2.Studies on the Effects fo Topically Applied Ginseng Saponin on Human Skin: III. Evaluation of anti-inflammatory effect.

The therapeutic properties of Ginseng have been known as popular Chinese midecine since time immemorial. It has been used in Korea, China, Tibet, Indonesia and India for thousands of years under various presentations, such as tes, tinctures, wines, pills, and unguents for the prevention of aging, tireness, headaches, amnesia, tuberculosis, diabetes, illness of the liver, heart, kidneys, nervous system etc. The main effective component of Ginseng has recently been considered to be saponins according to the chemical studies by Shibata's group and the pharmacological studies by Takagi et al., and also through the studies of Brekhman. The purpose of this study is to evaluate the response of topically applied Ginseng total saponin against inflammation. To induce the inflammation of skin the backs of 20 volunteers were treated with 10% sodium lauryl sulfate solution under occlusion for one hour. After removal of sodium lauryl sulfate. Gineseng total saponin ointment in various concentrations(2.5, 5, 10%) is topically applied under occlusion for 48 hours. Erythema on the areas treated with Ginseng total saponin ointment was significantly reduced than the control area. It is suggested that Ginseng total saponin has an anti-inflammatory effect on the inflammation of skin induced by sodium lauryl sulfate.

5.Studies on filariasis in Korea - On the morphology and development of larvae of Brugia malayi in Aedes togoi.

Since Senoo and Lincicome (1951) first have brought up for attention to the existence of malayan filariasis in Korea, several reports on the epidemiological investigations of the disease had already been made by many workers. However it is little known what kind of mosquitoes are involved as the major vectors in main endemic areas. In Cheju-Do, known as one of main endemic areas in Korea, Aedes togoi is most likely suspected as an important vector because of their abundant collections and vigorous biting attack to human. As a part of studies on filariasis in Korea, an essential preliminary is to determiine whether this mosquito, Aedes togoi collected in the above areas is receptive to the microfilariae of B. malayi. Therefore, the present paper is concerned chiefly with the development of B. malayi in A. togoi. It is also hoped that the studies on the larval morphology in the mosquito host and the structure of microfilariae will provide the base line data required for later investigation of the different vector hosts. The studies were summarized as follows: The measurements of the fixed points in percentage of the body length of microfilariae from the Giemsa stained thick films were made, and they showed that cephalic space was 8 percent,cephalic space length to width, 1.3:1, nerve ring, 21.2 percent, excretory pore, 30.8 percent, excretory cell, 36.5 percent, R1 cell, 66. 5 percent, anus 80.4 percent and body length 202 micrometer(l81-228 micrometer) maximun width 7.6 micrometer. A study on the development of microfilaria malayi in the mosquito, Aedes togoi was carried out at room temperature (24-30 C). Mosquitoes used in this experiment were reared from larvae collected from the tide water rock pool in the coastal areas of Cheju-Do and they were fed with a blood meal of carrier donors whose microfilaria densities were in the range from 0.5 to 0.7 per cmm of blood. All of the microfilariae ingested by mosquito exsheathed in stomach, penetrated into the body cavity and then migrated into the thoracic muscles of the mosquitoes within 10 hours, after two moults in the mosquito host, the length of the developing 3rd stage larvae reached in size of 1.3-1.7 mm x 23-32 microns with anal ratio, 2.6 to 3.6. The first appearance of 3rd stage larvae in the mosquito host in this experiment was in 8th day after infection. The larvae were observed in the various cavities of mosquito, such as head, thoracic cavity, abdomen, halters, eye and legs. During the larval development in larval development in the host, the shortening of body length was first observed and then elnongation was followed until becoming 3rd stage larvae. Aedes togoi was proved to be the most suitable host for this species of microfilaria malayi in the above endemic areas.

6.Efficacy of Frozen Section Biopsy in the Diagnosis of Adnexal Neoplasms.

