1.Determination of mequindox and its metabolite in pork luncheon meat by ultra-performance liquid chromatography/triple qudrupole tandem mass spectrometry

Objective To establish a reliable pretreatment method for the detection of mequindox and its metabolite in pork luncheon meat by ultra-performance liquid chromatography/triple qudrupole tandem mass spectrometry. Methods Samples were extracted with ethyl acetate, and the results of purification and enrichment by PAX and PEP solid-phase extraction columns were analyzed. Acetonitrile/methanol (3:11) - 0.1% formic acid water was used as the mobile phase, and Shimadzu Inertsil ODS-3-column (3µm, 2.1 × 100mm) chromatographic columns were used for qualitative and quantitative analysis using the multi-reaction detection positive ion mode. Results The results showed that PEP cartridge had good recovery rate. The detection limit of mequindox was 0.10µg/kg, and limit of quantitation was 0.30µg/kg. The average recoveries for spiked levels of 0.33, 0.83, and 1.65µg/kg were 127%, 72.0%, and 60.1%, respectively. The detection limit of 2-quinoxalinecarboxylic acid was 0.10µg/kg, and limit of quantitation was 0.40µg/kg. The average recoveries for spiked levels of 0.42, 1.05, and 2.1µg/kg were 125%, 99.0%, and 60.9%, respectively. Conclusion This method is suitable for the determination of mequindox and its metabolite 2-quinoxalinecarboxylic acid in luncheon meat.
2.Zymography identification of the matrix metalloproteinases synthesized by cultured hepatic stellate cells.

AIMTo establish a method for identification of the activities of matrix metalloproteinases (MMPs) synthesized by cultured hepatic stellate cells (HSC).
METHODSSDS-gelatin polyacrylamide gels electrophoresis was used for the identification of MMPs synthesized by cultured HSC.
RESULTSMMP-2 and MMP-9 in culture medium and HSC were sensitively identified, and MMP-2 activity was higher than MMP-9 in culture medium, the changes of MMPs in culture medium and HSC were also observed after 24h medicine treatment.
CONCLUSIONZymography is suitable for the studies on the role of HSC in the regulation of extracellular matrix turnover.

3.Effect of hypertensive disorders of pregnancy on peripheral venous blood cell count in preterm infants with a gestational age of 28-34 weeks.

OBJECTIVES:
To study the effect of hypertensive disorders of pregnancy on peripheral venous blood cell count in preterm infants with a gestational age of 28-34 weeks.
METHODS:
A total of 227 preterm infants with a gestational age of 28-34 weeks who were admitted to the Department of Pediatrics, the First Hospital Affiliated to Kunming Medical University, from January to December 2020, and whose mothers had hypertensive disorders of pregnancy were enrolled as the study group. A total of 227 preterm infants with a gestational age of 28-34 weeks who were admitted during the same period and whose mothers did not have hypertensive disorders of pregnancy were enrolled as the control group. According to maternal blood pressure during pregnancy, the study group was divided into three subgroups: gestational hypertension (n=75), mild preeclampsia (n=81), and severe preeclampsia (n=71). According to the birth weight of the preterm infants, the study group was divided into two subgroups: small for gestational age (SGA) (n=113) and appropriate for gestational age (AGA) (n=114). Peripheral blood cell count on day 1 after birth was compared between the study and control groups, as well as between the subgroups of the study group.
RESULTS:
Compared with the control group, the study group had significantly lower white blood cell count, absolute neutrophil count, and blood platelet count (P<0.05) and significantly higher incidence rates of leucopenia and neutropenia (P<0.05). The subgroup analysis showed that the mild preeclampsia and severe preeclampsia subgroups had significantly lower white blood cell count, absolute neutrophil count, and blood platelet count than the gestational hypertension subgroup (P<0.05), and that the SGA subgroup had significantly lower white blood cell count, absolute neutrophil count, and blood platelet count than the AGA subgroup (P<0.05).
CONCLUSIONS
Hypertensive disorders of pregnancy can affect the peripheral venous blood cell count of preterm infants, which is more significant in infants with maternal preeclampsia and SGA infants.

