3.A Clinical and Serologic Study of 21 Cases of Tsutsugamushi Disease Confirmed by Serologic test.

Tsutsugamushi disease is an acute febrile disease caused by Rickettsia tsutsugamushi, and which has been reported with increasing frequency thorough the nation since 1986. We experienced 21 cases of Tsutsugamushi Disease diagnosed with serologic test occurring in Taegu city and Kyungpook province during October-November, 1989. The results of survey are as follow. 1) Of 21 cases, 12 (57%) were males and 9 (43%) were females, and the peak incidence was the 4th decade. 2) The outbreak was in October to November and the peak incidence was in October. 3) The most frequent symptoms were fever and chill (100%), myalgia (95%), headache (90%), Eschar and rash were observed in 18 patients (86%) and the eschar was detected in all over the body, especially thorax (33%) and lower extremity (22%). 4) Laboratory features were SGOT elevation (83%), SGPT elevation (61%), LDH elevation (67%), leukocytosis (38%). 5) Indirect immunofluorescent antibody test was done in 18 patients and the antibody titer was above 1:320 in all patients. 6) The chloramphenicol, tetracycline or doxycycline regimens were very effective and mean duration of defervescence from initiation of therapy was 1.3 days. 7) The complication such as meningitis or shock, was not seen.

4.Various Perforationg Artery Pedicled Flaps.

The recent development of microsurgical tissue transfer has enabled any defect in any area to be reconstructed with free flaps. However, the need for a more functional and thinner flap has been raised due to bullkiness of the flap and donorsite morbidity. For better functional and aesthetic results, various perforator flaps excluding muscles or adjacent subcutaneous tissue have been recently reported. We report 44 cases of perforating artery pedicled flaps from April 1995 to August 1998, including 21 cases of anterolateral thigh flap, 12 cases of gluteal artery perforator flap, 4 cases of posterior interosseous flap, and 7 cases of paraumbilical perforator flap for various soft tissue defects. Even though marginal necrosis of flap occurred in 9 cases, complete healing without significant problem was possible. The advantages of perforator flaps are as follows: 1. It is possible to obtain a relatively thin fasciocutaneous flap, but still with sufficient volume 2. Donor site morbidity was reduced without damage of main artery. 3. In spite of diverse vascular pattern of the perforator, the location of perforating arteries can be Detected with relative ease and mapped preoperatively with an ultrasound Doppler. The need of meticulous and tedious dissection could be a sole disadvantage of these flaps. This report describes the clinical experience with a perforator-based flap, anticpating applications of many other types of perforator flap in the future.

5.A Case of Anti-Neutrophil Cytoplasmic Antibodies (ABCA) Positive Wegener's Granulomatosis.

Wegener's granulomatosis is a disease of unknown etiology that is characterized by the clinicopathologic complex of necrotixing granulomatous vasculitis of the upper and lower respiratory tract, glomerulonephritis, and variable degrees of small vessel vasculitis. Recently Antineutrophil Cytoplasmic Antibody (ANCA) has been reported to be a highly specific test for the diagnosis of Wegener's granulomatosis. We have experienced a patient of Wegener's granulomatosis in a 11 year old girl who was admitted with complaints f arthralgia, hematuria, convulsion and associated with otitis media and sinusitis. Serologic test of C-ANCA was positive and histologic findings of the kidney showed crescentic glomerulonephritis with sclerosis and surrounding infiltration of multinucleated giant cells. Patient was treated with pulse methylprednisolone without improvement. The clinical course progressed rapidly and expired due to the renal failure, gastrointestinal bleeding and status epilepticus. A brief review of literatures was made.

6.Effect of Focal and Grid Pattern PASCAL Photocoagulation Treatment in Diabetic Macular Edema.

