1.A Case of Porencephaly.

A 42-year-old male with headache, right homonymous hemianopsia, seizure, and right hemiparesis had been treated. A CT scan revealed a large ovoid well-definded area of cerebrospinal fluid densty in the left posterior parietal lobe, which appeared to communicate with the body of the ipsilateral lateral ventricle. A conray ventriculogram showed a cavity of 6+/-5cm in the left posterior parietal lobe, and a communication with the body of the ipsilateral lateral ventricle was visualized. The porencephaly was managed by creation of as large a window as possible to the ipsilateral ventricle, because the communication with the body of the lateral ventricle was very narrow. Microscopic, pathological evaluation confirmed the diagnosis of porencephaly due to an inflammatory lesion of an undetermined origin. The patient was discharged without any evidence of neurological deficit except the right homonymous hemianopsia.

2.A Case of Reticulum Cell Sarcoma(Microglioma) of the Brain.

A rare case of primary reticulum cell sarcoma(microglioma) of the brain in a 30-year-old female is reported. The tumor was located in the left parietal lobe. Brain CT scan showed a slightly high attenuated area with well demarcation in the left parietal lobe, and a tumor staining was appeared in the delayed arterial phase of the left carotid angiogram. Osteoplastic craniotomy on the left parietal area was performed for removal of tumor. Then the patient received radiation therapy with a good result.

3.A case of obstructive sleep apnea syndrome in childhood.

The obstructive sleep apnea syndrome can occur due to various etiologies in children. In otherwise healthy children, adenotonsillar hypertrophy is the leading cause of childhood obstuctive sleep apnea. Obstructive sleep apnea caused by adenotonsillar hypertrophy can lead to a variety of symptoms and sequelae such as behavioral disturbance, enuresis, failure to thrive, developmental delay, cor pulmonale, and hypertension. So if obstructive sleep apnea is clinically suspected, proper treatment should be administered to the patient after diagnostic examinations. More than 80% improvement is seen in symptoms of obstructive sleep apnea caused by adenotonsillar hypertrophy in children after tonsillectomy and adenoidectomy. However, when it is impossible to treat the patient using surgical methods or residual symptoms remained after tonsillectomy and adenoidectomy, additional treatments such as weight control, sleep position change, and continuous positive airway pressure (CPAP), should be considered. This paper reports a case using weight control and Auto-PAP to control mild sleep apnea and snoring, which in long-term follow-up were not resolved after tonsillectomy and adenoidectomy for severe obstructive sleep apnea.

4.Studies on the life cycle of Culex pipiens pallens in Korea.

The present studies were carried out, under natural room condition from June to July in 1969. The period required for growth of eggs and larva varied according to various temperatures and methods of rearing. The average number of eggs laid by the Culex pipiens pallens was 137 per egg raft. The period of gonotrophic cycle was from 3 to 6 days: incubation period of eggs was from 1.5 to 2 days: larva period was from 10 to 14 days: pupa period was from 2 to 3 days. Accordingly, the period from the beginning of gonotrophic cycle to the adult mosquitos in next generation required 25 days at the longest and 16.5 days at the shortest.

5.Doppler-Ultrasonographic Finding of Air in the Portal Vein: A Case Report.

PURPOSE: Classically air in the portal vein has been detected on plain radiography, but computed tomography and ultrasonography have been shown to be more sensitive. We report a case of air in the PV in a 10-day-old infant with pneumatosis intestinalis with its ultrasonographic and Doppler findings. MATERIAL AND METHODS: The patient was a 10-day-old infant born by cesarean section at 41 weeks. Simple abdomen film revealed branching pattern of radiolucent air shadows within the contour of liver, gas distention of bowel loops and thickenod bowel walls with lincar intraluminal air shadows in abdomcn, suggesting neerotizing enterocolitis. So we performed Doppler ultrasonography. RESULTS: Ultrasonography showed branching pattern of hyperechogenic dots along the lumen of left portal vein. The color Doppler study revcaled an aliasing duo to increased velocity and whirling pattern of blood flow, and the Duplex Doppler spectral display showed sharp, vertical bidirectional spikes by air in portal vein. CONCLUSION: Air in the portal voin can be easily diagnosed by the followign signs:hyperechogenic dots in the portal vein on ultrasonography and vertical, sharp bidirectional spikes superimposod on the usual Doppler tracing of the portal vein on Duplex ultrasonography.