4.Observation of Therapeutic Efficacy of Oxcarbazepine Combined with Levetiracetam in the Treatment of Cognitive Dysfunction in Adult Patients with Temporal Lobe Epilepsy

OBJECTIVE:To observe therapuetic effica cy of oxcarbazepine combined with levetiracetam in the treatment of cognitive dysfunction in adult patients with temporal lobe epilepsy. METHODS :According to inclusion and exclusion criteria ,83 adult patients with temporal lobe epilepsy were collected from neurology department of the First Affiliated Hospital of Kunming Medical University during January 2018 to October 2019,and then divided into control group (39 cases)and observation group (44 cases). Control group was given Oxcarbazepine tablet with initial dose of 8-10 mg/(kg·d),morning and night ,increased by 5-10 mg/(kg·d)each week according to patient ’s condition ,the maximal dose was 45 mg/(kg·d),lasting for 3 months with the lowest effective treatment dose. Based on treatment of control group ,observation group was additionally given Levetiracetam tablet with initial dose of 5-10 mg/(kg·d),morning and night ,increased by 5-10 mg/(kg·d)each week according to the severity of disease,the total dose was not more than 2 000 mg/d,lasting for 3 months with the lowest effective treatment dose. The event-related potential P 300 at Pz site in 2 groups was measured before and after treatment (including the incubation period of N 1, P2,N2 and P 3 waves,amplitude of P 3 wave). Webster ’s Adult Intelligence Scale [WAIS-RC ,including verbal IQ (VIQ), performance IQ (PIQ),full scale IQ (FIQ)scores] and Clinical Memory Scale [CMS ,converting to memory quotient (MQ) score] were adopted. The occurrence of ADR was recorded. RESULTS :Totally 5 patients of control group and 7 patients of observation group fell off. Finally ,a total of 71 patients participated in the entire study (34 cases in control group and 37 cases in observation group ). Before treatment ,there was no statistical significance in P 300 at Pz site ,WAIS-RC and CMS between 2 groups (P>0.05). Compared with before treatment ,incubation period of N 2 and P 3 waves was shortenedsignificantly in 2 groups,and amplitude of P 3 wave wasincreased significantly after treatment (P<0.05);VIQ,FIQ and MQ scores were increased significantly (P<0.05). The incubation pe riod of P 2 wave was shortened significantly,and PIQ score was increased significantly in observation group (P< 0.05). Compared with control group ,the shortening degree of incubation period of P 2,P3 waves,rising degree of amplitude of P 3 wave and rising degree of VIQ ,PIQ,FIQ,MQ scores were more bigger after treatment in observation (P<0.05). In the control group,there was 1 case of slight rash ,1 case of conscious asthenia ;in the observation group ,there was 1 case of nausea and vomiting,1 case of conscious asthenia ,and 1 case of emotional instability ;no serious ADR was found in 2 groups. CONCLUSIONS:Oxcarbazepine combined with levetiracetam can improve cognitive dysfunction in adult patients with temporal lobe epilepsy ,and the treatment effect is significantly better than oxcarbazepine monotherapy.
5.Analysis on the Publication of Revision Notices of Drug Instructions and Related Status in China in 2015-2019

OBJECTIVE: To analyze the situation of drug instruction revision and promote the standardization. METHODS: By reviewing the laws and regulations on the management of drug instructions in China, inquiring the official documents of drug instruction revision during Jan. 2015 to May 2019, the types of drugs involved and the revised items were analyzed, and the contents and shortcomings of the drug instructions and their revision process were summarized. RESULTS: A total of 112 revision notices were issued in 5 years (20, 28, 19, 34, 11 revision notices in 2015-2019, respectively), involving 270 varieties. There were 109 prescription drugs, 161 OTC, 158 Chinese medicines, 105 chemicals, and 2 biological products. The revised items mainly focus on safety information such as precautions, adverse reactions, contraindications, and additional warnings, including 100, 85, 103, 52 items. However, in combination with clinical practice, the author found that there were still some problems, such as non-standard writting drug instructions, lack of supervision on the implementation of the revision, and no notification to the relevant users after the revision, which seriously threatened the safety of drug use. CONCLUSIONS: The included specifications of drug instructions are not well regulated, the implementation of the revised instructions is not doing well, the delineation of rights and obligations is confused, and the legal binding force is not strong. The standardization of drug instructions in China needs further improvement.
6. Research progress on effective compounds group of Chinese materia medica

