1.Ebselen can attenuate the ischemia-reperfusion injury of the rat lungs from non-heart -beating donors

Objective To evaluate the effect of Ebselen on the ischemia-reperfusion injury (IRI) in rat lungs from non-heart-beating donors (NHBD).Methods Forty Sprague-Dawley rats were paired randomly divided into two groups:group Ⅰ,NHBD with 30 min of warm ischemia time (WIT); group Ⅱ,NHBD with 30 min of WIT and administration of Ebselen.The donor lungs remained ventilated at the room temperature for 30 min after asystolia and then flushed with LPD solution.The recipient rats underwent left lung transplantation.The recipients of group Ⅱ were administered with Ebselen (500 mg/kg body weight) one h before transplantation.Results All the recipients survived during the observation period.In the group Ⅱ,the MDA of the pulmonary tissue was 0.631 ± 0.23 nrmol/mg protein,and the polymorphonuclear neutrophils and the total protein of the bronchoalveolar lavage fluid were (78.4 ± 35.2) × 107/L and (0.41 ± 0.12) mg/ml respectively.The MPO was (25.09 ± 1.19) % and W/D was 0.359 ± 0.017.There was significant difference between group Ⅱ and group Ⅰ (all P< 0.05).Conclusion The administration of ebselen is an effective treatment to attenuate the acute injury resulted from the ischermia-reperfusion in the rat lungs from non-heart-beating donors.
2.Microsurgery on craniocervical junction meningiomas

Objective To report the microsurgical outeome of the meningiomas located in the eranio- cervical junction.Methods Seven eases of meningiomas arising from the craniocervical junction operated with microsurgical technique by use of the posterior approaehes including the suboccipital approach,the lower lateral suhoecipital approach and for lateral approach.Reviewed the related ariieales and were analysed relative to their approach selection and outcomes were analysed.Results The tumors ranged in size from 0.8 to 4.8 cm and the tumors location with 2 cases posterior,3 cases anterior and 2 cases lateral to the brain stem.3 in 7 cases were operated by use of the suboccipital approach,2 in 7 cases with the lower lateral suboccipital ap- proach and 2 in 7 cases with farlateral approach.Total excisions were got in 6 of 7 cases,4 in 6 cases with Simpson gradeⅠand 2 in 6 cases withⅡ.The subtotal excision was got in 1 of 7 cass.Follow-up with 6 ca- ses between 6 to 36 months,the occupied symptoms in 6 cases improved good.No remnant tumor and recur- rent were found in the MRI follow-up.Conclusion By using of the proper posterior approaches,the micro- surgical outonmes of the meningiomas arising from craniocervical junction are good.

4.Comparison of adjustable and set-pressure valves in the management of communicating hydrocephalus in children

Objective To analyze the outcomes of adjustable and set-pressure valves in the management of communicating hydrocephalus in children. Methods A retrospective study reviewing the medical records of 66 children with communicating hydrocephalus who received ventriculoperitoneal shunt at our center from January 2003 to December 2009 was performed. All the patients were divided into 2 groups: Codman Hakim adjustable valve group (n=27) and Medtronic set-pressure valve group (n=39). Rate of appearing complication and shunt survival time were analyzed between the 2 groups.Results The rate of shunt infection, proximal or distal obstruction, subdural hematoma and subdural hygroma showed no statistical significance between patients of the adjustable and set-pressure valve groups (P=1.000, P=0.727, P=0.455, P=0.691, respectively). The average shunt survival time of children in the adjustable valve group was 52.551 months (38.169-66.852, 95% CI) and that of children in the set-pressure valve group was 42.327 (31.636-53.019, 95%CI); no statistical significance on the average shunt survival time between these 2 groups was noted (x2=0.763, P=0.382).Conclusion The adjustable valve is not superior to set-pressure valve in the treatment of communicating hydrocephalus in children.
5.Diagnostic value of plasma concentration of pro-brain natriuretic peptide in congestive heart failure in pediatric patients with ventricular septal defects.

