2.The Effects of Longterm Stretching Exercise on Health Related Fitness of the Elderly Women.

The purpose of this study was to investigate the influence of regular stretching exercise for 1 year on health related fitness of the elderly women. We also determined whether the weekly stretching exercise sessions followed by intensive stretching exercise program was beneficial to maintain the altered health related fitness status. The thirty elderly women (F, 80.1+/-6.6 yr) were participated in this study. Subjects had no serious orthopedic problems. During the study period, they did not perform any regular physical activity beside the stretching exercise. The 1 year stretching exercise programs included 3 days/week stretching exercise session for the first 12 weeks followed by 1 day/week stretching exercise for rest of the study period. The subjects were tested 6 categories of health related fitness factors which included % body fat, flexibility, hand grips, back strength, balance, and agility. The summary of the results of this study were as followed: 1) There were significant increase in FFM and slightly decrease in %BF and WHR after the 3 days/wk stretching exercise program. However, there were significant decrease in FFM and WHR after the 1 day/wk stretching exercise program (p<.05). 2) There were significant increase in flexibility, hand grip strength (Rt.) and balance after the 3 days/wk stretching exercise (p<.05) while hand grip strength (Lt.) and back strength were slightly increase after the exercise. However, there were significant decrease in hand grip strength (both) and reaction time after 1 day/wk stretching exercise program while flexibility, back strength, and balance were slighly decrease after the program. In conclusion, this study showed that the 3 days/wk stretching exercise for 12 weeks result in significant increase of all health related fitness factors (p<.05). However, it showed that some of the HRF factors were decreased after the weekly stretching exercise unexpectedly.

3.A Case of Malignant Fibrous Histiocytoma of the Left Atrium.

Primary malignant neoplasms of the heart are very rare disorders, which are found at less 1/1000 necropsies. These malignancies are almost exclusively sarcomas and histologically assume a wide veriety of morphological subtypes, angiosarcomas, rhabdomysarcomas, and fibrosarcomas(or MFH) being the most frequent. We experienced a case in which left atrial myxoma was suspected by 2-D echocardigraphy and the histologic diagnosis of promary MFH was confirmed by operation. A 45-year old woman with primary MFH arising from posterior wall of left atrium, interatrial septum and mitral annulus is presented with a brief view of the literatures. The patient was admitted to our hospital on March 1988, because of exertional dyspnea and intermittent palpitation resction of the masses. To date, more than thirteen months after surgery, the patient is alive and well in state of NYHA class II.

6.Clinical Observations of the Drug Induced Hepatitis during Antituberculosis Medication.

BACKGROUND: In Korea, the prevalence of tuberculosis and hepatitis is high, and combined therapy with rifampicin and pyrazinamide is used in tuberculosis, so drug induced hepatitis is not only problem of tuberculosis therapy but also cause of treatment failure. However most of recent reports on drug induced hepatitis during antituberculosis medication have dealt with its pathogenesis and have stressed the biochemical, and histopathological aspects of the disorder, whereas this study was designed primarily to provide information on the clinical features. METHOD: The subjects of study were 1414 patients treated with antituberculosis drugs on the department of chest medicine at National Medical Center during the 5-year 6-month period from January 1, 1988, to June 30, 1993. Retrospective analysis of clinical features for the 29 patients who developed drug induced hepatitis was done. RESULTS: 1) The incidence of antituberculosis drug induced hepatitis was 2.1%. 2) Male to female ratio of antituberculosis drug induced hepatitis was 2:1, but case rates among males and females were not significantly different. 3) Rates of drug induced hepatitis according to age distribution shows the most common incidence between 35 to 49 year old age group, but rates among groups of age were not significantly different. 4) Drug induced hepatitis was most common in the case of moderate advanced Pulmonary tuberculosis(rate is 2.78%), but rates among types of tuberculosis were not significantly different. 5) 18 cases(62%) of antituberculosis drug induced hepatitis patients had no signs or symptoms. In remaining cases, they were nausea, vomiting, jaundice, hepatomegaly, icteric sclera, right upper quadrant -tenderness in order 6) 22 cases(76%) of antituberculosis drug induced hepatitis cases had occurred within the first month. 7) The duration of abnormal liver function was 28±5(Mean±SD), ranged from 5 days to 180 days. 8) One case of antituberculosis drug induced hepatitis died. 9) The levels of abnormal GOT ranged from 64 to 1055U/L and GPT from 68 to 931U/L. CONCLUSION: There are no decided predisposing factors of antituberculosis drug induced hepatitis, so it should be done biochemical monitoring as week as close monitoring for overt signs or symptoms of hepatitis to avoid the development of irreversible hepatic reaction, especially at the treatment of the first month.

7.A Systematic Content Analysis of the Closed Nursing Malpractice Claims Related to Patients’ Safety

Purpose:
The study aimed to classify patient safety accident and nursing malpractice types through a content analysis of litigation cases in which nurses were found guilty.
Methods:
Data on malpractice cases were collected from the official website of the Korean judiciary. This analysis was conducted in stages according to the systematic content analysis method. A total of 46 cases consisting of 14 criminal cases and 32 civil cases were selected for analysis, and classified based on the types of safety accident and nursing malpractice.
Results:
Eleven categories and 14 subcategories of accidents were extracted from 71 significant statements. The 11 categories of accidents consisted of falls, medication, burns, treatment, diet, medical equipment, delivery, exams, surgery, infection, and suicide. Five categories and 14 subcategories of nursing malpractice were extracted from 71 significant statements. The five categories of malpractice were as follows: failure to take affirmative action to prevent accidents, monitor, follow guidelines, use equipment properly, and reports.
Conclusion
These findings highlight the importance of nurses’ independent roles as patients’ safety managers. It can be used as primary data to develop organizational and educational support for nurses seeking to perform their professional roles in ensuring patient safety.
8.Clinical Observation on the Mangement of Female Urethral Diverticulum.