Effective compounds group of Chinese materia medica (CMM) is a pharmacological active compounds group containing all components that are closely related to their clinical application. It plays a core role in CMM formula. We think it maintains not only the characteristics of the whole system and the system integrity of CMM formula, but also their chemical compositions are clear. However, due to the complexity of CMM, the successful research cases are very few. In this paper, we summarized several outstanding research cases of effective compounds group in recent years and there are three strategies, including "research on decomposed-combined recipes of CMM based on assessment of chemical components in CMM way", "screening the effective components group based on knock-in and knock-out from chromatographic fingerprint", and "screening the effective components group based on serum pharmacochemistry". We also summarized the ways to illustrate properties of effective compounds group, such as combination of multiple component, clear chemical composition, and system integrity. This paper provided ideas for the future research of effective components of CMM.
7.Pneumonia, Multiple Pulmonary Infarction and Abscess Caused by a Bamboo Stick Accidentally Piercing into Chest: a Case Misdiagnosed as Pulmonary Tuberculosis.

Computed tomography (CT) examination is the major measure for detecting and diagnosis of foreign bodies in human body. Although CT has high sensitivity in diagnosis of foreign body, some interference factors may still lead to missed diagnosis or misdiagnosis. Here we report a rare case that a bamboo stick accidentally pierced into the left chest of a young man who was drunk and unware of this hurt. The patient experienced cough, chest pain, fever, hemoptysis, and was misdiagnosed as primary and secondary tuberculosis based on chest CT examinations at a local hospital, although no tubercular bacillus detected by sputum smear. He subsequently received anti-tuberculous treatments in the following three years, but no improvement of his symptoms was observed. Until one month before his death, the bamboo stick was detected by spiral CT examination as well as three-dimensional image reconstruction at another hospital. Postmortem examination revealed pneumonia, pulmonary infarction, and abscess as the causes of his death. We analyze the potential reasons of misdiagnosis in this case, aiming to provide reference for the diagnosis and treatment of pulmonary inflammation associated with foreign body in the future.

8.The Functional Role of SUMOylation in The Tumor Microenvironment

Tumors continue to be a major challenge in human survival that we have yet to overcome. Despite the variety of treatment options available, we have not yet found an effective method. As more and more research is conducted, attention has been turned to a new field for tumor treatment—the tumor microenvironment (TME). This is a dynamic and complex environment consisting of various matrix cells surrounding cancer cells, including surrounding immune cells, blood vessels, extracellular matrix, fibroblasts, bone marrow-derived inflammatory cells, signaling molecules, and some specific cell types. Firstly, endothelial cells play a key role in tumor development and the immune system’s protection of tumor cells. Secondly, immune cells, such as macrophages, Treg cells, Th17 cells, are widely involved in various immune responses and activities in the human body, such as inflammation responses promoting survival carefully orchestrated by the tumor. Even though many studies have extensively researched the TME and found many research schemes, so far, no key effective method has been found to treat tumors by affecting the TME. The TME is a key interaction area between the host immune system and the tumor. Cells within the TME influence each other and interact with cancer cells to affect cancer cell invasion, tumor growth, and metastasis. This is a new direction for cancer treatment. In the complex environment of the TME, post-translational modifications (PTMs) of proteins have been proven to play an important role in the TME. PTMs are dynamic, strictly regulated changes to proteins that control their function by regulating their structure, spatial location, and interaction. Among PTMs, a reversible post-translational modification called SUMOylation is a common regulatory mechanism in cellular processes. It is a post-translational modification that targets lysine residues with a small ubiquitin-like modifier (SUMO) in a reversible post-translational modification manner. SUMOylation is widely involved in carcinogenesis, DNA damage response, cancer cell proliferation, metastasis, and apoptosis, playing a pivotal role in the TME, such as DNA damage repair, tumor metastasis, and also participates in immune cell differentiation, activation, and inhibition of immune cells. On the other hand, SUMO or sentrin-specific protease (SENP) inhibitors can interfere with the SUMOylation process, thereby affecting many biological processes, including immune response, carcinogenesis, cell cycle progression, and cell apoptosis, etc. In summary, this review aims to introduce the dynamic modification of protein SUMOylation on various immune cells and the application of various inhibitors, thereby exploring its role in the TME. This is a challenging but hopeful field, and we look forward to future research that can bring more breakthroughs. In conclusion, the TME is a complex and dynamic environment that plays a crucial role in the development and progression of tumors. Understanding the intricate interactions within the TME and the role of PTMs, particularly SUMOylation, could provide valuable insights into the mechanisms of tumor development and potentially lead to the development of novel therapeutic strategies. The study of SUMOylation and its effects on various immune cells in the TME is an exciting and promising area of research that could significantly advance our understanding of tumor biology and potentially lead to the development of more effective treatments for cancer. This is a challenging but hopeful field, and we look forward to future research that can bring more breakthroughs.
9.Predictive value of combination of MRI tumor regression grade and apparent diffusion coefficient for pathological complete remission after neoadjuvant treatment of locally advanced rectal cancer.

