1.Adrenal tuberculosis:contrast-enhanced CT features with clinical correlation

Objective To investigate the correlation between specific features on contrast-enhanced CT and its clinical findings of adrenal tuberculosis so as to improve the diagnostic accuracy of the disease. Methods Contrast-enhanced CT features in 30 patients with documented adrenal tuberculosis were retrospectively evaluated blindly for the features of location,size,morphology,attenuation and enhancement patterns on CT images,and compared with clinical and pathological materials.Results The common CT manifestations were as follows:enlargement of the adrenal glands in all 30 cases(bilateral 90%,mfilateral 10%)including mass-like enlargement in 13 cases and enlargement but the contours preserved in 17 cases, heterogeneity(28 cases,93.3%),calcification(17 cases,56.7%),and low attenuation in the center with peripheral enhancement(16 cases,53.3%)of the lesions.After antituberculosis chemotherapy, 5 cases of enlarged adrenal glands decreased in size or returned to normal size and configuration,with disappearance of the central low attenuation and new appearance of dot-like calcification in 2 cases.Cochran Armitage trend test showed there was an increasing tendency of calcification rate with clinical duration(X~2= 7.47,P
2.Extramedullary Plasmacytoma Involving the Bilateral Adrenal Glands on MR Imaging.

We report here on a 64-year-old woman with extramedullary plasmacytoma involving the bilateral adrenal glands. Primary adrenal extramedullary plasmacytoma is extremely rare and only three cases of extramedullary plasmacytoma in the unilateral adrenal gland have currently been reported on. This case is of interest in that the bilateral adrenals were involved. In this article, we present the MRI findings and we briefly review the relevant literature.

3.Status analysis on medical advertisements during 2010-2013 in Shenzhen

Objective To understand status and analyze the characteris-tics of illegal medical ads, through a study of Shenzhen medical advertis-ing provide references for further improving supervision and administra-tion of medical advertisements.Methods Rate and proportion were used to descriptive analysis the monitoring data in June 2010 to May 2013.Chi-square test is used to compare differences between different rates.Results Medical advertising monitoring system for three years has monitored medical ads for 253080 times.Among them, illegal adver-tisements up to 64371 times, the ratio are 25.44%.Ninety seven medical institutions and forty two media outlets issued medical advertisements.Annual monthly average publication rate of illegal medical ads as a whole showed a decreasing trend.There were statistically significant differences in illegal rates of the different medical institutions areas, natures and media types.Conclusion Medical advertising monitoring system plays a good supporting role in supervising medical advertisements.But we still need to strengthen the supervision of broadcast, print medical advertis-ing.Regulation of the Internet should be the future direction.
4.Analysis on influence factors of illegal rate of medical advertising in Shenzhen city

Objective To understand the impact factors about the illegal rate of medical advertising, and to provide references for medical adver-tising regulation and related policy making through a study on Shenzhen medical advertising.Methods Rate and proportion were used to make descriptive analysis on the monitoring data in 2011 and 2012.Chi -square test was used for single-factor analysis about illegal rate of medi-cal advertising.Binary Logistic regression was used for multiple factor analysis about illegal rate of medical advertising.Results Medical advertising monitoring system has monitored medical advertisement 202502 times in 2 years.Among them, illegal advertisement accounted for 42141 times, with the ratio of 20.81%.A total of 66 medical institutions issued medical advertisement.The factors of seasons, medical structure categories, regions, properties ( divided according to the ownership of assets) , properties ( divided according to business scope ) , levels of medical institutions, media categories, departments of medical advertising were associated with illegal rate of medical advertising.Conclusion The significant factors of illegal rate of medical advertising, such as print media, internal curative department, out -patient department, anorectic branch, Bao′an district should be attached importance and directional regulation should be strengthened to further improve medical advertising regulatory work.
5.Analyzing the status of pharmaceutical administration in medical institutions of Shenzhen city