OBJECTIVEThe value of plasma brain natriuretic peptide (BNP) and N-terminal proBNP (NT-proBNP) can reflect cardiac function and therefore can be used for diagnosing congestive heart failure (CHF) and evaluating cardiac function. There are few reports, however, on the value of BNP and NT-proBNP in pediatric cases of congenital heart defect. The aim of this study was to assess the value of plasma NT-proBNP in the diagnosis of CHF and evaluation of cardiac function in pediatric patients with ventricular septal defect (VSD).
METHODSFifty-one patients with VSD aged from 2 months to 2 years old (mean 7.9 months) were enrolled. According to the modified Ross Score, the patients were divided into three groups, no CHF group (20 patients), mild CHF group (18 patients) and moderate to severe CHF group (13 patients). Fifteen age-matched normal children were used as controls. Plasma NT-proBNP was measured using enzyme immunoassay. All patients had complete echocardiographic study, including measurement of left ventricular end diastolic volume index (LVEDVI), left ventricular end systolic wall stress (LVSEWS), heart rate corrected mean velocity of circumferential fiber shortening (mVcFc), left ventricular ejection fraction (LVEF), left ventricular fractional shortening (LVFS), and contractility index (Con). The correlation between plasma NT-proBNP level and modified Ross Score and echocardiographic cardiac functional indexes was determined. The sensitivity, specificity and ROC curve of plasma NT-proBNP for diagnosing CHF was studied.
RESULTSPlasma NT-proBNP was positively correlated with modified Ross Score (r = 0.75, P < 0.01). Plasma NT-proBNP concentration in moderate to severe CHF group (2061 +/- 908) fmol/ml was significantly higher than that of mild CHF group (810 +/- 335) fmol/ml, and Plasma NT-proBNP concentration in mild CHF group was higher than that in no CHF group (309 +/- 68) fmol/ml. 97.14% of normal controls and subjects in no CHF group had their plasma NT-proBNP below 400 fmol/ml. 83.3% of children in mild CHF group had their plasma NT-proBNP between (400-1400) fmol/ml while in moderate and severe CHF group 84.6% of children had their plasma NT-proBNP beyond 1400 fmol/ml. Plasma NT-proBNP was also positively correlated with LVEDVI and LVSEWS. There was no correlation among mVcFc, LVEF, LVFS, Con and plasma NT-proBNP concentration. Using plasma NT-proBNP concentration > or = 400 fmol/ml as cut-point for diagnosing CHF, the sensitivity was 89.3%, the specificity was 91.2%, and the area under the ROC curve was 0.944.
CONCLUSIONSPlasma NT-proBNP level could be used to assess cardiac function and diagnose CHF in pediatric patients with VSD.

6.Treatment and prognosis of posterior fossa tumor combined with hydrocephalus in children: an analysis of influencing factors

Objective To explore whether preoperative treatment of hydrocephalus could change the occurrence of postoperative hydrocephalus after tumor resection in children with posterior fossa tumor, and analyze the risk factors of postoperative hydrocephalus. Methods Sixty-three children with posterior fossa tumors accompanied with hydrocephalus, admitted to our hospital from January 2003 to December 2009, were chosen; conservative treatment of preoperative hydrocephalus was performed in 43 patients and surgery was performed in the other 20 patients. The clinical data of these patients were retrospectively analyzed and the occurrence of postoperative hydrocephalus in patients performed conservative treatment and tumor resection was compared. Gender, age, Evans index, location of the tumor, extent of the tumor resection, pathology of the tumor were analyzed as risk factors using logistic regression. Results Occurrence of hydrocephalus after tumor resection showed no statistical differences between patients accepted conservative treatment (14/43, 32.6%) and patients received surgical treatment ([8/20, 40.0%], P=0.564). Evans index, location of the tumor and extent of resection were related to hydrocephalus relapse after tumor resection(P=0.029, P=0.036, P=0.038). Conclusion Preoperative treatment of hydrocephalus could not change the occurrence of hydrocephalus after tumor resection. Evans index, location of the tumor and extent of tumor resection are all risk factors of postoperative hydrocephalus.