Objective: Pelvic high-resolution magnetic resonance imaging (MRI) has now become a standard method for evaluating the efficacy of neoadjuvant treatment for locally advanced rectal cancer (LARC). However, this traditional morphological qualitative assessment method based on T2-weighted imaging (T2WI) is not effective in predicting pathological complete remission (pCR). The purpose of this study is to investigate whether combining the magnetic resonance tumor regression grade (mrTRG) with apparent diffusion coefficient (ADC) can improve diagnostic value for pCR after preoperative neoadjuvant chemoradiotherapy (nCRT) of LARC. Methods: This was a diagnostic study. Clinicopathological data of 134 LARC patients who received nCRT and radical surgery in the First Affiliated Hospital of Kunming Medical University from January 2017 to December 2019 were retrospectively analyzed. All the patients underwent MRI which included T2WI and DWI sequences before and 8 weeks after nCRT. Two radiologists independently drew ROIs on T2WI and DWI to estimate mrTRG stage and calculate the mean ADC value. Receiver operating characteristics (ROC) method was applied to evaluate the predict value of mrTRG combined with mean ADC value for pCR. Results: Of 134 LARC patients, 85 were male and 49 were female with median age of 58 (28-82) years. After nCRT, MRI suggested 21 patients (15.7%) had clinical complete remission (cCR), e.g. mrTRG stage 1-2. Postoperative pathology revealed 31 (23.1%) patients had pCR. The evaluations of mrTRG and ADC value by the two readers were highly consistent, and the intra-group correlation coefficients were 0.83 (95% CI: 0.703-0.881) and 0.96 (95% CI: 0.989-0.996), respectively. There was a negative correlation between mrTRG and pCR (r(s)=-0.505, P<0.01), and a positive correlation between mean ADC value and pCR (r(s)=0.693, P<0.01). The ROC curve showed that mrTRG alone had a medium predictive value for pCR, with an area under the curve (AUC) of 0.832 (95% CI: 0.743-0.921); the mean ADC value had a higher predictive value for pCR, with AUC of 0.906 (95% CI: 0.869-0.962). The predictive value of the combined model of mrTRG and ADC value for pCR was significantly better than that of mrTRG alone (P=0.015), and the AUC was 0.908 (95% CI: 0.849-0.968). Conclusion: Both mrTRG and mean ADC value can be non-invasive methods to predict the efficacy of nCRT for LARC. Combining the mean ADC value with mrTRG can result in better pCR prediction.

Drug problem is a major social and public security problem in the world. Drug abuse poses a great threat to economic development, social stability and public health. In recent years, synthetic drugs represented by methamphetamine have surpassed traditional drugs such as morphine, heroin, ketamine and become one of the most abused drugs in the world. In order to solve the problem of drug abuse, it is of great theoretical value and practical significance to carry out all-round and multi-level scientific research on drug-related issues. Based on the current situation of drug abuse, this article reviews research progresses on the epidemiology of methamphetamine abuse, the monitoring technology, the basic researches on toxicity damage, the withdrawal drug screening, the related clinical comorbidity and the testing technologies, comprehensively presenting the development trend of methamphetamine abuse related issues.
