1.The Prevalence of Varicocele and Evaluation of External Genital Organs of Boys on 3rd grade in Middle School in Chonju City.

PURPOSE: The goal of this study was to estimate development of secondary sex characters and genitalia including testicular volume, penile length and girth, and prevalence of varicocele. In addition, we evalua ted the relationship between testicular volume and presence of varicocele. MATERIALS AND METHODS: A cross sectional study was conducted on 893 school boys between the ages of 14 to 15 years, 3rd grade middle school in Chonju city during the period september to October, 1999. The testicular volume was measured in the upright posture using Prader's orchidometer and penile length and girth were estimated with a ruler. Varicocele was evaluated by dubin's method. RESULTS: The testis volume (mean+/-SD) was 10.20+/-2.6ml on right and 9.93+/-2.6 on left and penile length (mean+/-SD) was 10.9+/-1.6cm and penile girth (mean+/-SD) was 8.0+/-0.8cm. The prevalence of varicocele was 38.2% (342/893) in left side, 16%(146/893) in right side. The difference in testicular volume between right and left testis was statistically significant in students with unilateral varicocele than the students without varicocele (0.78+/-1.06ml v/s, 0.54+/-1.01ml, p=0.004). CONCLUSIONS: In this study, we obtained mean testicular volume, penile length and girth and higher incidence rate of varicocele on adolescent school age group in Chonju area. The difference in testicular varicocele. The reduction of the testicular volume with varicocele correlated with the severity of grade of varicocele need following up to disclose the effect of the varicocele on the testicular growth and fertility potential.

3.Natural Course of Low Grade Squamous Intraepithelial Lesion.

OBJECTIVE: The purpose of this project was to suggest guidelines for management of Low grade Squamous Intraepithelial Lesion(LSIL) by evaluating natural course of LSIL of the uterine cervix. MATERIAL & METHODS: Among the women who visited Korea University Kuro Hospital from Jan. 1993 to Oct. 1998, One hundred fifty eight patients who were diagnosed as LSIL with colposcopy directed biopsy were followed up by colposcopy and/or cytology, HPV DNA test every 3 months. RESULT: In I58 patients who were diagnosed as LSIL, colposcopic examination confirmed progression to high grade intraepithelial lesion(HSIL) in 17(10.7%) patients, persistence of LSIL in 87(55%) patients, regression to normal in 54(34.2%) patients during the 3 year follow-up period. In prediction of LSIL subgroup, abnormal Pap test is 39%, 64%, 71% and abnormal HPV test is l6%, 29%, 65% in regression, persistent and progression group respectively. The shortest time of transition from minor lesion(LSIL) to high grade lesion was 12 months (the range, 12-51). CONCLUSION: Of the patients who were diagnosed as LSIL and monitored by colposcopy for 60 months, 34% had disease that regressed, 55% had persistent disease, and 11% had progressive disease. HPV DNA test(p=0.002) is more informative than pap test(p=0.567) in prediction of disease progression.

4.The Prediction Model of the Number of Industrial Injured Persons Using Data Mining.

OBJECTIVES: This study is to see the transition and pattern of the industrial iureal worker, and to develop the prediction model. METHODS: The data of the study are based on the samples from data-warehouse of Occupational Safety & Health Research Institute and are summed monthly from Jan 1986 to Dec 1999. This study data used data mart and Meta data from DW in KOSHA. The prediction model of the injured worker in Industry is designed by using a winters time series method after data preparing (i. e. sample, explore, modify) from DW. RESULTS: Thls predicted model obtained Winters-method multiplicative in exponential smoothing among applied all models, after the tlme series (total 163 months). It showed that the prediction power was 95.5 %. CONCLUSIONS: In the process of exploring the data, totally the rate of industrial injureal workers reduced, and in the yearly circulation, in February and September the number is the lowest but in June, July, October and November the higher. The number of monthly average injureal workers is 8709 (95 % confidence interval 8277, 9140). From the developed prediction model, since Aug 1999 the industrial injureal worker reduced rapidly in Dec 1999 and first period of 2000. But In second period of 2000 the number of the injured workers is increasing. To conclude, as the total economic situation is becoming better in 2000 than In 1999, its is supposed that the injured workers will increase more than the predictive injured workers because of the increase of production rate and labor force.

