1.Analysis of related factors affecting the short outcome of abdominal surgery among elderly patients with acute abdomen

Objective To investigate the related factors that affect the short outcome of abdominal surgery among elderly patients with acute abdomen.Methods Clinical data of 378 patients aged >60 years with acute abdo-men were retrospectively analyzed.The key factors under analysis in this study were the occurrence of major complica-tions,death from any cause within 30 days and related factors after the operation.Results The most common diagno-ses were acute appendicitis (134 cases,35.4%),biliary tract disease (96 cases,25.4%),intestinal obstruction (68 cases,20.0%)and inguinal hernia(42 cases,11.1%).The majority of patients(327cases,86.5%)underwent open surgery.12cases(23.5%)of all 51 cases underwent laparoscopic surgery were converted to open surgery.The 30 day mortality rate and rate of operation -related complications were 9.5% and 22.0%,respectively.Analysis showed that patients'age,low body mass index,open surgery and ASA grade Ⅲ or above may increase mortality(all P <0.05).Conclusion The acute abdomen in elderly patients was with much complications,operation treatment is still the first choice.The patient's older age,low body mass index,open surgery may increase the mortality rate.ASA grade higher may predict a bad prognosis.The operation of elderly patiens had a high morbidity and mortality,as general surgeon,we should pay more attention to acute abdomen in elderly patients.
2.Analysis on comprehensive noise control effects in an automobile engine manufacturing enterprise

ObjectiveTo analyze the measures and effects of comprehensive noise control in an automobile engine manufacturing enterprise. Methods An automobile engine manufacturing enterprise with noise exposure work-sites was selected as the research subject. The noise hazard status of the enterprise was investigated. The noise hazard exceeded the national standard in work-sites was comprehensively treated. The treatment effect was analyzed. Results A total of 395 work-sites exposed to noise were detected. Among them, 16 work-sites exceeded the national noise standard, with the rate of 4.1%. The work-sites with noise exceeding the national standard were mainly in the machine workshop and the casting workshop, with normalization of equivalent continuous A-weighted sound pressure level to a nominal eight hours working day (LEX,8h) ranging from 86.8 to 89.2 and 85.2 to 90.2 dB(A), respectively. In addition, the classification of occupational noise exposure of the mold protection group in the casting workshop was level Ⅱ, and classification of occupational noise exposure of other work-sites with excessive national standard was grade Ⅰ. After the enterprise implemented noise control measures such as process reform, equipment replacement, sound insulation, noise reduction, sound absorption, and vibration isolation, the LEX,8h of nine positions were lower than those before rectification [(86.7±1.1) vs (83.3±1.3) dB(A), P<0.01]. Conclusion The use of comprehensive control measures can achieve the purpose of reducing noise from the sources, and can be used for reference by relevant enterprises with noise hazards.
3.Laparoscopic splenectomy combined with pericardial devascularization in the treatment of cirrhosis-induced portal hypertension

ObjectiveTo investigate the safety and application value of laparoscopic splenectomy combined with pericardial devascularization (devascularization) in the treatment of cirrhosis-induced portal hypertension.MethodsClinical data of 32 patients with cirrhosis and portal hypertension undergoing laparoscopic devascularization in Lingnan Hospital, the Third Afifliated Hospital of Sun Yat-sen University from November 2011 to November 2013 were retrospectively studied. Among the 32 patients, 26 were males and 6 were females with the age ranging from 40 to 69 years old and the median of 53 years old. The informed consents of all patients were obtained and the local ethical committee approval had been received. The patients received laparoscopic devascularization under intratracheal intubation anesthesia combined with intravenous anesthesia. One week before surgery, the patients were instructed to quit smoking and drinking, and received respiratory function training and in-bed toilet training. Vital signs were closely monitored and drainage volume was observed after surgery.ResultsAll patients received larparoscopic devascularization successfully, with the average operation time of (110±15) min, intraoperative blood loss of (200±52) ml and length of hospital stay of (7±2) d. The blood coagulation and color Doppler ultrasound of portal vein were regularly followed up within 3 months after surgery. Seven cases suffered portal vein thrombosis after surgery and recovered after adjusting the anti-platelet and anti-coagulation therapy according to the situation of platelets and thrombus. No death, massive hemorrhage, digestive tract ifstula and infection occurred during perioperative period.ConclusionsLaparoscopic devascularization in the treatment of cirrhosis-induced portal hypertension is safe and effective. The rigorous perioperative management guarantees the safety of surgery.
4.Long noncoding RNA PTENP1 inhibits proliferation and migration of hepatocellular carcinoma cells

Objective To investigate the effect and mechanism of long noncoding RNA (lncRNA) PTEN pseudogene 1 (PTENP1) on the proliferation and migration of hepatocellular carcinoma (HCC) cells. Methods Lentiviral vectors expressing PTENP1 were constructed. HCC cells BEL-7404 were infected with LV003-GFP-PTENP1 and control vectors LV003-GFP. BEL-7404 cells stably expressing PTENP1 were constructed and the experimental and control groups were established. The proliferation and clone formation abilities of HCC cells in two groups were detected by CCK-8 assay and clonogenic assay. The migration ability of HCC cells was detected by wound healing assay. The expression of p44/42 mitogen-activated protein kinase (MAPK) and p38 MAPK proteins were detected by Western blot. Results The absorbance values A450 of the cells at 48 and 72 h in the experimental group were 1.4±0.3 and 2.3±1.1, signiifcantly lower compared with 3.2±1.7 and 3.4±1.1 in the control group (t=-5.78,-4.23;P<0.05). The number of cell clone formation in the experimental group was 55±12, signiifcantly less than 154±45 in the control group (t=-3.98, P<0.05). The percentage of cell migration in the experimental group was (21.7±2.6)%, signiifcantly lower than (57.7±4.9)%in the control group (t=-8.34, P<0.05). Western blot revealed that the expression of p44/42 MAPK and p38 MAPK proteins in the experimental group was significantly down-regulated compared with those in the control group. Conclusion lncRNA PTENP1 can inhibit the proliferation and migration of HCC cells probably through regulating MAPK signaling pathway.