Objective To understand the status of pharmaceutical ad-ministration in medical institutions of Shenzhen City , find the main prob-lems , and provide direction for further improvement of pharmacy adminis-tration.Methods We used the scale to assess pharmaceutical adminis-tration in medical institutions upon the spot.The mean and standard de-viation were used to describe the scores of pharmaceutical administration in medical institutions.The deduction items were counted to explore the existing problems in pharmaceutical administration.Results The total of 108 medical institutions were assessed.The average score was ( 83.25 ± 7.03 ).The following problems were founded.Institutional defects exis-ted in regulations for hospital management , pharmaceutical department lacked of professional personnel , staff business knowledge wasn ’ t enough skilled , the academic qualifications of pharmaceutical personnel were generally low , the monitoring efforts of key drugs and reagents were nee-ded to be increased , pharmacy working condition was poor , and so on.Conclusion Although the pharmacy management scores of part of the hospital were high , there were also some disadvantages.In the future , we should take the supervision as the center , intensify the management of medical pharmacy management , crack down on illegal behavior to pro-mote the pharmaceutical administration in medical institutions.
6.Screening low molecular weight protein biomarkers relevant to portal vein tumor thrombi in serum of patients with hepatocellular carcinoma.

OBJECTIVETo screen low molecular weight protein biomarkers relevant to portal vein tumor thrombi (PVTT) in serum of hepatocellular carcinoma (HCC) patients.
METHODSSerum samples were obtained from 12 healthy volunteers, 12 HCC patients without PVTT and 12 HCC patients with PVTT. Using two-dimensional gel electrophoresis (2-DE) in which the second dimension was 16% SDS-PAGE, serum protein images of the 3 groups were analyzed by ImageMaster software. The differential protein spots were further identified by MALDI-TOF MS/MS.
RESULTSComparing the results using 12.5% SDS-PAGE gel, there were more protein bands (between 3 x 10(3) and 20 x 10(3)) and low molecular weight (MW) protein spots (less than 20 x 10(3)) were clearly shown in the 16% SDS-PAGE gel. Fifteen differential protein spots representing 5 proteins were found in the 3 groups by inter-class comparison and they were then identified. Compared with those in the healthy group, apolipoprotein A-I, lipoprotein CIII, transthyretin and DNA topoisomerase II were all down regulated in HCC groups and haptoglobin-2 was over expressed. All 5 proteins decreased more in the PVTT group than in the non-PVTT group.
CONCLUSIONThe expression of low MW serum protein obviously changes in the beginning and in the progressive stage of HCC, and differentially expressed low MW proteins might be potential biomarkers in an early prognostic prediction and surveillance in the treatment for HCC and PVTT.

7.Study on the efficacy of hepatitis B virus vaccination and its influencing factors among children in rural area of Jiangsu province.

OBJECTIVETo evaluate the efficacy of hepatitis B viruse (HBV) vaccination and its influencing factors among children in rural area of Jiangsu province.
METHODSTwenty-five hundred and twenty-two children born after 1998 in rural area were selected as the study population using multistage cluster sampling method. HBsAg and anti-HBs were detected by enzyme linked immunoassay (ELISA) and radio-immunoassay (RIA), respectively. Anti-HBs negative children were boosted using different hepatitis B vaccines and the efficacy was compared. Factors causing HBV infection in HBsAg positive children were also investigated.
RESULTSHBsAg positive rates in 1-7 year olds were 0.28%-1.28%, and the anti-HBs positive rates decreased from 76.7% to 45.5%. The HBsAg positive rate in children not timely vaccinated was significantly higher than those with HBV vaccine injection within 24 hours after birth (1.4% vs. 0.5%, P = 0.031). More than 90% of the anti-HBs negative children had protective level of anti-HBs after boosted with HBV vaccine.
CONCLUSIONHBsAg positive rate in children born after 1998 in rural area of Jiangsu province decreased significantly, with an average of 0.8%. The reason for HBsAg carriage in children might be attributed to mother-to-infant transmission or not timely HBV vaccination.

8.Application of PHILOS plate through mini-open deltoid-splitting approach for the treatment of proximal humeral fractures.