PURPOSE: To investigate the effect of focal and grid pattern PASCAL photocoagulation treatment in diabetic macular edema patients. METHODS: A total of 72 patients (75 eyes) who were diagnosed as diabetic macular edema were retrospectively analyzed. The patients were divided into 3 groups as follows: group 1 (31 patients, 31 eyes) received an intravitreal bevacizumab injection, group 2 (17 patients, 20 eyes) underwent focal and grid pattern PASCAL photocoagulation treatment, and group 3 (24 patients, 24 eyes) underwent focal and grid pattern argon photocoagulation treatment. Macular thickness and visual acuity (logMAR) of 75 eyes were compared among groups before and after treatments. RESULTS: Visual acuity improved in 13 eyes (65.0%) in group 1, 16 eyes (51.6%) in group 2, and in 10 eyes (41.6%) in group 3; however, there was no statistical significance. Macular thickness reduction occurred in 24 eyes (77.4%) in group 1 and 13 eyes (65.0%) in group 2 which were statistically significant decreases at only 1 month after treatment (p < 0.05). CONCLUSIONS: Focal and grid pattern PASCAL photocoagulation is a good therapeutic option for diabetic macular edema. Focal and grid pattern PASCAL photocoagulation is as effective as intravitreal bevacizumab injection or focal and grid pattern argon photocoagulation treatment.

7.Effects of Positive Acceleration on Atrial Natriuretic Peptide (ANP), Plasma Renin Activity (PRA), Kidney and Testis.

Hypergravity has been shown to affect the human physiology, especially the kidney and heart. The kidney is affected during or after tests on positive acceleration. PRA was increased after positive acceleration. ANP has inverse relationship with renin, which has sodium and water retention. The experiment was design to investigate possible ANP decrease and changes in function and structure of the testis, kidney after exposure to 9G, 120 sec positive acceleratory forces as imposed on rat by the Human Centrifuge. As result, positive radial acceleration has made no change in concentration of ANP in statistically with increasing PRA, After acceleration, the glomerulus of the kidney showed no ischemic changes, but tubule of the kidney showed containing of cast. The seminiferous tubule of the testis showed normal.

8.Effects of Angiotensin III in Rabbit Corpus Cavernosum Smooth Muscle Contraction: Comparing with Angiotensin I and Angiotensin II.

PURPOSE: Angiotensin(ANG) II regulates tone of penile smooth muscle for erection. ANG III is a product converted from ANG II by aminopeptidase A. The effects of ANG III have not been clarified in the penile corpus cavernosum. The purpose of the present experiment was to determine whether the ANG III has regulatory function in the control of rabbit corpus cavernosum smooth muscle tone. MATERIALS AND METHODS: A strip of rabbit corpus cavernosum was mounted in an organ chamber to measure the isometric tension. We compared the effects of ANG III(10-7M to 10-5M), ANG II(10-8M to 10-6M) and ANG I(10-7M to 10-5M) on the contractility of the corpus cavernosum smooth muscle. RESULTS: ANG III, ANG II, and ANG I contracted corpus cavernosum smooth muscle strips dose-dependently. The contraction of smooth muscle induced by ANG III was 10 fold less by ANG II. Contractile response to ANG III was not attenuated by captopril(angiotensin converting enzyme inhibitor). Contractile response to ANG III was significantly inhibited by Dup 753 of 10-7M(type 1 specific ANG II receptor inhibitor) but not inhibited by PD 123,319 of 10-6M(type 2 specific ANG II inhibitor). CONCLUSIONS: The present results suggest that ANG III is involved in the regulation of corpus cavernosum smooth muscle tone, and contractile effect to ANG III produced via activation of type 1 ANG II (AT1) receptor. The rank order of potency of contraction was as follows, ANG II>ANG IIIANG I.

10.Gottron's Acrogeria Syndrome: a case report.

Gottron's acrogeria syndrome is a rare genetic disease of unknown etiology with some features suggestive of accelerated aging. The typical clinical presentation consists of premature aging of the skin with atrophy of the cutis and the subcutis. After a brief review of the literature available on this syndrome, we report a case of Gottron s acrogeria syndrome in a 19 year-old man whose right rectus femoris muscle had a fibrotic contracture.
