1.Clinical Effect of Rosoxacin on Acute Gonococcal Urethritis.

Fourty gonococcal urethritis patients were treated with Rosoxacin at the Department of Urology, National Police Hospital from December, 1982 to April, 1983. All patients were male and confirmed as acute gonococcal urethritis by Gram stain and culture. A new bactericidal agent, Rosoxacin, was administered orally in a single dose of 300mg in 40 cases of acute gonococcal urethritis and following results were obtained. 1. Gram negative intracellular diplococci were controlled in 30 patients (100%) among 30 patients with pure gonococcal infection with a single oral dose of Rosoxacin. 2. 7 patients (23.3%) of 30 patients with pure gonococcal infection were revealed to nonspecific urethritis, and they were treated with other antibiotics. 3. Gram negative intracellular diplococci were controlled in 10 patients (100%) among 10 mixed infected patients with a single dose of Rosoxacin. 4. 8 patients (80%) of 10 patients with mixed infection were revealed to nonspecific urethritis, and they were treated with other antibiotics. 5. There was no side effect of Rosoxacin in all patients.

2.A Clinical Observation on 85 Cases of Hydronephrosis.

A Clinical observation was made on 85 cases of hydronephrosis who were admitted to the department of urology in National Police Hospital during 5 years and 4 months from Jan., 1978 to April, 1983. The following results were obtained; 1. The rate of hydronephrosis accounted for 8.6% of the total patients (981 cases) admitted to the urologic department. 2. Age distribution in this series ranged from a 4 years old male to a 69 years old male patient. The most common age group was the 3rd decade (29.4%). The male and female ratio was 2.7:1. 3. The left side was affected 1.4 times more than the right. 9 cases (10.9%) were bilateral. 4. In underlying diseases, the congenital causes were 11 cases (12.9%) and among them congenital ureteropelvic stenosis was the most Common (5 cases). The acquired causes were 74 cases (87.1%) and ureteral stone was the most common acquired cause (39 cases). 5. In the level of the obstruction, the upper tract was 76 cases (89.4%), mid & lower tract were 7 cases (8.2%) and I case was extraurinary tract disease. 6. The most common chief complaints on admission was flank pain (52 cases). 7. In laboratory findings, increased BUN over 20mg% were 8 cases (9.4%) and the increased creatinine over 2.0rag% were 4 cases (4.7%). In urinalysis, pyuria were noticed in 41 cases (48.2%) and among them urine culture were performed to 32 cases. And E. coli were isolated in 12 cases (37.5%). 8. In treatment conservative treatment was done in 18 cases(20%) and surgical treatment was performed in 67 cases (80%). Of the surgical treatment, the most common operation was the ureterolithotomy in 32 cases (37.6%) and nephrectomy was done in 14 cases (16.4%).

3.A Case of Paratesticular Embryonal Rhabdomyosarcoma.

Rhabdomyosarcoma is the most frequent cancer involving the bladder, prostate, vagina and paratesticular tissues in children, and is highly malignant neoplasm with a tendency toward early infiltration of adjacent structures and metastases to regional lymph nodes and distant organs. We report a case of paratesticular embryonal rhabdomyosarcoma developed in 20-year-old male with brief review of literatures.

4.A Case Report of Priapism Associated with Leukemia.

5.Therapeutic Effect of Selective Nerve Root Injection for Sciatica in the Lumbosacral Radiculopathy.

The purposes of this study were to verify the effect of selective nerve root injection for sciatica caused by lumbosacral radiculopathy and to compare the difference of the therapeutic effects with the various clinical parameters and the morphologic types of herniated intervertebral disc on MRI finding. Forty-two patients, 14 males and 28 females, with an average age of 51 years (range, 26~71) and an average duration of symptoms of 17.8 months (range, 1~120) presenting with sciatica thought to be due to lumbosacral nerve root compromise were admitted to the study. All of the patients underwent clinical examination and lumbosacral MRI. Epiradicular infiltration of corticosteroids and local anesthetics by the transforaminal route under the image intensifier was done as a treatment. All the patients were followed up at 2 weeks and 3 months after the injection. A high proportion of patients made a satisfactory relief of sciatica with a selective nerve root injection at 2 weeks (85.7%) and 3 months (76.2%) follow-up. Among the patients who showed a significant symptom relief at 3 months, 12 patients were followed up and the relieved symptoms lasted for 6 months to 1 year in majority of those patients. In a few patients with clinical improvements at 2 weeks after the injection, the symptoms recurred at 3 months (9/36, 25%) and between 6 months to 1 year (3/12, 25%). There were no differences of the therapeutic effects according to the duration of the disease, neurologic findings on physical examination and the MRI findings of herniated intervertebral discs. The selective nerve root injection can be an effective therapeutic modality for the treatment of sciatica in lumbosacral radiculopathy.

