2.A Study on Knowledge about Myocardial Infarction in Korean Population.

The purpose of this study was to identify the degree knowledge of myocardial infarction(MI) among the general population and to provide a direction for developing public health education programs. The subjects for this study were 187 people who lived in Seoul or Wonju City, Korea. The data were collected by questionnaire from Nov. ito Dec.5, 1997. The questionnaire was developed by the researcher based on a literature review, It consisted of 34 items of which nine items were on .iology, ten items on preventive measures, ten items signs & symptoms, five items on pain area. Analysis the data was done using descriptive statistics, t-test and ANOVA with the SPSS PC program. The results of the study are as follows: 1. The scores for knowledge of etiology(5.94), preventive measures(7.13), signs & symptoms(5.33) and area of pain(1.62). The highest score for knowledge about MI was preventive measures. the lowest score was for area of pain. Almost half of the subjects were not aware that 'hyperglycemia'and 'fatigue'can cause a MI. Also more than half of the subjects were not aware that 'reduced high carbohydrated diet'can prevent ML So this study showed that the general population has a need for more information about the relationship of hyperglycemia to MI. Less than half of the subjects knew that 'alteration in sleep pattern(49.4%)'. 'nausea, vomiting. indigestion(36.4%)''pain in the area of radiation and not in the chest(24.2%)' are sign & symptom of MI. Dankook University slightly to the left of the sternum(733%) but were not aware that pain may also radiate to the shoulder or arm(74.8%), neck or teeth(90.6%) Also 35% of the subjects think there is no treatment for an MI after it has occurred. 2. In the relationship between social demographic characteristics and degree of knowledge, there were significant differences due to sex, education level, and marital status. 3. This study showed hat most subjects got their in formation about MI from T.V. and radio(62.7%), but the score for knowledge was not high. The following are suggested based on the above results 1) It is necessary to reinforce information about signs & symptoms, area of pain and treatment of MI in public education programs, because people who do not know that their symptoms represent a heart at tack will delay seeking care and medical attention, 2) Because there are significant differences between knowledge and demographic characteristic(sex. education level, marital status), it is necessary to develop effective health education programs to consider these characteristics. Also we need behavioral strategies to change lifestyle and prevent more people from entering the MI high risk group. 3) Patients are almost always frightened and fearful which can cause a fatal delay. Education should focus on the emotional reasons behind people's resistance to calling the emergency medical system. 4) Educational programs need to target the potential witnesses rather than the individuals who suffer cardiac symptoms, focusing on the independent role that family members should take in the face of cardiac symptoms. Families of high risk group members, particularly their spouses, should receive special education about handling changes in cardiac symptoms. 5) Further studies are needed to identify factors which can cause patients to delay seeking treatment and to investigate the adaptive coping strategies of MI patients & their families when they have signs & symptoms.

Malignant epithelial neoplasm usually occurs in postmenopausal women, with less than 10% of cases occurring in women under the age of 20. In patients below this age, the majority of ovarian tumors are of germ cell origin and malignant ovarian epithelial tumors are uncommon. We report US & CT imaging in a case of mucinous cystadenocarcinoma in a 17-year-old nullipara who complained of amenorrhea for 45 days and a palpable mass.

6.Optimization of Extraction Conditions and Quantitative Analysis of Isoquercitrin and Caffeic Acid from Aster scaber

To determine the optimum extraction conditions that give the highest yield of isoquercitrin and caffeic acid from Aster scaber, the effects of four extraction variables (solvent concentrations, extraction time, number of repeated extraction, and solvent volumes) on isoquercitrin and caffeic acid yield was examined via HPLC-UV. Our results showed that the highest extract and isoquercitrin yield were observed when A. scaber was extracted with 450 mL distilled water for 8 hr repeatedly for three times. In case of caffeic acid, the content was higher in the two repeated extracts. Also, content analysis of isoquercitrin in Aster species was performed in which A. fastigiatus, A. ageratoides, and A. scaber exhibited the highest isoquercitrin content at 6.39, 5.68, and 2.79 mg/g extract, respectively. In case of caffeic acid, the highest content of A. scaber and A. glehni was 0.64 and 0.56 mg/g extract, respectively. This study reports an optimized method for extraction of isoquercitrin and caffeic acid from A. scaber and evaluates potential sources of the compounds.

