OBJECTIVE:To draw a comparison between 6% Hetastarch(H)and Succinylated Gelatin(G)in fluid expansion and their effects on blood coagulation activity.METHODS:A total of 60 surgery patients who needed volume expansion with colloid fluid were randomized to H group and G group.According to the design,the same dose of H and G were infused into the patients.Before and after infusion,the vital signs were monitored and the venous blood samples were taken for the determination of indexes of coagulation,fibrinolysis and electrolyte.RESULTS:There were no significant differences in the vital signs between two groups.After volume expansion,blood platelets counts in two groups were significantly decreased,much as in H group.And the APTT in H group had been significantly prolonged.In group G,only FIB decreased significantly.After volume expansion,free Ca2+ and total Ca2+ were decreased in both groups.Low level of K+ in group G was noted following diluti-on,and Cl-in group H increased significantly.CONCLUSION:Both group showed good volume expansion effects and stable fi-brinolytic function.Hyperchloremia should be alert in group H,and if necessary,Ca2+ should be promptly added in two groups.

Objective To explore the level of serum transforming growth factor-β1 (TGF-β1) in patients with prehypertension and impaired fasting blood glucose.Methods A total of 180 healthy subjects with matched age and sex was divided into 3 groups:group A (blood pressure ＜ 120/80 mmHg and fasting blood glucose ＜6.1 mmoVL),group B (prehypertension and fasting blood glucose ＜6.1 mmol/L),group C (prehypertension and 6.1 mmol/L≤ fasting blood glucose ≤7.0 mmol/L).There were 60 subjects in every group.Serum TGF-β1 was measured by Enzyme-Linked Immunosobent Assay (ELISA),fasting blood glucose,triglyceride,total cholesterol,low-density lipoprotein cholesterol ( LDL-C),high-density lipoprotein cholesterol ( HDL-C),Uric Acid ( UA),body mass index (BMI) were determined.Results The level of serum TGF-β1 in group B and C was higher in that in group A( t =2.071,and 2.147,P ＜0.05 ).The level of serum TGF-β1 was higher in group C than group B,but no significant difference was observed ( P ＞0.05).The pearson correlation analysis showed there was a significant correlation between TGF-β1 and systolic blood pressure ( r =0.279,P =0.047).Conclusions The level of serum TGF-β1 was higher in patients with prehypertention which may mediate the target organ damage caused by the prehypertention.

Objective: To study the antifatigue effect of tea containing Acanthopanax senticosus.Methods: Forty eight mice were randomized into 4 groups:three groups of tea containing Acanthopanax senticosus at doses of 50, 75, 100 ml/(kg?d), and the saline group.After 4 w, muscle and liver glycogen, serum LDH activity, BUN and blood LAC were measured.Results: Three tea groups showed increase of muscle and liver glycogen,and serum LDH activity, and decrease of serum BUN and LAC in swimming mice.The 75-100 ml/(kg?d) group was the best.Conclusion: Tea containing Acanthopanax senticosus has antifatigue effect.

BACKGROUND:Conventional surgical repair can cause large traumas in patients with knee injuries, and patients often recover slowly after implant fixation, most of whom can appear to have poor recovery of knee function. OBJECTIVE: To explore the folow-up effect of arthroscopic double-steel wire clip fixation on tibial eminence avulsion fracture of anterior cruciate ligament. METHODS: A retrospective analysis was performed on the clinical data of 23 patients with tibial eminence avulsion fractures, who were given arthroscopic double-steel wire clip fixation. The patients were folowed up for 1-6 months. Short- and middle-term therapeutic effect as wel as IKDC and Lysholm scores before and after treatment were observed and analyzed. RESULTS AND CONCLUSION:The operation time was 35-65 minutes, and no complications, such as blood, nerve and anterior cruciate ligament injuries occurred. Moreover, no infection and other poor biocompatible reactions occurred after internation fixation. Al patients were folowed up for 1-6 months. The excelent and good rate was 87% at 1 month after treatment and 96% at 6 months after treatment. Al the patients had improved IKDC score and Lysholm score after treatment (P < 0.05), indicating that the knee function of patients was improved significantly.