8.Transposition of orbital fat and orbicularis muscle flap over the orbital rim for correction of lower eyelid pouches complicated with lacrimal groove deformity.

OBJECTIVETo introduce the experience in the treatment of lower eyelid pouches orbital rim.
METHODSAn incision was made along the margin of lower eyelid and dissection was performed under the orbicularis muscle to expose the orbital septum and periosteum of lower orbital rim. The fat released from orbital septum was transposed just below the lower orbital rim and fixed on the periosteum. If lacrimal groove deformity was not corrected completely, the musculocutaneous flap, which may be excised beside the incision, was kept to correct the deformities further with only the muscle portion.
RESULTS72 cases with lower eyelid pouches complicated with lacrimal groove deformities were treated with transposition of orbital fat and orbicularis muscular flaps. Satisfactory results were achieved in all the patients after a follow-up period of 3-6 months.
CONCLUSIONIt is an effective and feasible technique to correct lacrimal groove deformities with transposition of orbital fat and orbicularis muscular flaps.

9.Dendrobium officinale cliff epiphytic cultivation method.

To solve the issues of costly planting of facility cultivation method and inferior efficacy than wild herbs of Dendrobium officinale, the cliff epiphytic cultivation method was studied. To research the growth, agronomic traits, yield, polysaccharide and alcohol-soluble extract contents were measured on the D. officinale from different water regulation and cliff slope gradients treatments. The results showed that D. officinale epiphytic at 85 degrees-90 degrees cliff and sprayed water 1-2 h x d(-1) at the growing season can get better growth and obtain high yield, and the morphology has no different from wild cliff D. officinale, even in the environments without shade. The contents of polysaccharide and alcohol-soluble extract are closely related to the physiological ages, but significantly higher than the facility cultivation. It is possible that environmental stresses benefit the accumulation of polysaccharides, alcohol-soluble extract and other efficient ingredients.

10.A preliminary study on the fabrication of silk fibroin/chitosan/nano-hydroxyapatite osteochondral scaffolds with a gradient pore size structure

Objective To explore the feasibility and methods of preparing silk fibroin(SF)/ chitosan(CS)/nano hydroxyapatite(nHA)composite osteochondral scaffolds with a gradient pore size structure.Methods We prepared an SF solution,a CS solution and an nHA suspension,all with a 2% concentration,and mixed them with equal proportions.The mixture was used to prepare SF/CS/nHA composite osteochondral scaffolds with a pore size gradient through a centrifugal freeze drying and chemical cross linking method.The porosity,hot water loss rate,water swelling rate and mechanical properties of the scaffold were measured,and the dissolution curve and stress strain curve were drawn.The internal structure and morphology of the scaffold were observed by scanning electron microscopy(SEM)and the sizes of pores in the scaffold were measured.Results The porosity of the scaffold was(91.30± 3.35)%;The five week hot water loss rate was(16.57± 3.18)%;And the water swelling rate was (3218.53 ± 84.37)%.Mechanical test results demonstrated a good compression performance of the scaffold.SEM showed that the internal pores of the scaffolds were honeycomb structured with communicating passages;The density of pores gradually increased with decreasing pore sizes from the top to the bottom(pore sizes at four different levels:(141.11± 11.85)μm,(119.94± 9.05)μm,(93.10 ± 14.98) μm,and (79.95 ± 8.65)μm,respectively,F =22.973,P =0.000).Scaffold cytotoxicity test results indicated no significant difference between A values of the extract group and of the negative control group at any time point(t24 h =0.520,P =0.610;t48 h =0.665,P =0.515;t72 h =0.439,P =0.666),and all RGR values were greater than 100%.Conclusions SF/CS/nHA composite osteochondral scaffolds with a gradient pore size structure can be prepared with a centrifugal-freeze drying and chemical cross-linking method.Scaffolds prepared this way have a three-dimensional structure,a gradient pore size structure,high porosity,strong water absorption,suitable degradation rates and good compressive resistance.Besides,the good cell compatibility and low cytotoxicity of the scaffolds satisfy the requirements for osteochondral tissue engineering materials.