6.Titration of Phenol Nerve Block Effects on Peripheral Nerves.

Chemical neurolysis can be an extremely effective intervention for reducing spasticity. Phenol nerve block as a method of chemical neurolysis has been used over 40 years, nevertheless, many clinicians remain reluctant to perform this procedure. This is largely due to the fear of its side effects and complications such as excessive muscle weakness or increased spasticity of antagonistic muscles. The purpose of this study was to titrate the nerve block effects of phenol with different concentrations and different volumes of the phenol solution. Left tibial nerves of forty eight adult rabbits were injected with phenol solution of different concentrations(5%, 4%, 3%) and volumes(0.3 ml, 0.2 ml, 0.1 ml) into the epineural sheath. Nerve conduction study of the gastrocnemius muscle was performed before and after the nerve blocks(1day, 1week, 2weeks, 4weeks, 8weeks). The proportion of compound muscle action potential(CMAP) amplitudes and areas before and after the nerve blocks was used for the evaluation of nerve block effect. There was a statistically significant difference in the proportion among three volumes of phenol solution(p<0.05). However there was no statistical difference in the proportion among three concentrations of phenol solution(p>0.05) although the higher concentration of phenol solution showed the tendency for smaller proportion. The area of histological degeneration appeared at 2 weeks following nerve block alongside the perineurium. The depth of degeneration area of nerve fascicle varied according to the distance from injection point. These data suggest that the nerve block effect of phenol can be titrated more easily with the volume of phenol solution rather than the concentration until 8 weeks after the nerve block. The variation of the extent of degeneration with different volumes of phenol solution seems to be the mechanism for the titration.

7.Comparison of IVF-ET Outcome after Various Therapeutic Approaches for Ovarian Endometriomas.

OBJECTIVE: To compare COH characteristics and IVF outcomes among IVF-ET patients who were treated with various therapeutic modalities for ovarian endometriomas and to propose effective pre-cyclic therapeutic modalities to improve IVF-ET outcomes in the patients with ovarian endometriomas. METHODS: All cases that had undergone IVF-ET after laparoscopy between January 1997 to August 2003 were reviewed. Forty-eight patients with tubal factor were assigned to Group I. Twenty seven, 22 and 38 patients diagnosed as severe pelvic adhesion with ovarian endometriomas by laparoscopy received only medical therapy (Group II), cyst aspiration (Group III), and sclerotherapy (Group IV), respectively. Laparoscopic cystectomy was performed in 20 patients (Group V). Resistance index was measured on day administering hCG. RESULTS: As compared with Group I, in Group II resistance index increased (p<0.05) but number of oocytes, good-quality oocyte ratio (mature and intermediate oocytes/total retrieval oocytes), fertilization rate, and embryo development rate decreased (p<0.05). In Group III fertilization rate and embryo development rate decreased (p<0.05). There was no difference between Group IV and Group I in all parameters except basal FSH which increased (p<0.05). In Group V basal FSH, and resistance increased (p<0.05) and number of oocytes and good-quality oocytes ratio decreased (p<0.05). CONCLUSION: Sclerotherapy is an effective therapeutic option which can be done prior to IVF-ET cycles in the patients with ovarian endometriomas. Further studies on a large scale are necessary to confirm these data.

8.A Case of Sparganosis Infesting in the Epididymis.

Sparganosis is a parasitic infestation caused by sparganum of several species of tapeworm related to Diphyllobothrium latum. We experienced a 28 year old man who was suffered from adult thumb-tip sized mass on the tail of right epididymis. From the mass during epididymectomy, whitish yellow colored sparganum measuring about 7cm in length, 0.3cm in width, 0.2cm in thickness could be extracted. So we reported this case with a brief review of the literatures.