OBJECTIVETo study clinical effects of PHILOS (proximal humeral internal locking system) plates through mini-open deltoid-splitting approach for the treatment of proximal humeral fractures.
METHODSFrom March 2006 to August 2010, 22 patients with proximal humeral fractures were treated with PHILOS plates through mini-open deltoid-splitting approach. According to Neer classification, 6 cases were type II, 15 cases were type III and 1 case was type IV. Through the anterolateral approach to the shoulder, anterolateral vertical incision of 4 cm length was perforrmed from 1 cm under acromion, and separated deltoideus muscle vertically to touch the fracture,reduced the end of fracture directly and indirectly. PHILOS plate was inserted downward into anterolateral surface of humerus through deltoideus muscle, the distal end and proximal end was fixed by locking screws. The Neer score for shoulder function was evaluated within 1 year after operation.
RESULTSThe operative time ranged from 30 to 70 minutes with an average of 45 minutes. No blood transfusion was required during the operation, and all incisions healed in stage I. All the patients were followed up, and the duration ranged from 6 to 18 months with a mean time of 12.5 months. All the fractures healed up perfectly, and the union time ranged from 6 to 12 weeks. According to Neer criteria for shoulder joint function, 10 patients got an excellent result, 9 good, 2 poor and 1 bad. There were no complications such as axillary nerve injuries, screw loosening, steel plate breakage, dislocation of shoulder joint and necrosis of humeral bone.
CONCLUSIONPHILOS plate through mini-open deltoid-splittin approach for the treatment of proximal humeral fractures has follow advantages: simple recover,minor-injuries and small tissue invasion, which is an ideal method to treat proximal humeral fractures.

9.Expression and identification of truncated Nsp7 protein of North American and Europe genotype porcine reproductive and respiratory syndrome virus.

Porcine reproductive and respiratory syndrome virus (PRRSV) non-structural protein 7 (Nsp7) plays an important role in the induction of host humoral immune response and could serve as an ideal antigen for serological genotyping assay for PRRSV based on the significant difference in immunoreactivities of North American (NA) and European (EU) PRRSV Nsp7. In this study, Nsp7 of NA and EU PRRSVwas separately expressed and purified using prokaryotic expression system. The purified recombinant Nsp7 proteins reacted with serum antibodies against corresponding genotype PRRSV in Western blotting. However, nonspecific reaction of whole recombinant Nsp7 with antibodies against another genotype PRRSV was observed, indicating that whole NA PRRSV Nsp7 and EU PRRSV Nsp7 have similar antigenic epitopes and recombinant proteins could not be used for genotyping of antibodies against PRRSV. Based on the analysis of similar antigenic epitopes at the hydrophilic region of NA PRRSV Nsp7 and EU PRRSV Nsp7 by bioinformatics assessment, partial Nsp7 gene region deleted sequences encoding similar antigenic epitopes was constructed by fusion PCR. The recombinant truncated Nsp7 (NA-deltaNsp7 and EU-deltaNsp7, about 43 kDa) was expressed and the molecular weight was about 43 kDa. The results of Western blotting showed that NA-deltaNSP7 and EU-deltaNSP7 could be specifically recognized by positive serum to NA or EU PRRSV individually and nonspecific reaction was eliminated. This study provided a basis for further development of serological genotyping assay for North American and European genotype PRRSV infection.

10.Clinical value of genome-wide chromosome microarray technique in diagnosis of fetal cerebral ventriculomegaly.

OBJECTIVETo investigate the clinical value of gnome-wide chromosome microarray (CMA) technique in genetic etiological diagnosis of fetal cerebral ventriculomegaly.
METHODSA retrospective analysis was conducted in 109 women with singleton pregnancy, who were admitted in Nanfang Hospital with the diagnosis of cerebral ventriculomegaly in the fetuses by ultrasound between January, 2014 and December, 2016. Routine karyotype analysis and chromosome microarray analysis were performed to identify the chromosomal abnormalities in the fetuses.
RESULTSKaryotype analysis detected chromosomal abnormalities at a rate of 12.84% in these fetuses, significantly lower than the rate of 26.60% with CMA technique (P=0.004); the combined detection rate of the two techniques was 28.44%. In 17 cases, karyotype analysis yielded normal results while CMA microarray showed abnormalities with an extra abnormal detection rate of 15.60%. Among the 17 fetuses with chromosomal abnormalities, 6 had micro-deletion, 9 had micro-duplication, 1 had both micro-deletion and micro-duplication, and 1 had heterozygous loss of single parent diploid.
CONCLUSIONCMA technique can be used to detect abnormal chromosomal copy numbers in fetuses with cerebral ventriculomegaly to increase the detection rate of chromosomal abnormalities and facilitate prenatal consultation and prognostic evaluation.
