and repair of the penetrated wall of the stomach. Right retroperitoneal hematoma at the initial laparotomy expanded and ruptured, requiring a massive transfusion. At the second laparotomy, we performed lateral arteriorrhaphy of the proximal SMA. Follow-up abdominal computed tomography angiography on post-injury day 7 showed a pseudoaneurysm on the left side of the SMA trunk. The patient was treated successfully with an endovascular stent graft.

Congenital cystic adenomatoid malformation and Extralobar Pulmonary sequestration are very rare congenital anomalies. We experienced a 4 year-old female patient who had Congenital cystic adenomatoid malformation in her lower lobe of left lung. We accidently found extralobar pulmonary sequestration associated with Congenital cystic adenomatoid malformation at operation field. The resection of the left lower lobe and the extralobar pulmonary sequestration were performed. The arterial supply of the extralobar pulmonary sequestration was one anomalous artery arised from the thoracic aorta. The Venous drainage of expralobar pulmonary sequestration was intercostal vein into the azygous vein. The patient was discharged without any problem.

7.Polypoid Amelanotic Melanoma on the Thigh.

Amelanotic maliganat melanoma is a comparatively rare disease. It accounts for 1.8~8.1% of all malignant melanoma. It is sometime difficult to diagnose amelanotic malignant melanoma because there is no pigmentation, clinically. Polypoid melanoma is a variant of nodular melanoma, which in depth seldom reaches the reticular dermis. The main part of the tumor is located above the nearby epidermis, raised in the form resembling cauliflower. We report a rare case of amelanotic malignant melanoma with polypoid feature in a 78-year-old woman who presented a single bright red nodule on the left thigh.

Basal cell carcinoma (BCC) is the most common of all skin cancers, and ultraviolet radiation is the major etiologic agent in the pathogenesis of BCC. BCC occurring on areas that are not exposed to the sun seems to have different pathogenesis that is not fully understood. Many histologic variants of BCC exist, reflecting its pluripotential origin from epithelial germ cells. Herein we report a case of BCC on the right axilla, which is an unusual place for its occurrence.

9.A Case of Tinea Faciei Caused by Trichophyton mentagrophytes with Atypical Presentation.

Tinea faciei is a dermatophyte infection of the glabrous part of facial skin most commonly caused by the Trichophyton species. Tinea faciale has been classified as part of tinea corporis but recently for its unique features, it is being considered a separate disease entity. Tinea faciale is a relatively rare dermatophytosis and is characterized by clinical polymorphism, which leads to frequent misdiagnosis and mistreatment with corticosteroids. Long term treatment with glucocorticoids can subsequently lead to tinea incognito. Herein we describe an unusual case of tinea faciei caused by T. mentagrophytes in a 40-year-old women with erythematous facial eruptions, initially misdiagnosed as allergic contact dermatitis and rosacea. By using mycological and histological examinations, dermatophytosis caused by Trichophyton mentagrophytes was confirmed. Oral terbinafine was administered together with topical clotrimazole and hydrocortisone for 4 weeks.

10.Comparing the efficacy of a high pressure spray oral hygiene appliance and a sonic vibration toothbrush in reducing dental plaque.

OBJECTIVES: This study aims to compare home care dental devices for their dental plaque removal ability. METHODS: A single blinded randomized crossover clinical study design was used to measure plaque index (Turesky Modification Quigley-Hein Plaque Index). RESULTS: All 3 groups showed a statistically significant decrease in the plaque index after usage (P<0.001). In manual tooth-brushing group, the teeth on the right dental arch showed a greater decrease in the plaque index than on the left arch (P=0.041). All appliances showed greater plaque reduction in the upper right posterior teeth than in the lower right posterior teeth (P=0.009, 0.004, 0.007). CONCLUSIONS: The results of this study support development of more effective oral hygiene appliances and emphasis on oral hygienic education.