Objective To explore the community elderly depression effect on quality of life and provide the basis for the elderly mental health intervention. Methods Adopting stratified random sampling method, using the SF-12,CES-D and psychological capital questiennaire of elder to survey among 500 community elderly people and established the structural equation model. Results The age, degree of education, income situation, depression and the old psychological capital of the community elderly can be used to predict the quality of life, can explain 88.6%variance of life quality(P<0.05);depression on quality of life has a direct effect of-0.21(P < 0.05); depression through psychological capital indirect effect on quality of life for-0.64(P<0.05). Conclusions The elderly depression through psychological capital intermediary effect on the quality of life.

OBJECTIVE To study the current status of the AmpC enzyme and ?-lactamases genes for multi-resistant Acinetobacter baumannii isolates in clinic in Nanjing.METHODS The samples of 20 multi-resistant A.baumannii isolates were collected from Jul 2007 to Oct 2007 from the patients in hospitals of Nanjing.To determine the sensitivity to the 11 antibacterials by using a broth induction method,and two kinds of AmpC enzyme genes and eighteen kinds of ?-lactamases genes of multi-resistant A.baumannii analyzed by polymerase chain reaction(PCR).RESULTS In all of the 20 A.baumannii isolates,the AmpC genes of chromosome type were found in 14 isolates(70.0%),there were the AmpC genes of plasmid type in 1 isolate(5.0%),the TEM genes in 12 isolates(60.0%),the SHV genes in 1 isolate(5.0%),CTX-M-1 genes in 2 isolates(10.0%),OXA-23 genes in 1 isolate(5.0%),and OXA-24 genes in 1 isolate(5.0%),respectively.The OXA-24 genes from No.9 isolate was the type of OXA-72 after DNA sequencing.The amino acid sequence translated by AmpC genes of chromosome type from No.16 and No.17 isolates was the new subtype.CONCLUSIONS The A.baumannii has not only TEM,VEB,GES,IMP,VIM,OXA-10 and OXA-23 genes,but also the type of chromosome and plasmid of AmpC,SHV,CTX-M-1 and OXA-24 genes in Nanjing.It is the first time that activity of AmpC enzyme and the genotype of chromosome and plasmid of AmpC enzyme for matching are tested at the same time.

Hydrolytic amino acids were extracted by acid hydrolysis method, then derivatized with phenyl isothiocyanate (PITC). And the samples were analysed by HPLC on an Ultimate Prime C18 (4.6 mm x 250 mm, 5 microm) column with gradient elution of 0.1 mol x L(-1) sodium acetate buffer solution (adjusted to pH 6. 5)-acetonitrile (93:7) (A) and acetonitrile-water (8:2) (B) at a flow rate of 1.0 mL x min(-1). Column temperature was 40 degrees C and the detected wavelength was 254 nm. Amino acids derivative solution remained stable in 36 hours. The response was linear for 16 amino acids with a correlation coefficient r > 0.999 5. The average recoveries were 98.01% -101.8%. The method is reliable with good accuracy and repeatability, which is useful for the determination of amino acids in Galli Gigerii Endothelium Corneum.

Objective To introduce the technique of metal-on-metal hip resurfacing arthroplasty,evaluate its preliminary clinical results and review the factors which affect the results.Methods From January 2006 to October 2010,104 patients (116 hips) with osteonecrosis of the femoral head,hip osteoarthritis,hip dyplasia,or ankylosing spondylitis were treated with the total hip resurfacing arthroplasty.Among them,59 patients (66 hips) were male,and 45 patients (50 hips) were female,with an average age of 39.7 years (range,16-67 years).Standard operation technique was performed for all patients with Conserve Plus prosthesis.Each patient was followed up at 1 month,3 months,6 months,1 year postoperatively.Results Three patients (4 hips) were lost,so 101 patients (112 hips) were followed up for an average period of 33 months (range,9-66 months).The average Harris hip score was improved significantly from 37.6±6.3 preoperatively to 91.3±5.3 at final follow-up; 104 hips were excellent,7 hips were good,1 hip was poor,and the excellent and good rate was 99.1％.At final follow-up,the average abduction angle of the acetabutar cup was 42.7°(range,36°-55°); the average femoral prothesis stem-shaft angle was 138.5° (range,132°-146°).Two patients presented with heterotopic ossification (Brook Ⅲ and Brook Ⅱ ).One patient presented with prothesis dislocation,which healed after being treated with manual reduction.There was no femoral neck fracture,infection,or prosthesis loosening.Conclusion With strict patient selection criteria and optimized technique,the short-term results of metal-on-metal hip resurfacing arthroplasty are satisfactory.