5.Morphometric Study of the Pedicle of Lumbar and Selected Thoracic Vertebrae for Surgical Spinal Fixation

The pedicle instrumentation has become a popular way of spinal fixation. Placement of a screw through the pedicle into vertebral body appears to be a very successful way to accomplish spinal fixation. However, the configuration of the pedicle morphometry must be understood. The measurement includes pedicle width, angle of pedicle axis to the transverse plane, ideal screw length, ideal screw entry point and ideal angle. This study was accomplished using computerized axial tomogram(CT) of 704 vertebrae(T10-L5). The results were as follows. 1. Transverse pedicle diameter were narrowest at T10, widest at L5. 2. The pedicle axis is oriented anteromedially at all levels except T11 and T12, then increase from L1 to L5. 3. Screw lengths are fairly constant between all levels, thus the range of screw lengths need is limited. 4. The incidence of pedicle less than 6 mm in the transverse diameter is most common at T10 and followed by levels L1, T12 and L2. Preoperative determination of transpedicular screw diameter and length can be made, by direct measurement from the patient's CT scan.

6.Cementless Total Hip Arthroplasty Using Hydroxyapatite-Coated Femoral stem.

PURPOSE: Evaluation of clinical and radiologic results of THR using HA-coated femoral stem. MATERIALS AND METHODS: From Jan. 1991 to Dec. 1992, we carried out 177 cases of Total Hip Arthroplasty using hydroxyapatite-coated implants of 167 patients, and among thses, 153 cases in 144 patients were followed up more than 24 months. The implants used were 74 cases of Mallory-Head Hip system, 41 cases of Omnifit system and 38 cases of Profile system. RESULTS: The average Harris Hip Score was 48.3 points preoperatively, 94.7 points at POD 1 year and 95.7 at POD 2 years. Seven patients(5.2%) complained thigh pain at POD 2 years. On the radiologic findings, cancellous condensation was noted around the hydroxyapatite coating region of femoral stem in 84 cases(54.2%), and radiolucent lines were noted at the non-coating region of distal stem in 102 cases(67.7%), but were less than 2mm and not progressive. There was no statistically significant differences among the groups of these three implants(P < 0.05). Comments : This early result of femoral stems with hydroxyapatite-coating was quite satisfactory, however, long-term follow-up studies will be necessary.

7.A Clinical and Serologic Study of 21 Cases of Tsutsugamushi Disease Confirmed by Serologic test.

Tsutsugamushi disease is an acute febrile disease caused by Rickettsia tsutsugamushi, and which has been reported with increasing frequency thorough the nation since 1986. We experienced 21 cases of Tsutsugamushi Disease diagnosed with serologic test occurring in Taegu city and Kyungpook province during October-November, 1989. The results of survey are as follow. 1) Of 21 cases, 12 (57%) were males and 9 (43%) were females, and the peak incidence was the 4th decade. 2) The outbreak was in October to November and the peak incidence was in October. 3) The most frequent symptoms were fever and chill (100%), myalgia (95%), headache (90%), Eschar and rash were observed in 18 patients (86%) and the eschar was detected in all over the body, especially thorax (33%) and lower extremity (22%). 4) Laboratory features were SGOT elevation (83%), SGPT elevation (61%), LDH elevation (67%), leukocytosis (38%). 5) Indirect immunofluorescent antibody test was done in 18 patients and the antibody titer was above 1:320 in all patients. 6) The chloramphenicol, tetracycline or doxycycline regimens were very effective and mean duration of defervescence from initiation of therapy was 1.3 days. 7) The complication such as meningitis or shock, was not seen.

8.Use of microplate on flxation of orbital rim fracture.

The orbitozygomatic area occupies a key anatomic position in midface, is prone to injury, and plays a prominent role in facial aesthetics. So the facial appearance including orbital shape can be altered by complications of orbitozygomatic fractures. Most possible initial complications include blindness, hyphema, retinal detatchment, and paralysis or entrapment of extraocular muscles. Long term sequelaes include infraorbital nerve dysfunction, loss of malar projection, enophthalmos, and dystopia. Accurate anatomic reduction and rigid fixation is essential for management of orbitozygomatic fractures to minimize those late sequelaes. Conventional fixation devices to fix displaced fracture of facial bone are interosseous wire and miniplate. But interosseous wirings are unstable for primary bone healing and time consuming. Miniplates have great deal in rigid fixation but their high profile and palpability are the main complaints in many patients, especially in orbital rim area. In this article, we reviewed the 30 cases of zygomamaxilla complex fractures with orbital rim fracture fixed with microplates, and discussed the stability of microplate and superiorities in final aesthetics result. The use of microplates in these area permits enough stability of fracture segment with ease of procedures and superiorities in final results without any palpability.