PURPOSE: Urethral diverticulum in female can be easily detected by physical examination but sometimes the diagnosis can be difficult. Difficulty in prompt diagnosis and treatment arises from the constellation of nonspecific urinary symptoms. Furthermore, treatment may be unsuccessful and result in persistent or recurrent troublesome symptoms. We reviewed our experience with managing symptomatic diverticula for the prompt diagnosis and the definite cure. MATERIALS AND METHODS: We reviewed 33 cases of urethral diverticulectomy performed at our institution for the last 10 years. At presentation patient age ranged from 24 to 57 years (mean 39 years). Two women (6.0%) were nulliparious and average parity was 1.9 births (0-5). Investigations included voiding cystourethrogram, excretory urogram, cystourethroscopy, urodynamic studies and recently transvaginal ultrasound. Transvaginal diverticulectomy was performed using a 3-layer closure (urethral wall, periurethral fascia, vaginal wall) in all women. Concomitant bladder neck suspension was performed in 1 woman. RESULTS: Voiding cystourethrography adquately demonstrated the diverticulum in 11 of the 14 women (78.6%). Transvaginal ultrasound was performed in 21 women and identified the diverticulum in 19 women (90.5%). Of 33 women 32 were cured (97.0%) and secondary transvaginal diverticulectomy was performed for incomplete excision of diverticulumin in 1 woman. According to McGuire's classification 13 cases were classified as pseudodiverticulum which is a mucosal herniation through a periurethral fascial defect, and the rest 20 cases as true diverticulum. This classification had no influence on the surgical success rate. Complications of diverticulectomy included 2 cases of de novo stress urinary incontinence and 1 urethrovaginal fistula. CONCLUSIONS: Preoperative radiographic imaging helps to delineate diverticular anatomy and transvaginal ultrasound is useful test because of absence of ionizing radiation, reduced risk of infection, less invasiveness and higher detection rate than voiding cystourethrogram. With meticulous excision and 3-layer repair, definitive cure can be achieved with a single operation.

9.Genome-Wide Identification and Classification of MicroRNAs Derived from Repetitive Elements.

MicroRNAs (miRNAs) are known for their role in mRNA silencing via interference pathways. Repetitive elements (REs) share several characteristics with endogenous precursor miRNAs. In this study, 406 previously identified and 1,494 novel RE-derived miRNAs were sorted from the GENCODE v.19 database using the RepeatMasker program. They were divided into six major types, based on their genomic structure. More novel RE-derived miRNAs were confirmed than identified as RE-derived miRNAs. In conclusion, many miRNAs have not yet been identified, most of which are derived from REs.

10.A the Study on Relationship Among Diphosphoglycerate (2.3-DPG), Arteriovenous Oxygen Content Difference (A-VO2) and Tissue O2 Delivery of the Patients with Chronic Lung Disease (CLD).

OBJECTIVES: The presence of chronic low oxygen delivery to the tissues (blood flow X Hb concentration X arterovenous oxygen content difference) in patients with chronic lung disease (CLD) leads to a aggravated dyspnea. Therefore, total oxygen delivery to the tissues is a reverse correlation with severity of dyspnea. So far, the treatment of patients with CLD has been limited to only the increase of ventilation and perfusion capacity. However, this study focused on the capacity of oxygen delivery to the tissues in patients with CLD. We measured blood oxygen parameters as one of the extended approaches. METHOD: Simultaneous arterial and venous blood samples from 15 patients with CLD at rest were evaluated for 2.3-DPG, CO-Hb (Carboxy-monohe-moglobin), Met-Hb (methomoglobin), ABGA (Arterial blood gas analysis), VBGA (Venous blood gas analysis) and Hb (Hemoglobin). Smokers and patients with other disease were excluded from the study in order to avoid the factors (2.3-DPG, CO-Hb etc.) which can shift the oxygen dissociation curve. There were 12 men and 3 women, ranging in age from 40 to 75 years. The disease onset varied from 3 years to more than 30 years. RESULTS: 1) As total oxygen delivery to the tissues decreased, Ao2(Arterial oxygen partial pressure) and A-VCb (Arteriovenous oxygen content difference) decreased, while 2.3-DPG increased. Total oxygen delivary to the tissues showed a significant correlation with more A-V and 2.3-DPG than AO2. 2) AO2 had a correlation with A-VO2 but AO2 did not clearly reflect A-VO2. 3) 2.3-DPG activity depended on more A-VO2 than AO2. 4) As A-VO2 decreased, there was more significant relationship among 2.3-DPG, A-VO2 and tissue oxygen delivery. 5) A-VO2 showed a significant reverse correlation with 2.3-DPG as well as Met-Hb % + CO-Hb %. 6) ACO2 (arterial carbon dioxide partial pressure) showed a correlation with 2.3-DPG as well as A-VO2. 7) Arterial PH did not correlate with 2.3-DPG as well as A-V. CONCLUSION: 1) AO2 in patient with CLD correlated with A-VO2 but AO2 did not clearly reflect A-V. 2) Total oxygen delivery (severity of dyspnea) showed a significant correlation with more A-VO2 and 2.3-DPG than AO2. 3) 2.3-DPG activity depended on more A-VO2 than AO2. 4) 2.3-DPG activity had a reverse correlation with A-V in spite that 2.3-DPG shifts the oxygen dissociation curve to the right, because 2.3-DPG shift is secondary change associated with the low oxygen delivery to the tissues, More extensive k prospective investigations are needed to clearly define correlation among A-VO2, 2.3-DPG and prognosis of patients with CLD.