6.Decompressive Craniectomy for Acute Cerebral Infarction.

We present a series of 10 Patients(Seven men and three women with an average age of 53 years) who underwent decompressive craniectomy for treatment to massive brain swelling following acute cerebral infarction. Clinical signs of cerebral herniation(anisocoria or fixed and dilated pupil, and/or hemiplegia with decerebrate righidity) were present in all patients. Computed tomography and magnetic resonance imaging showed the mass effect by cerebral edema through midline shift. All patients were treated with an extensive craniectomy and duroplasty. Among them, one recovered without neurological deficit, three were moderately disabled but functionally dependent, three remained in a persistent vegetative state and three died within 9 days after surgery(good recovery=1, moderate disability=3, persistent vegetative state=3, death=3). The results suggest that decompressive craniectomy can be an useful lifesaving procedure for massive cerebral edema following widespread hemispheric infarction.

7.The Clinical Follow-up Study on Total Hip Replacement

Total Hip Replacement is well documented and widely used procedure for painful arthritic hip in past two decades and each year still seems to be increased in numher of total hip replacement. And recently, there is a few reports of follow-up study on total hip arthroplasty in our country. The aim of this study was to present the clinical material which was performed on 27 hips of 21 patients who were treated at Dept. of Orthopedic Surgery. College of Medicine, Chungnam university from jan, 1979 to Jan. 1982. The longest follow up was 4 years, shortest was 8 months, average time was 2 years and 8months The results were obtained as follow; 1. The age incidence ranged from 26 to 69 years old, and average were 47 years old. 2. There were 17 males (81%) and 4 females (19%). 3, The causes of hip disease were mostly avascular necrosis of the femoral head (67%). 4, The most commonly used prosthesis were Muller type with trochanteric ostcotomy approaches. 5. Several complications were found: perforation of acetabulum with bone cenent in pelvis, postperative pneumonia wire breakage and painful bursitis around the trochanteric area which was on osteotomy sites. 6. Postoperative functional evaluation was evaluated by method of d'Aubigne and Postel, and improved from 10.0 scores to 15.8 scores. Relief of the pain is the most effect of the total hip replacement in this study, But case are not enough to more detail evaluation, especially, the problem of loosening of stem, acetabular cup wearing and heterotopic bone formation after total hip replacement.

9.Brain Stone as Calcified Arteriovenous Malformation: Case Report.

A 35-year-old man, with a previous history of intermittent generalized seizure attacks since 13 years of age, presented with status epilepticus and spastic right hemiparesis. Plain skull films showed a mottled, granular calcified core and a surrounding halo of calcification in the subcortical area of the central fissure, and a smooth erosion of the overlying skull inner table. Left carotid angiogram revealed neither definite mass effect nor abnormal vascularities. Surgery through a left parietal craniotomy was performed. Upon direct inspection of the brain a 2.5+/-3.0cm, stony hard, generally circular mass was noted within the left frontal lobe. Although embedded within the frontal lobe, the mass was partially covered by a gliotic cyst which might be made from cortical tissue. The tumor was obviously calcified, demonstrating numerous superficial white excrescences, and readily removed without difficulty. Microscopic, pathological evaluation confirmed the diagnosis of densely calcified arteriovenous malformation. The patient was discharged without any evidence of neurological deficit except dull mentality.

10.A Case of A-V Malformation Associated with Porencephaly.

Authors report a rare case of arteriovenous malformation associated with porencephaly in left frontal lobe of a 35 years old man. Local brain atrophy is generally ascribed to local pressure effect by the vascular malformation or to localized circulatory disturbances, so as in this case.
