1.Production of CD44v6 Antibody Fragments and Comparision of Their Speciticities.

CD44v6 was known as tumor marker for tumor progression and metastasis in various kinds of carcinomas. The CD44v6 monoclonal antibody was produced by cell cultures or mouse ascite fluids using CD44v6 hybridoma cells, and its immunogloburin G (IgG) was purified by Protein A column. Using immobilized ficin and cysteine, the antibody fragment Fab was produced and purified by Protein A. Four CD44v6 scFv molecules were produced from the recombinant DNA and phage antibody technology and prurified by His-tag affinity chromatography. In order to inspect the function and specificity of each antibody molecule, western-blotting and ELISA against CD44v5-6 recombinant proteins and irnmunodetection in human ovarian carcinomas were estabilished. The results showed that immunodiagnosis did not distinguish the types of antibody fragments, but western-blotting and ELISA results did show some difference of their specificities and biological properties. These studies will contribute as a model study for the immunodiagnosis and therapy using the IgG, Fab and scFv of CD44v6 antibody to obtain the early detection of tumor progression and metastasis using immunoscintigraphy.

2.Cases of Vaginoplsty Using Pudendal-thigh Flaps.

We performed 3 cases of pudendal-thigh flaps in patients with congenital absence of the vagina from June, 1996 through February, 1999. Pudendal-thigh flaps are posterior labial artery axial flaps based on the terminal vessels of the internal pudendal artery. The flaps are raised bilaterally in the groin crease just lateral to the labia majora and are then transposed toward the midline and sutured together to form a neovagina. With adequate flap dissection, reliable blood supply is maintained. Thus, complications such as flap necrosis do not occur and early wound healing is possible. It is s relatively simple technique which can be completed with little blood loss. This method can be completed in one stage, and the prolonged use of stents of dilators is not necessary, since the reconstructed vagina is stable. The linear scars of the donor sites are well hidden in the groin crease and perineum. Normal function is maintained postoperatively without change in sensation. The authors report these methods with a review of the literature.

4.Taxonomic Study on Inocybe in Korea.

Dried specimens of the genus Inocybe collected from mountain areas throughout the Korean country from 1982 to 1998 and preserved in NIAST were investigated. Out of them, Inocybe hystrix were confirmed as an unrecorded species in Korea. In 1991 Lee et al., previously recorded Inocybe kasukayamensis only with its scientific name and korean common name. In this study we report it with full descriptions of morphological characteristics and diagnosis of micro-structures of this species.

5.Affecting Factors of Outcome of CC/hMG Ovarian Stimulation and Intrauterine Insemination.

OBJECTIVE: We have attempted to identify prognostic factors regarding CC/hMG ovarian stimulation and IUI in infertility and to seek factors valuable in planning infertility treatment and predicting the success rate of IUI therapy in individual couples. METHODS: The variables selected for initial analysis were female age, duration of infertility, type and diagnosis of infertility, number of pre-ovulatory follicles (>or=16 mm, >or=18 mm follicles), thickness of the endometrium, number of the treatment cycles, result of semen analysis, ovarian stimulation protocol, number of IUI. A logistic regression method was used to identify significant variables that contribute to the success of CC/hMG/IUI treatment. RESULTS: Logistic regression analysis revealed four predictive variables as regards pregnancy: duration of infertility (p=0.011), infertility etiology (p=0.049), number of IUI (p=0.004), method of ovarian stimulation (p=0.042). A cause of infertility, especially ovarian dysfunction other than tubal factor, a shorter duration of infertility (<6 years) and CC/hMG minimal ovarian stimulation protocol with double IUI resulted in better treatment success in CC/hMG with IUI cycles. CONCLUSION: We concluded that careful patient selection criteria coupled with successful ovarian stimulation and increased sperm count in female reproductive tract is the model for CC/hMG/IUI sucess.

6.Retroperitoneal cystic lymphangioma in an aged man: report of a case and review of the literature.

Retroperitoneal cystic lymphangioma is a rare benign tumor which had previously been difficult to diagnose preoperatively by conventional radiographic technique(1). Recent reports describe the computed tomographic and ultrasonographic findings in cystic lymphangioma(1,2). We report a case of retroperitoneal cystic lymphangioma, accurately diagnosed by lymphography, with computed tomographic findings and percutaneous drainage findings. To the best of out knowledge, this is the most aged case of retroperitoneal cystic lymphangioma of preoperatively diagnosed.

