2.Biological factors influencing the fate of onlay bone graft on the craniofacial skeleton.

The superior volume maintenance of membranous over endochondral bone grafts, which was shown in several studies has provided the basis for its preferred clinical use as an onlay grafting material on the craniofacial skeleton. The scientific rationale for this seeming embryological advantage, however, has never been proven, Since the cortical component of membranous bone is proportionally greater than that of endochondral bone, it follows that membranous grafts would show greater volume maintenance over time. Our hypothesis is that the pattern of onlay bone graft resorption is primarily determined by a graft's micro-architecture (relative cortical and cancellous composition) rather than its embryololgical origin(membranous versus endochondral). Fourty adult New Zealand white rabbits were used for this study. There were 8 animals in each of 4 groups. The rabbits of each group were sacrificed at 3, 8, and 16 weeks. Four types of grafts were placed subperosteally, onto each rabbit's cranium: a hydroxyapatite, a cortical bone graft of membranous origin, a cortical bone graft of endochondral origin and a cancellous bone graft of endochondral origin. Membranous bone grafts were obtained from the lateral mandible and endochondral bone grafts were obtained from the ileum. In order to determine post-sacrifice volume and density of the bone grafts, a caliper technique and bone densitometry(bone densitometer: LUNAR, DPX-L, U.S.A.) were performed on all of the bone grafts. Bone graft specimens were histologically examined at 3, 8, and 16 weeks.The measurement of volume and density show that there is a statistically greater resumption in the cancellous endochondral bone grafts for all parameter, compared to either the endochondral or membranous cortical bone grafts or hydroxyapatite at all time points(p< 0.05). In addition, there is no significant difference in the resorption rates between the endochondral and membranous cortical bone grafts for all parameters at all time points. By placing cortical bone grafts and cancellous bone grafts on the recipient sites separately, we have shown that the former grafts maintain their volumes, widths and projections significantly better than the latter grafts. Futhermore, we found no statistical difference in resorption rates between the two cortical bone grafts of different embryologic origins, a finding which has never been previously shown. Bone volume fraction, measured with bone densitometry, was shown to be higher in cortical bone than in cancellous bone at all time points, further illustrating the differences between cortical and cancellous bone.From our results, we believe cortical bone to be a superior onlay-graftiong material, independent of its embryololgic origin.

3.Spotted Grouped Pigmented Nevi: A case report.

The spotted grouped pigmented nevi is one of special forms of non-giant congenital melanocytic nevi. Histologically the nevus cells tend to proliferate around the skin appendages. We have experienced a case of spotted ground pigmented nevi in a 11 year old girl who had a pigmented plaque consisting of grouped brown black regularly distributed papules on the left inguinal region since birth. Histology revealed intradermal nevus. The skin appendages, especially the hair follicles and eccrine sweet ducts were enveloped by the neuvs cells. Electron microscopic study revealed that this nevus cells were mostly of type B cells. We would like to consider that the spotted grouped nevi are closely related pathogenically with the skin appendages.
4.Radiological diagnosis of perinephric abscess

5.Pycnodysostosis: A case report

Maroteaux and Lamy coined the term Pycnodysostosis derived from the Greek, Pycnos (thick or dense), Dys (defective), and Ostosis (bone). The principal characters of this syndrome are short stature, dysplasis of the skull, obtuse manibular angle, dysplastic clavicles,partial or total aplasia of the terminal phalanges, and generalized increased roentgenographic density of the skeleton. Patient so affected are predisposed to fracture. Pycnodysostosis seems to be a genetic disorder, inherited as an autosomal recessive trait. Here, it is reported a case of incidental finding of pycondysostosis with chronic renal failure in 12 years old girl.

6.A case of Neuroblasoma with multiple skeletal metastasis

Neuroblastoma is a malignant tumor of sympathetic nerve origin. They are derived from sympathetic formative cells, the primitive neuroblasts that have failed to mature. They may be located wherever sympathetic nerve tissue is found, The majority of cases occured in the suprarenal medulla, posterior mediastinum, and orbit. The tumor metastasized early by the hematogenous or lympathic route, or both to skeletal system, liver, and skin, etc. Since the vast majority of children with neuroblastoma have local or distant metastases at the time of original diagnosis, the clinical manifestation and the prognosis are variable. The age distribution is from birth to 19 years old. Many methods of treatment of neuroblastoma have been advocated such as surgical excision,roentgen thrapy, and its combipation therapy but the effect is questionable. Here, it is reported a case of neuroblastoma with systemic skeletal metastasis originated from posterior mediastinum in 8 years old girl.

7.Clinical Study of Meniscus Tear

It is assumed that the frequency of the meniscus tear on the right and the left knee and the mechnism of injuries in westerners are different from those in easterners. The writer had experienced 32 cases of the meniscus tear in National Police Hospital, during the period from 1973 to 1977, and inquired into its frequency and cause based on appropriate references. This is the report on the result of our study. 1. Out of 32 cases, 21 cases were found in the group of 20 to 29 years of age. 2. Eleven years of age with a discoid type was noticed as the youngest. 3. Out of 32 cases, 20 cases were found on the lateral meniscus and 12 on the medial, showing every five lateral cases to every three medial cases. 4. Out of 32 cases, 17 cases were found on the right knee joint with 14 lateral and 15 on the left knee joint with 9 medial. 5. Out of 32 cases, 27 cases were found on the male and 5 on the female, showing 5. 4 male cases to every one female case. 6. Out of 32 cases, 29 cases had the apparent history of trauma. 7. Transverse tear was found more frequent on the lateral meniscus and longitudinal or bucket handle tear was found more frequent on the medial meniscus.
