2.Analysis on influencing factors of concurrent free anterolateral femoral skin flap repair in radical operation for oral cancer

Objective To explore the curative effect of oral cancer radical surgery plus free anterolateral thigh flap repair and analyze the influence factors of skin flap necrosis.Methods The data of 98 patients who underwent oral cancer radical surgery with concurrent free anterolateral thigh flap repair in our hospital from January 2010 to June 2015 were retrospectively analyzed.The complete survival rate of skin flaps and incidence of complications in all patients were statistically analyzed.The age,diabetes,infection in implanted skin flap area,smoking and mental status,etc.of patients with survived or necrotic skin flaps were compared between patients with survival flaps and patients with flap necrosis,and multivariate Logistic regression analysis was performed.Results Among the 98 cases,86 cases of flaps survived completely and the survival rate was 87.8%.Fifteen cases with the skin flap necrosis,the incidence of complications was 15.3%,including 7 cases of surgi-cal site infection and 5 cases of skin flap necrosis with massive hemorrhage.The donor site incisions of all patients were primary healing and the patients were satisfied with the postoperative appearance.Among the patients with flap necrosis,the incidence rates of ≥60 years old,with diabetes,infection in flap implanted area,smoking and poor mental state (41.7%,41.7%,58.3%,75.0%,83.3%)were higher than those corresponding proportions in the patients with with survival flaps(P <0.05).Multivariate Logistic regression analysis showed that age,diabe-tes,infection in implanted skin flap area,smoking and poor mental status were the influencing factors for flap necrosis.Conclusion Oral cancer radical surgery with concurrent free anterolateral thigh flap repair has good curative effects.The age,combined with diabetes,smoking,infec-tion in flap implanted area and poor mental state are the risk factors for flap necrosis in patients with oral cancer after flap repair,which should be given intervention to improve the survival rate of skin flap in clinic.
3.Impact of Cold Wave on Mortality of Cerebra-cardiovascular Diseases in Beijing:a Case-crossover Study

Objective To estimate the impact of cold wave on daily death of cerebra-cardiovascular diseases in Beijing by using case-crossover design. Methods The data of death counts was provided by Beijing Center for Disease Control and Prevention while the meteorological data was provided by Institute of Urban Meteorology,China Meteorological Administration (CMA),Beijing. The impact of six cold waves from Jan 1,1998 to Jun 30,2000 on daily death of cerebra-cardiovascular diseases were analyzed by using 1∶1 one-directional retrospective design and 1∶2 bi-directional symmetry control design. The cases were the people who dead during winter. Controls were the seventh day before or after the date of cases (or the fourteenth day). Results There were six cold waves in the investigation duration. The third cold wave in which minimum temperature declined obviously with decreasing humidity and increasing air pressure had significantly statistical significance in OR and the lag time of this cold wave was 0 day,the OR values were 1.500 (95%CI:1.032-2.181) for cardiovascular disease (CVD),respectively 1.913 (95%CI:1.066-3.432) for acute myocardial infarction,1.679 (95%CI:1.139-2.474) for cerebra-vascular disease(CBD) respectively. The other five cold waves do not have significant impact on cerebra-cardiovascular diseases. Conclusion Most cold waves studied in Beijing do not impact the daily death counts caused by cerebra-cardiovascular diseases. The cold wave with significant increased air pressure may increase the risk of death of cerebra-cardiovascular diseases without any lag time in Beijing.

8.The expressio nof microRNA-216a in breast cance r and the effect on autophagy

Objective To investigate the expression of microRNA -216a(miR -216a)in patients suf-fering with breast cancer ,and identify the function and mechanism of miR -216 a on autophagy .Methods The expression of miR-216 a in 30 tumor tissues and paired normal tissue of breast cancer were detected by qRT -PCR.Inhibiton of miR-216a in MCF-7 cell lines was done by transfection of miR -216a inhibitor ( AMO-216a).Cell viability was detected by MTT assay .The level of Beclin 1 was detected by western blot .Results The level of miR-216 a was significant elevated in tumor tissue .Cell viability was markedly decreased owing to inhibition of miR-216a in MCF-7 cells.The level of Beclin 1 was significantly increased by transfection of miR-216a inhibitor in MCF-7cells(P<0.05).Conclusion The level of miR-216a is increased in breast canc-er tissue, and it might at least in part promote breast cancer via downregulating Beclin 1 and affecting autophagy .
9.Application of problem-based learning in clinical anesthesiology demonstration for clinical un-dergraduates

Objective To explore the effect of applying problem-based learning(PBL)in clin-ical anesthesiology demonstration for clinical undergraduates. Methods Eighty-nine 2009 clinical undergraduates were divided into two groups:experimental group and control group. Students in ex-perimental group received PBL while those in control group received traditional teaching. The effect was evaluated according to the results of case examination and investigation table. The results were analyzed by X2 test. Results Most students (42/45) answered questions correctly in experimental group while only 30 of 44 students(30/44)did in control group(P<0.05). Concerning the results of investigation,PBL had more advantages than traditional teaching method in the following 5 aspects:self-studying ability(86.7%,39/45),clinical thinking ability(73.3%,33/45),information seeking ability (75.6%,34/45),expressing ability (82.2%,37/45),communicating between students (75.6%, 34/45)(P<0.05). Conclusions PBL has evident advantages in anesthesiology clinical demonstration. It can inspire students,cultivate abilities,enhance critical thinking and finally improve teaching effect.
10.Combined Pharmacokinetics-pharmacodynamics Study of Rabeprazole in Inhibition of Gastric Acid Secretion

Objective To investigate the pharmacokinetics ( PK ) and pharmacodynamics ( PD ) processes of rabeprazole in inhibiting gastric acid secretion with the combined PK-PD model. Methods A total of 10 healthy volunteers were given a intravenous infusion of 20 mg rabeprazole over a 30-min period. The concentration of rabeprazole in the plasma at different time points was detected by HPLC,and the PK parameters were calculated by DAS 2. 0 software. At the same time the intragastric pH was monitored over 24 hours to fit the PD parameters with indirect inhibition model. Results The main PK parameters,t1/2,Cmax,and AUC were(60. 5±17. 3)min,(1 299. 1±201. 0)ng·mL-1,and(106. 4±26. 0)mg·min·L-1, respectively.The corresponding PD parameters,Kin,Ke,IC50 and Imax were(8.200±3.362)h-1,(1.080±0.378)h-1,(0.286± 0. 129)mg·L-1 and(6. 93± 2. 15)pH,respectively. Conclusion The PK of rabeprazole in healthy volunteers conforms to one compartment model,and the PD fits the indirect response inhibition model. The equation can effectively establish the relationship between the blood drug concentration and the effect.