Recently, the possibility of inappropriate management of undiagnosed early cancers increased with spreading laparoscopic approach of adnexal tumors. The pathologic diagnosis of frozen section biopsy(FBx) guide the surgeon to perform the appropriate surgical procedure of adnexal tumors. This study was conducted to verify preliminarily the criteria for the laparoscopic management and determine the accuracy and efficacy of conditioned FBx according to the criteria. The inclusive criteria of FBx results in this study was designed by means of the preoperative transvaginal sonography(morphologic score >or=10) or tumor marker level of serum(CA-125 level>or=35 U/ml in postmenopause and >or=65 U/ml in postmenopause) or suspected gross findings of the malignancy during operation. On the other hand, cases which were convinced as dermoid cysts, endometriomas and advanced(>or=IIIb) ovarian cancers on the basis of gross findings were excluded in this study. Two hundred seventy-six operations of adnexal tumors were performed between January 1995 and June 1996 and 74 results of FBx were picked up in this study. The overall rate of conditioned FBx was 26.8%(74/276). The applied criterion in this study had the sensitivity of 0.97, specificity of 0.78, negative predictive value of 0.99, positive predictive value of 0.36. The accuracy of frozen sectional biopsy were 87.8% in the histopathologic diagnosis and 97.3% in the clinicopathologic diagnosis. The sensitivity, specificity, negative predictive value and positive predictive value for ovarian cancers(borderline+malignant) were 1 in all. It was thought that frozen sectional biopsy was a appropriate guide for the decision of methods during operation of adnexal tumors and that the criteria in this study could be used as a guideline for frozen section biopsy in the laparoscopic surgery of adnexal tumors.

7.Efficacy and Need of Sclerotherapy with Ethanol Injection on the Bleeding Peptic Ulcer.

The peptic ulcer is the most common cause of the upper gastrointestinal bleeding. We evaluated the efficacy of the sclerotherapy by endoscopic injection of ethanol for the bleeding peptic ulcer patients. We also evaluated the need of the sclerotherapy on the bleeding peptic ulcer according to the bleeding stigmata on the ulcer base. We classified the bleeding stigmata on the ulcer base with 4 group: active bleeding(21/345, 6%), visible vessel(122/345, 35%), red spot(97/345, 28%), no stigmata(105/345, 31%). The patients who manifested hernatemesis, melena, or both initially, were diagnosed as benign gastric ulcer(BGU, 179 patients) and duodenal ulcer(BDU, 166 patients). The median age was 57 year-old in the BGU patients and 37 year-old in the BDU patients. The sclerotherapy was performed to the 109 patients(58 BGU and 51 BDU patients), 96 patients(96/109, 89%) show permanent hemostasis. The hemostasis rate was 77% with active bleeding in the BGU patients, 75% with active bleeding in the BDU patients. There was no significant difference between sclerotherapy and observation group with visible vessel and red spot on the ulcer base of the bleeding peptic ulcer patients. Conclusively, the endoscopic sclerotherapy was very useful and effective treatment method for the bleeding peptic ulcer, but we suggested that all the bleeding peptic ulcer patients should not have done the sclerotherapy.

10.A Case of Cavernous Hemangioma of the Cervix and Vagina.

Hemangioams are usually present at birth of appeat shortly thercafter, as red or purple patches varying in size and most often in the skin. Hemangiomas include the cervix in their ubiquitous distribution; the cervix itself is very vascular and many reported hemangiomas are nothing more than a conspicuous demonstration of local casculatity. The cervical hemangioma is a rare condition which usually presents as vaginal bleeding of unusual cause, Many of the capillary hemangiomas and some of the cavermous types frequently resolve spontaneously. However, some hemagiomas ulcerate and a severe hemorrhage results. We recently encountered a case of cervical hemangioma involving the vagina in a 56-year-old woman. This case os especially interesting in that the histologic type was a cavernous hemangioma with uncommon cariety n the uterine cervix.