Objective To evaluate the safety and effectiveness of partial cystectomy combined with chemotherapy in treating localized muscle invasive bladder cancer.Methods The patients admitted to localized muscle invasive bladder cancer from October 2011 to December 2015 were screened according to the strict inclusion criteria,and the final 53 patients were enrolled in the study,37 males and 16 females,aged 36 to 85 (63.72 ± 13.31)years.Tumor diameter 1.5-4.0 (2.52 ±0.56)cm,the number of≤ 3 and the edge of the tumor from the ureteral opening ＞ 2 cm.All pathological types are urothelial carcinoma.All patients were examined by MRI clinical stage T2a-T2b period.A total of 53 patients with muscle invasive bladder cancer who refuse or older,merge the heavier medical disease,difficult to tolerate radical cystectomy.Using partial cystectomy,intraoperative electrosurgical excision to deep muscle,parallel to the tumor bed frozen to confirm negative margins,postoperatively systemic chemotherapy of gemcitabine combined cisplatin and bladder perfusion chemotherapy of hydroxycamptothecin.Results Patients were successfully opcrated,and no margin positive or incision planting cases.The operating time is 55-110 min and average (79.11 ± 17.25)min.There was no serious complication in operation and postoperative,pathological diagnosis was invasive epithelial cell carcinoma of the urinary tract,pathology classification:low level 41 cases,and high level 12 cases.Follow up of 12 to 62 months,an average of (27.11 ± 12.22)months,21 cases of recurrence,12 patients had superficial recurrence in the bladder who was treated successfully by transurethral resection of bladder tumor,5 cases of multiple myometrial invasion who was treated by radical cystectomy (3 cases died of bladder cancer after the surgery in 8,12,and 18 months later),4 cases of myometrial invasion with recurrence of pelvic lymph node metastasis or even distant metastasis to give up treatment (all died within six months of bladder cancer).The first tumor recurrence time 3-18 months,11 months and the median time.Conclusions Our data indicate that partial cystectomy combined with chemotherapy is a safe and effective in treating localized muscle invasive bladder cancer,with less trauma,shorter operative time and higher safety,so as to provide an alternative treatment for patients with advanced age,high risk or who can not be implemented transurethral resection of bladder tumor or who refused to undergo radical cystectomy.

Objective To evaluate the molluscicidal effect and the using value of qiangluocide powder by dusting in the fields. Methods The two different ways of dusting with qiangluocide and spraying with luomiecide for controlling snails were compared in the fields, and the cost-effectiveness was analysed. Results The corrected mortalities and the reduced rates for density of snails were 88.48%, 96.26%, 96.96%, 95.67%, and 87.58%, 95.40%, 96.14%, 96.67%, respectively, on the marshland in Longtan, Nanjing, 73. 65% , 98. 31% , 99. 22% , 99. 26% and 69. 30% , 98.14%, 98.84%, 98. 60% , respectively, on the marshland in Yizheng, Yangzhou, on 1 d, 3d, 7 d, 15 d after dusting with qiangliocide power; and those on 1d, 3d, 7d, 15d after the spraying with luomiecide were 69.15%, 88.01%, 86.46%, 85.56% and 75.27%, 84.99%, 87.50%, 87.96% on the marshland in Longtan, Nanjing, 18.70%, 63.11%, 1.25%, 12.60% and 34. 88% , 89. 02% , 45.37% , 46. 59% , respectively, on the marshland in Yizheng, Yangzhou. The st of dusting with qiangluocide powder was 0. 20 yuan/m2, and that of spraying with luomiecide as 0. 21 yuan/m2 for snail control in the fields. The costs of reduced one percentage for the cor rected mortalities and the density of snail were 6. 27-10. 07 yuan and 6. 21-10. 14 yuan, respectively, on 15 d after dusting with qiangluocide power and those on 15 d after spraying with luomiecide were 7. 36-83. 33 yuan and 7. 16-22. 54 yuan , respectively. Conclusion The dusting with qiangluocide powder is a promising new formulation, which is suitable for the areas without water resource and possessed of high effectiveness against snails. The method ofdusting with qian-gluocide powder for controlling snails is simple, convenient, cheap and easy to be carried out for the inspection and quality control.
