2.Comparing Oral Health Care Awareness and Practice in Pregnant Women with and without Oral Health Education Experience.

PURPOSE: The purpose of this study was to examine differences in awareness and practice of oral health care in pregnant women with and without oral health care education experience. METHODS: Participants in the survey for this study were 221 pregnant women in female exclusive hospitals located in B city and G city. Data were collected from October to December, 2014 using self-report structured questionnaires and analyzed using SPSS/Win 21.0 program. RESULTS: The number of pregnant women experienced in oral health education was 36.2%. The oral health care awareness of pregnant women experienced in oral health education total score was 15.14±1.96, and inexperienced pregnant women total score was 14.14±2.63 (t=3.26, p=.001). The oral health care practice of pregnant women experienced in oral health education total score was 12.90±2.48, and inexperienced pregnant women total score was 10.95±4.06 (t=3.89, p<.001). The relationship between the awareness and practice of oral health care was positively correlated. CONCLUSION: Based on this study outcomes, considering the importance for oral health education in the antenatal care program when the pregnant women visiting the hospital for medical check-up.

3.Effects of octreotide on the contractility of isolated rat vas deferens.

This study was performed to investigate the effect of octreotide on the contractility of rat vas deferens. The -smooth muscle strips isolated from the prostatic portion were myographied in isolated organ bath. Electric -field stimulation (monophasic square wave, duration : 1. mSec, voltage : 50 V, frequency : 5 Hz or 30 Hz, train : 10 Sec) produced reproducible contraction. The contraction was composed of two component, first phasic component (FPC) and second tonicc component (STC).. These contractions were abolished by -tetrodotoxin (1 microM). Octreotide inhibited the field stimulation induced contractions both FPC and STC concentration- dependently. The FPC was decreased by a desentization of purinergic receptor by pretreatment of mATP, and the STC was decreased by pr,,creatment of reserpine (3 mg/kg, EP) 24 hours before experiments. Octreotide reduced the field stimulation induced contraction in the presence of mATP and of reserpinized muscle strips. The inhibitory effect of octreotide was more potent at 5 Hz than at 30 Hz. Octreotide did not affect basal ton and exogenous norepinephrine- or ATP-induced contraction. These results suggest that octreotide inhibit the contractility of the isolated rat vas deferens by inhibition of the release of neurotransmitters, both ATP and norepinephrine from adrenergic nerve terminal.

4.Development of a Two Triplex Real-Time Polymerase Chain Reaction for Rapid Detection of Six Carbapenemase Genes in Enterobacteriaceae

Carbapenem resistance is a serious clinical and public health threat. Carbapenemase can confer carbapenem resistance, and most carbapenemase genes are plasmid encoded so resistance can easily spread. In this study, we aimed to develop a novel system based on the TaqMan platform for the rapid detection of 6 clinically prevalent carbapenemase genes: The triplex assay was verified by testing genomic DNA of 6 carbapenemase-producing No amplification was detected among the negative samples. The result showed 100% concordance with the genotypes previously identified. The entire assay, including DNA extraction and real-time PCR, was completed within 2 hours. The newly developed triplex real-time PCR assay was useful for the rapid, accurate and simultaneous detection of 6 carbapenemase genes in Enterobacteriaceae, suggesting its potential to allow an early decision on the appropriate treatment, management, and prevention of the spread of resistant infections in hospitals.

6.An Accidental Intrathecal Morphine Administration Associated with Total Spinal Anesthesia.

The epidural injection on morphine is an effective method for postoperative pain management. The associated side effects have, however, precluded its widespread use in a variety of clinical settings. Intrathecal administration of morphine incurs a high incidence of pruritus, nausea and vomiting, somnolence, urinary retention and life-threatening respiratory depression in severe cases. We report here one case of accidental total spinal anesthesia with lidocaine, followed by intrathecal injection of morphine. A 39-year-old female with an ovarian tumor was scheduled for a total abdominal hysterectomy under epidural anesthesia. After the epidural space was identified erroneously, 20 ml of 2% lidocaine mixed with 1:200,000 epinephrine was administered into the epidural space, immediately followed by injection of morphine 3 mg. All signs of a total spinal block were observed. She was then intubated and her respiration was controlled without delay. The schedulled operation was carried out uneventfully for 1 hour and 20 minutes. In the recovery room, a bolus injection of naloxone 0.4 mg was performed for prevention of respiratory depression. Dripping of naloxone 0.4 mg/100 ml/hour was continued for 20 hours postoperatively. However, her respiratory rate started to be decreased to 9/min around 8 hours after the surgery so that naloxone 0.2 mg was again injected intravenously. Headache and other side effects were not observed. The patient was out of ICU and discharged on the 6th postoperative day.

