2.Effects of Exercise on Glycemic Control in Patients with Type 1 Diabetes Mellitus.

PURPOSE: Exercise is frequently recommended in the management of diabetes mellitus(DM) and can improve blood control by increasing insulin sensitivity and psychological benefits. Exercise can result in good glycemic control in type 2 DM, when combined with diet and drug therapy. However, in type 1 DM, the expected improvement in glycemic control with exercise have not been clearly established. Effects of exercise on glycemic control in patients with type 1 DM were investigated. METHODS: 20 patients with of type 1 DM, who were no retinopathy, neuropathy, nephropathy and cardiac disorders, were enrolled and exercised for 30min. with 50% of individualized maximum VO2. Blood sugar concentration were measured before, immediate and 15min after exercise. The results were evaluated with HbA1c, C-peptide and DM duration. RESULTS: Blood sugar concentration were significantly decreased from pre-exercise 198+/-9.7mg/dL to immediate 145+/-7.1mg/dL and at 15min. Post-exercise 134+/-4.0mg/dL(P<0.05). In case with HbA1c<10%, there was significantly decreased in blood sugar level from pre-exercise 176+/-9.7mg/dL to immediate 123+/-63.2mg/dL and at 15min post-exercise 113+/-1.9mg/dL(P<0.05). In case with HbA1c>10% there was also significantly decreased in blood sugar levels 222+/-64.7mg/dL, 169+/-6.1mg/dL, 157+/-1.2mg/dL respectively(P<0.05). Group with moderate decreased blood sugar(40-99mg/dL) was 8 patients(42.1%), group with mild decreased blood sugar(<40mg/dL) was 7 patients(36.8%) and group with decreased more than 100mg/dL was 4 patients(21.5%). There was no correlation in degree of decreased blood sugar concentration among HbA1c, C-peptide, DM duration. CONCLUSION: Proper exercise in type 1 diabetic children can decrease blood sugar level significantly and make good glycemic control and can decrease DM microvascular complications in addition to motivating physically active lifestyle.

5.A Study on the Normal Position of Angiographic Sylvian Triangle in Koreans.

The determination of the normal position of angiographic sylvian triangle was carried out in the 100 Korean subjects over the age of 12 in accordance with Vlahovitch's method for the clinical purpose of evaluating distorsion and displacement of the sylvian triangle in the pathological condition. And each measurement of the anterior and posterior extremities and inferior point of sylvian triangle was compared with the determination by Vlahovitch. The following results were obtained: 1. The anterior extremity of sylvian triangle was close to or on the line MM in the great majority, showing the mean location at 0.89+/-2.69 mm in front of the line. The posterior extremity of sylvian triangle revealed more variable position, at 9.60+/-7.94 mm posterior to the line AA The inferior point of sylvian triangle lies at the junction of one-fourth of AA and one-third of AO with variation of 2.21+/-2.43 mm above the line of one-fourth of AA and 2.28+/-2.04 mm posterior to the line of one-third of AO. 2. Angles of sylvian triangle: Anterosuperior angle:76.28((P-95(12.77(). Posterior angle:27.91((P-95(6.81(). Inferior angle:73.73((P-95(12.49(). 3. Height of sylvian triangle: One-fourth the hemispheric height. 4. Length of superior insular line: One-third the hemispheric height. 5. Distance of superior insular line from external meatus along the hemispheric height: One-half the hemispheric height.

Recently therapeutic efficacy of stomach cancer was greatly improved due to early diagnosis and irnproved diagnostic and therapeutic modalities. Especially in early gastric cancer, 5 year survival rate is near 100%. Recarrence of early gastric cancer after curative surgery is rarely reported. Recently authors experienced one case of recurrent stomach cancer arising from remmnant stomach 14 years after gastrectomy of EGC type I(m), so we reports it here with review of literatures.

8.Radiologic Aspects of Moyamoya Disease.

A retrospective study was made in 21 patients who angiographically diagnosed as moyamoya or moyamoya-like disease over the last 10 years. Computed tomography of the patients showed semorrhage, infarction or normal. Carotid angiography revealed various stages unilaterally or bilaterally. Seventy-five percent of the venous phase on the side of arterial occlusion demonstranted some abnormalities. Two children with unilateral abnormality showed arterial occlusion at supraclinoid portion of internal carotid arteries. On the other hand 5 adults with unilateral abnormality showed occlusion at variable sites of internal carotid artery.

9.Bougie Dilatation of a Patient with Esophageal Lye Stricture Fed with Gastrostomy Tube for 21 Years: A case report.

The patient was a 51 year-old woman suffering fraen dysphagia due to upper esoyhageal lye stricutue whieh had developed as a result of a suicide attempt 21 years ago. Shortly after that, she underwent feeding gastrostomy and has lived in the gastrostomy state for 2l years. After admission, she underwent a barium esophagoram which revealed a near total obstruction at the cricoid cartilage level. Bougie dilatation with American Dilation System was tried on day 3. But the spring tip marked guide wire which was to be used with the American Dilation System could not be passed through the stricuture. Therefore, we performed a bougie dilatation using angiographic guide wire M(H-AG-35in-150 cm) with success. On day 14, she underwent a barium esophagogram which revealed her improved condition, and she was discharged on day 16.