7.Design of Collaborative Telemedicine Application System on Integrated Multimedia DooRae Framework.

We propose the various Telemedicine applications on the DooRae (Distributed Object Oriented Multimedia Application Crafting Environment for Collaboration) framework(or platform). DooRae-Telemedicine supports the application development of the collaborative multimedia environment connected with the network(LAN, MAN, WAN) in the CBM(Computer based Multimedia) based. Also, it is not restrict on space between a doctor(s) and a patient(s) into cyberspace made by network. The kinds of Telemedicine in this paper have different distinction, and the development of these application system is possible on DooRae environment. DooRae-Telemedicine has a interaction, various communication type, a question and an answer, multi-session, application sharing and whiteboard which resembles chalkboard. Also, various supplementary functions such as video conference and voice conference for DooRae-telemedicine are what makes face-to-face medicine effective. Also it supports real or non-real type. The operating system developed on windows 95 and windows NT This research has been done creating multimedia-based telemedicine system for home PCs in network environment. Most of telemedicine used hardware and exclusive line of high cost as CATV broadcasting technique. But DooRae-telemedicine will contribute to change the Home-PC for better use of collaborations among doctor and patients through various modes o interactions in cyberspace.

8.Laser assisted palatoplasty and phartngeal flap.

The purpose of this study was to determine the efficacy of the CO2-laser energy in the operation of cleft palate and pharyngeal fiap compared with the traditional scalpel surgery. This investigation involved a series of 34 patients who underwent CO2-laser assisted palatoplasty and pharyngeal flap between 1995 and 1997. The 19 males and 15 females ranged from 13 months to 36 years in age (mean age of 5.6 years) at the time of surgery. The postoperative clinical follow-up ranged from 6 to 24 months (mean 12 months). The CO??-laser provided the surgeon with bloodless operative field with reduction in blood loss giving greater visibility. It also offered an advantage in deep surgical cavity in Ro-pharynx through an operative arm, reaching areas that would otherwise have been difficult to access by traditional means. There was less bleeding during the surgery using, and therefore, complication. associated with bleeding was minimalized. There were no significant differences in their wound healing between the traditional scapel method and the laser method. The mean hospitalization in the group using the laser was approximately one day less than the group using the scapel. Preliminary findings suggest that the laser assisted palatoplasty and pharyngeal flap has a definitive advantage in providing the surgeon with bloodless operative field, reduction of blood loss, early recovery. Disadvantages of the laser surgery is a learning curve, the expenses of equipment, and safety precaution.

9.Outcomes in the Management of Spontaneous Cerebellar Hemorrhage.

OBJECTIVE: The operative indications on cerebellar hemorrhage have been controversial especially when the patient condition is grave. Therefore we investigated whether it can be justifiable if we perform the surgery in poor clinical grade. METHODS: Clinical records and computerized tomography(CT) films of the 89 patients, who were undergone hospital treatment due to spontaneous cerebellar hemorrhage between May 1997 and May 2004, were retrospectively researched. RESULTS: The study population consisted of 36 males and 53 female patients. The mean age was 65years (range 23~89). As a result of treatment, the patients, whose Glasgow coma scale(GCS) score were higher, showed better outcomes (p=0.001). 13 patients (14.6%) were below 5 in GCS score and 10 patients of these were operated. Among 10 patients, 4 patients (40%) showed good outcome and 5 patients (50%) had been dead. 3 patients (60%) of these dead patients had the findings of intraventricular hemorrhage, fourth ventricular obliteration and hydrocephalus in CT scan. CONCLUSION: This study suggests that operation may be justifiable in clinically poor grade patient with spontaneous intra cerebellar hemorrhage.

10.Clear Cell Meninioma: Case Report.

Clear cell meningioma is a recently recognized variant of meningiomas. We present a case of a 47-year old man with clear cell meningioma which involved right cerebellar hemisphere. Magnetic resonance imaging revealed a 3(34cm sized well enhanced round nodular lesion with mild mass effect. He underwent suboccipital craniectomy with a successful total removal of the tumor. To our knowledge, this is the first report of clear cell meningioma in our country.