A case of multiple myeloma with massive pleural effusion is reported. A 53 year-old previous known multiple myeloma patient vistited our hospital complaining of cough with sputum. Radiologic study revealed multiple osteolytic bony lesions and left side pleural effusion. The effusion were bloody exudates containing numerous atypical plasma cells. The tumor cells showed pleomorphism, eccentric nuclei, prominent nucleoli, perinuclear halo, multincleation, and chromatin patterns of occasional cart-wheel appearance. The cytological examination of pleural fluid established the malignant nature of the effusion with multiple myeloma.

8.The Effect of Salbutamol on Succinylcholine - induced Hyperkalemia.

In the present study, patients were traumatized with muscle-crushing injuries, or they were non-traumstized. The plasma concentration of potassium and sodium were measured before the induction of anesthesia and at selected intervals after the administration of succinylcholine(SCC) in some patients were inhaled the beta2-agonist, salbutamol (about 800 ug) via an endotracheal tube and in the others were not given it. There was no significant difference in the base lines of potassium and sodium between the traumatized and nontraumatized patients. Salbutamol diminished the rise of potassium(-0.30 to 0.09 mEq/L) but did not affect the plasma sodium level. There was no significant side-effect of this agent, but sinus tachycardia was the only thing noted. These results are consistent with the proposed mechanism that the decrease in serum potassium due to salbutamol is most likely a shift of potassium ions from the extracellular space to the intracellular space. Inhalation of salbutamol may be a useful method for the prevention of SCC induced hyperkalemia without any serious side-effect in patients with preexisting hyperkalemia properties who have gone through traums.

9.A Study on coding application of ICD-9.

In Korea medical record administrators/technicians are coding diagnoses and procedures of discharged patients based on their medical records mostly using International Classification of Diseases, 9th revision(ICD-9) and International Classification of Procedures(ICPM) by WHO. This study examined consistency of coding in 63 hospitals in the year of 1992. The statistical data showed great inconsistency in coding patterns among many hospitals. The main reasons of inconsistency were coders errors, ill-defined diagnoses/procedures, no unified route to make new code numbers for the new or ambiguous diagnoses/procedures, inconsistency of selection of using optional(additional) codes, and inconsistency of reference records on coding. Near half of the hospitals do not recheck the accuracy of coding after completion of medical records by physicians. Most of the coders review operation record, admission and discharge record, discharge summary, progress notes, pathology report, and consultation record as references on coding, but 14 hospitals do not review the whole record when they code diagnoses and procedures. Twenty-three hospitals discuss with physicians when they have questions in assigning code numbers. Further study should be done continuously for valid and reliable statistics of diseases and operations and for establishing a systematic unified channel for the new and ambiguous cases.

10.Comparison of Blood Gases in Patients during the Reeovery Period.

In the present study, an effort was made to evaluate the degree of hypoxemia in the early recovery period with general anesthesia. The patients were chosen in each of the following 3 groups, upper abdominal, lower abdominal and non-abdominal operations. The blood samples were taken preperatively, on arrival and 30 minutes after arrival in the recovery room. The blood gases on arrival showed significant decreases of PaO2 and increases of PaCO2 in all three groups in comparison with the preoperative gases. The blood gases at 30 minutes after arrival showed significant increases of PaO2 from the arrival data, in all three groups however there were no significant differences in Paco2. AS a result of this study, it is suggested that conservative and safe management of patients is better to provide supplement oxygen in the earlyer recovery period to all patients with general anesthesia.