7.Trends of Nursing Science Inquiry in Doctoral Dissertations.

PURPOSE: The purpose of this study was to identify the theoretical characteristics and direction of inquiry in the discipline of nursing by analyzing doctoral dissertations. METHOD: The materials used in this study were 277 doctoral dissertations from five universities in Korea. The framework for the study was derived from Kim's(1993) alternative linkage among philosophy, theory, and method in nursing science. RESULT: Of the 277 dissertations it was found that there were 13 types of linkages out of a possible 54 types. Most of the dissertations (128 of 277) were done within the linkage of realism/etic/quantitative/explanatory knowledge type. Of the 218 dissertations within scientific realism, 42 were within relativism, and 17 within practicism. There were 134 dissertations of the explanatory knowledge type, 112 descriptive ones, and 31 prescriptive ones. Studies done within the etic quantitative methodology included 209 dissertations and within the emic perspective, 43 with qualitative methodology, and 7 with quantitative. CONCLUSION: The results show that it is necessary to develop more alternative linkages for nursing practice and this will lead to expanding nursing knowledge.

8.Transcriptional Regulation of the Xbr-1a/Xvent-2 Gene by BMP-4 Signaling during Xenopus Embryonic Development.

BMP-4 signaling is mediated through Smad proteins which may translocate to the nucleus to activate transcription. Little is known about how BMP-4 signaling regulates the transcription of its target genes, e.g., Xvent genes. Therefore, we isolated the genomic clone of a BMP-4 responsive homeobox gene, Xbr-1a/Xvent-2. This clone contains a promoter and three exons for the entire coding region. Using the primer extension, we identified the transcription initiation site corresponding to position -64 bp upstream to the ATG codon of the Xvent-2 gene. The promoter was linked to the luciferase reporter gene, and promoter activity determined by luciferase assay. The temporal promoter activity peaked between embryonic stages 13~17, in agreement with its temporal mRNA expression in the whole embryo. Through the serial deletion mutation, the upstream -235 bp of the promoter retains the full transcriptional activity, and is regulated by BMP-4 signaling. The present results suggest that the BMP-4 responsive element is located on the upstream 235 bp of the promoter.

BACKGROUNDS AND STUDY AIMS: Multiple biliary papillomatosis (MBP) is an uncommon disease. This study was undertaken to review cases with respect to describing clinical features and characteristical cholangiographic findings, and to define the role of cholangioscopy in the diagnosis and treatment of this disorder. PATIENATS AND METHODS: from 1990 to 1997, nine patients (mean age; 57.3 years, 6 men and 3 women) were diagnosed as having MBP arnong 5,361 cases of endoscopic retrograde cholangiography (ERC) conducted at our center. A retrospective analyses of the cholangiographic and cholangioscopic findings as well as clinieal features were performed.

10.Two Cases of Mucinous Ductal Ectasia of Pancreas with Normal Duodenoscopic Findings.

Fairly specific and important duodenoscopic findings for diagnosing mucinous ductal ectasia (MDE) of pancreas include a prominant ampulla, patulous ampullary orifice and extrusion of mucin through the ampulla. However, we have experienced two cases of mucinous ductal ectasia without these pathognomonic appearance of ampulla. Initially, two cases were admitted for evaluation of pancreatic multicystic lesions, and MDE was incidentally found. Their pancreatographic findings showed normal main duct and cystic dilatation confined to the branch ducts. On gross findings of surgical specimens, a duct dilatation with gelatinous mucin was localized at the uncinate process of the pancreas. Microscopically, the ductal epithelia containing mucin were lined with well-differentiated tall columnar cells showing focal hyperplastic change. We report these unusual cases of MDE with review of the literatures.
